The presentation of the report "THE UNILATERAL VESTIBULAR HYPOFUNCTION" has been placed in the scope of the treatment technique, which resulted from the findings and experiences of the author could obtain by his investigations in the clinical cases with unilateral vestibular hypofunction. After having emphasized some main concepts concerning correct evaluation of unilateral hypofunction, which is the central theme of the report, the author indicates how these experiences led him to the conception of this treatment technique: THE VESTIBULAR

In this paper we have tried to explain and to substantiate the physiopathological basis of a new method of treatment for vertigo. In accordance with experimental and clinical evidence, we have based it upon the principle of habituation, which favours the development of what is well-known in Otoneurology as 'central vestibular compensation'. The habituation effect, with pattern building and rebuilding, when applied to the functional sequelase of a labyrinthine lesions, can explain how a new, central regulation of balance can be rebuilt in spite of the persistence of peripheral dysfunction.

Typical Paroxysmal Positional Nystagmus (PPNy) is a highly specific clinical finding that indicates the existence of a specific peripheral lesion: Paroxysmal Positional Vertigo (PPV). However, besides this typical nystagmus of high topodiagnostic significance, atypical PPNy's are also frequently found. Such atypical findings can lead one to suspect a central vestibular system pathology and often require additional diagnostic testing. Of 1805 patients with complaints of vertigo, 305 proved affected by PPV. The present study evaluated how patients affected by the posterior semicircular canal PPV (278/305, 91.2%) responded to canalith repositioning maneuvers. In 246 patients (88.5%) the Semont maneuver was performed as simplified by Toupet, while the remaining 32 subjects (11.5%) were treated with the Epley maneuver as modified by Herdman. The Semont maneuver provides the relief of symptoms in 96% of patients while the Epley maneuver resolved 78% of cases. The patients that did not respond to the maneuvers, even after several attempts, were subsequently treated using "habituation"-based rehabilitation techniques (the Brandt-Daroff technique and Vestibular
Habituation
Training). The presence of atypical PPNy was found in 57 subjects. Of these, the liberatory maneuver always resolved the PPV. In the present study, the choice to perform a neuroradiological study was based on the lack of response to physical therapy, whether performed with liberatory maneuvers or with habituation techniques. In 2 of the 3 cases of PPV which remained unchanged despite treatment, imaging revealed a severe expansive CNS pathology. In these patients the
dizziness
complaints disappeared after neurosurgery: this fact appears to confirm the central origin of these PPNy cases. These data clearly indicate that the results of PPV treatment must be carefully evaluated. In cases of PPV of the PSC, when either typical or atypical PPNy persists one month after diagnosis, with no appreciable period of remission, neuroradiological tests should be performed in order to rule out any CNS pathology.

The aim of this study was to develop a standardized procedure for reproducible quantification of galvanic-induced body sway (GBS). This was a prospective experimental study conducted in a tertiary referral centre. An exploratory study was first conducted to define the galvanic vestibular stimulation (GVS) method that resulted in the best reproducible responses. Ten subjects underwent computer-controlled GVS using five different types of monaural and binaural stimulation with 2-mA currents. Cosinusoidal stimulation gave the most reproducible responses. The frequency and current variability of this stimulus type were then tested in the same 10 subjects. A monaural continuous 1-cosinusoidal current of 0.5 Hz and 2 mA gave the most reproducible responses (< 20% test-retest variation) and the largest GBS amplitude. The other (sinusoidal) stimuli resulted in variabilities exceeding 50%. This stimulus was thus used for further testing in our normative study. In this study we measured GBS amplitude at 0.5 Hz in 60 subjects, with eyes closed and an inter-feet distance of 0 cm, using a force platform. In addition to body sway, responses included slight

, taste sensations and a tingling sensation at the site of stimulation.
Habituation
to the applied stimulus was seen. Binaural prestimulation, performed in 50/60 test subjects, is necessary to reduce habituation and achieve optimal reproducibility in order to be able to compare the sensitivity of the left and right vestibular systems. The test-retest variability was determined in detail in 12 additional subjects. Prestimulation reduced habituation, but improved the sensitivity of the method; some test-retest variability persisted (< 20%).
