The purpose of this study was to investigate the effect of anxiety on the postural stability of a variety of dizzy patients during upright standing. To address this issue, 54 patients complaining of
dizziness
were enrolled in this study. The degree of anxiety in patients was evaluated on the basis of a routine vestibular examination together with their
dizziness
handicap inventory (DHI) scores as well as the hospital anxiety and depression scale (HADS). The patients were divided into 3 groups. If there was no vestibular dysfunction, they were defined as psychogenic (PSY) (N=16). The remaining subjects were further divided on the basis of their HADS score. If the score of A (anxiety) was less than 5, they are defined as organic (ORG) (N=25), and the rest were defined as a combination of psychogenic and organic (PSY+ORG) (N=13). Posturographic measurements were performed in a quiet and stable standing position on a force platform, as one of the vestibular examinations. The total length, the area of body sway, and the ratio of maximum perturbation of antero-posterior axis (A/P ratio) were registered.
Spectrum
analyses of the left-right axis and antero-posterior axis were also performed by using the fast Fourier transform (FFT) method of body sway. We found a significant correlation between anxiety and postural instability in the antero-posterior axis in all subjects as a group and in either group PSY or PSY+ORG. However, no significant correlation was found in group ORG. Using power spectrum analysis (FFT), we identified 3 frequency components of postural sway: group A (0.02-0.21Hz), group B (0.22-2.01Hz), and group C (2.01-10Hz). Statistical significance of the data was examined by ANOVA. Group C reflected somatosensory inputs, and group A reflected vestibular inputs. The power of group C decreased in the high anxiety group, whereas the power of group A increased in the high anxiety group. These phenomena disappeared in the eyes-closed condition. Our study shows that the effect of visual input on vestibular and somatosensory input is affected by anxiety. In conclusion, our results indicate that anxiety affects the postural perturbation in the antero-posterior axis and that anxiety possibly affects the interactions of visual inputs with vestibular and somatosensory inputs in the maintenance of postural balance in patients complaining of
dizziness

The authors present a preliminary experience with ethyl-enevinylalcohol copolymer (

) for hemangioblastoma vessel embolization before surgical resection. The patient presented with neck pain,
dizziness
, blurred vision, vomiting, and loss of balance. Diagnostic imaging revealed a posterior fossa cystic mass with a nodular component. Angiography demonstrated a significant vascular blush with arteriovenous shunting that was characteristic of a hemangioblastoma. Tumor vessels originating off the left posterior inferior cerebellar artery were embolized before surgery using
Onyx
18 (ev3, Covidien Vascular Therapies, Mansfield, MA, USA). This resulted in complete obliteration of all tumor vessels, transforming a highly vascular tumor into an avascular mass. A safe and uneventful surgical resection was performed the next day.
Onyx
is a valuable embolic agent for preoperative hemangioblastoma vessel embolization. Because of its low viscosity,
Onyx
penetrates deeply into the tumor vasculature and allows complete obliteration of tumor vessels. Risks of the intervention have to be carefully weighed against the benefits. If preoperative embolization is indicated, the use of
Onyx
should be strongly considered.

Cervical hemangiopericytoma (HPC) is a rare tumor involving dura and bone. Because HPC shares some imaging features with meningioma, hemangioblastoma, schwannoma and solitary fibrous tumors; histology with appropriate immunohistochemistry is essential for its diagnosis and treatment. HPC is highly vascular and has a high rate of local recurrence following resection. Thus it can pose significant therapeutic challenges. To the best of our knowledge this is the 13th reported case of HPC of the cervical spine and the first case of cervical HPC treated using embolization with
Onyx
-18. In addition, a systematic review of the literature is presented describing previous experience with HPC of the cervical region. The case of a 61-year-old woman is reported who presented with a 7 month history of
dizziness
, mild ataxia and left-sided tinnitus when supine. MRI of the cervical spine demonstrated a gadolinium enhancing mass arising from the dorsal cervical dura, extending from the occiput to C2, with internal flow voids and extension into the C2 lamina and spinous process. Digital subtraction angiography demonstrated multiple corkscrew-shaped supplying vessels, marked hypervascularity, rapid arteriovenous shunting and delayed contrast washout. Super selective injection of
Onyx
-18 yielded approximately 90% embolization of the lesion, followed by subtotal resection and radiotherapy. Histology confirmed the presumptive diagnosis of HPC. Preoperative embolization with
Onyx
-18 should be considered for large HPC as it may decrease the risk of intraoperative hemorrhage. In this case, embolization with
Onyx
-18 did not preclude histopathologic diagnosis of HPC.

Superior sagittal sinus (SSS) dural arteriovenous fistulas (DAVF) are rare and present unique challenges to treatment. Complex, often bilateral, arterial supply and involvement of large volumes of eloquent cortical venous drainage may necessitate multimodality therapy. We report a case of a DAVF of the SSS in a patient who presented uniquely with increasing
dizziness
and disequilibrium who was treated with a single modality, endovascular embolization with ethyl vinyl alcohol co-polymer (
Onyx
, EV3, Irvine, CA). The patient underwent staged embolization in 2 sessions with no complications. An angiographic cure was achieved and the patient's symptoms were ameliorated. Single modality therapy with endovascular embolization of a SSS DAVF can be achieved. Careful attention to technique during embolization with
Onyx
is required, but complete obliteration is possible without the need for adjunctive surgical resection.

Distal cortical middle cerebral artery (MCA) aneurysm is a rare entity. Despite the challenging procedure, the role of endovascular treatment is emerging due to its safety and efficacy in obliterating the microaneurysm. We report a 25-year-old male, who presented with a history of

and headache for almost 2 weeks. Computed tomography scan showed a right front parietal subdural hematoma (SDH). We could not identify any underlying defining etiology of SDH neither head injury nor coagulopathy disorder. Therefore, diagnostic cerebral angiogram was performed, which showed a microaneurysm in the distal right MCA cortical branch. Hence, complete obliteration of this microaneurysm was performed using
Onyx
for endovascular embolization. Therefore, this case report demonstrates the efficacy of this modality in the treatment of microaneurysms with SDH.
