A large, open, multi-centre study was carried out in general practice to evaluate the effectiveness and tolerance of a combination of 10 mg pindolol plus 5 mg clopamide, in single tablet form, in the treatment of patients with essential hypertension. Computer analysis of the records of 8989 patients who completed the 8-weeks' study period showed that treatment with the combination product, in a dosage of 1 tablet daily in 83% of the patients, resulted in excellent blood pressure control in the majority (75%) of cases, irrespective of age or previous antihypertensive treatment, and was particularly effective in those with mild to moderate hypertension who had previously not received any therapy. Side-effects were generally not troublesome and only 8.3% of patients stopped treatment for this reason. The most commonly reported side-effects were

Monitoring observations made on 60 operators involved in pesticide application work in godowns and warehouses and 60 matched control workers are reported. Occupational exposure history and medical history are noted. Biochemical investigations, plasma and RBC cholinesterase estimations are included along with medical examination of the workers. Workers were found to be mostly exposed to Celphos, DDVP, Malathion, Pyrethrum, etc, and the use of protective devices were very limited. Cases of significant reduction in plasma and RBC cholinesterase activity were found. Frequency of symptoms like

, lachrymation, burning sensation in eyes, nausea and anorexia, etc, were much more in the exposed workers. No cases of clinical poisoning attributable to occupational exposure to pesticides were reported by the workers.

The authors report on four children with subclavian steal associated with coarctation or interrupted aortic arch. All children showed a stenotic origin of the left subclavian; in addition three of them had an extreme hypoplasia of the proximal segment of this artery. Two children had typical symptoms of insufficiency of the vertebral and basilar circulation (

, syncopal attacks, vision and hearing disorders) since the age of 7 and 8 years, respectively. The clinical findings, such as reduced pulses and blood pressure of the respective arm, may suggest the provisional diagnosis. The principles of congenital subclavian steal are discussed.

This paper focuses on recent developments in the areas of mode of action and clinical efficacy of mebendazole use since its introduction in 1974. Mebendazole blocks glucose uptake by adult intestinal-dwelling nematodes and cestodes and their tissue-dwelling larvae. The drug's poor absorption does not appear to affect clinical efficacy except in the treatment of systemic helminth infections. Mebendazole has been found to be ovicidal for the eggs of roundworm, hookworm, and whipworm with the disadvantages of cost and long duration of therapy; it is also considered the best drug for the treatment of trichuriasis. Among its nonapproved uses, it shows promise in the treatment of capillariasis and hydatid disease. Further investigation is needed to establish its role in the treatment of taeniasis, Hymenolepsis nana, Strongyloidiasis, Trichinosis, and Dipetalonema perstans. An epidemiological survey focused on farmers engaged in vinyl-house culture of strawberry was carried out among the inhabitants of a farming village. For avoiding residential and habitual factors, all residents (851 in number) over 12 years of age in the area were inquired about symptoms, personal and family histories. There were no marked difference in age and sex among three groups, i.e. farmers with and without vinyl-house culture and people with no relation to farming in this area. Incidence of one of the inquired symptoms were 54% and 45% in farmers with and without strawberry culture in vinyl-house respectively, and 34% in the non-farming people. This difference became more larger up to 83% in the strawberry farmers of allergic constitution. Symptoms found more in the farmers than the other were

, conjunctivitis, rhinorrea, sneeze, palpitation and neuralgia, and the last four symptoms were more marked among farmers in charge of strawberry culture. Incidence of the allergic symptom consisting of conjunctivitis, rhinorrea and sneeze was more marked among the persons of allergic constitution . In conclusion, health effects of strawberry culture in vinyl-house in farmers can be look upon from three aspects; overwork, a particular stance during the work and allergic responses to the dust in the vinyl-house.

Eleven nickel-hypersensitive patients with chronic, dyshidrotic hand eczema aggravated by oral challenge with 0.6-2.5 mg nickel were treated with 100 mg tetraethylthiuramdisulfide (Antabuse) two to four times daily for 4-10 weeks. Nine of the patients experienced a flare of the dermatitis shortly after initiation of the treatment. During the course of treatment the dermatitis of seven patients cleared, improvement was seen in two patients, and in two the dermatitis remained unchanged. Flare was seen in six patients when the treatment was discontinued. Seven patients experienced side effects such as fatigue,

A 1.5% nonachlazin solution containing substances which promote its absorption from the gastrointestinal tract increases coronary blood flow in cats 2--3 minutes after its administration into the stomach. After single administration this solution arrests a developing attack of angina pectoris in patients, improves the findings of spiroergometry, and in regular (3 times daily) medication prevents attacks of angina pectoris or makes them occurs less frequently in the very first 24 hours. The solution is most effective in patients with a high or moderate coronary reserve and affection of one or, in a lesser degree, of two coronary arteries. Ketoprofen and indomethacin were compared as specific therapies for primary dysmenorrhea in this study involving 23 primary dysmenorrheic women; the study was double-blind and cross-over. Each patient was given a code-numbered package of capsules of ketoprofen (50 mg) or indomethacin (25 mg); medication (1 capsule 3 times daily) was started 1 day before menstruation and was continued until cessation of dysmenorrheic symptoms (no longer than 4 days). Patient estimation of the effect of ketoprofen was ranked as good in 70% of cases, moderate in 18%, and nil in 12%. For indomethacin, the figures were 58, 31, and 10%, respectively. Initial dysmenorrhea score of 9.6 was reduced to 3.6 during ketoprofen therapy and to 4 with indomethacin (P.001). Ketoprofen alleviated pelvic pain in 84% of cases; indomethacin in 78%. Mean duration of pelvic pain was reduced to 5.1 hours from an initial period of 10.6 hours with ketoprofen and 5 hours with indomethacin (P.01); this statistic excluded cycles of total relief. Other symptoms relieved were similar with both drugs, including: lower back pain, vomiting, diarrhea, and
dizziness
(alleviated in 82-97%) and

, fatigue, and nervousness (alleviated in 40-67%). Blood loss was subjectively estimated to decrease in 42% and increase in 4% of ketoprofen-treated patients, whereas for indomethacin these figures were 36% and 7%, respectively. An investigation of a case of apparent mass psychogenic illness was undertaken in a midwestern electronics assembly plant. The plant employed 500 workers, of whom 80% were female. The illness outbreak involved a total of 90 female first shift workers who reported a variety of nonspecific symptoms such as
headache

, and lightheadedness in response to a strange odor in the workplace. Although environmental testing revealed some localized concentrations of a few airbone contaminants, no environmental toxins were discovered that could account for the continuing outbreaks of illness. An ad hoc sample of affected and nonaffected workers was surveyed to assess the influence of psychological, sociological, and work environment factors in the outbreak. Analysis of the data revealed that affected workers reported more physical discomfort (temperature variations, poor lighting) in the workplace as well as psychological job stress (increase in workload, conflicts with supervisors) than did nonaffected workers. Moreover, affected workers scored significantly higher than nonaffected workers on personality tests measuring extraversion and hysteria traits.

On Sept 21, 1973, during and following a football game at which they had participated, 57 members of an Alabama high school marching band (and one accompanying adult) experienced an illness characterized by

. Six girls fainted. Thirty-six students were treated at a hospital emergency room. Those who had played wind instruments and had worn heavier uniforms including an impermeable plastic jacket overlay were affected earlier and more frequently than the others. Several organic causes were examined in an epidemiologic investigation and considered unlikely to explain the epidemic. Female preponderance, a bimodal epidemic curve, hyperventilation, relapses, and clinical features characterized by subjective complaints in the absence of physical findings suggested a syncopal reaction to heat exacerbated and propagated by mass hysteria.
