Antineoplaston A10 injections were administered to 18 patients diagnosed with 19 types of neoplastic disease. The patients' diagnoses included: adenocarcinoma of the rectum and colon, Stage IV (8 cases); adenocarcinoma of the pancreas (4 cases); adenocarcinoma of the breast, Stage IV (3 cases) and single cases of adenocarcinoma of the lungs, Stage III; adenocarcinoma of the stomach, Stage IV; chondrosarcoma of the nose and right maxillary sinus; and carcinoid. The treatment was administered from 52 to 640 days. The highest dosage taken was 2210.5 mg/kg/24 h. Most of the patients were taking from 206.9 to 387.1 mg/kg/24 h. The treatment was associated with minimal side-effects including febrile reactions, muscle and joint pain, muscle contraction in the throat, abdominal pain of short duration and single incidences of nausea, dizziness and headache. Desirable side-effects included increase of platelet count and white blood cell count. Objective response to the treatment was noticed in 8 patients including one patient diagnosed with intraductal carcinoma of the breast, Stage IV, 2 patients with adenocarcinoma of the sigmoid, Stage IV, 1 patient with adenocarcinoma of the rectum, Stage IV, 2 patients with adenocarcinoma of the pancreas, 1 patient with adenocarcinoma of the lung, Stage III, and 1 chondrosarcoma.

A 68-year-old man suffered right facial palsy and left deafness, however, his condition was considered to be idiopathic and he was followed. Three months later, bloody sputum and hoarseness caused him to be admitted to our hospital. An abnormal shadow was detected in the right upper lung field and adenocarcinoma of the lung with multiple brain metastases was diagnosed. He underwent gamma-knife radiosurgery for the brain lesions and subsequent systemic chemotherapy consisting of combined carboplatin and paclitaxel, which were not effective. Subsequently various neurological symptoms appeared, such as muscle weakness of the extremities, dizziness, and gait disturbance. Adenocarcinoma cells confirmed in the cerebrospinal fluid were similar to those in the obtained by transbronchial curetting. Whole-brain irradiation was performed, however, the neurological symptoms worsened and he died. Leptomeningeal carcinomatosis is difficult to diagnose while the patient is alive. It is thought that cranial neuropathy due to leptomeningeal carcinomatosis is a rare form of onset for lung cancer.

Garcin syndrome consists of unilateral palsies of almost all cranial nerves without either sensory or motor long-tract disturbances and without intracranial hypertension, and it is caused by a malignant osteoclastic lesion at the skull base. A 60-year-old woman presented with dizziness and left facial palsy. Progressive left cranial nerve palsies developed over 2 months until gadolinium-enhanced magnetic resonance imaging of the brain revealed an intracranial extension of a tumor from the left skull base. A systemic survey revealed adenocarcinoma of the lung, which had metastasized along the skull base. We experienced a rare case of Garcin syndrome due to skull base metastases from lung cancer.
