We report a rare case of spontaneous rupture of an asymptomatic, intracranial
dermoid cyst
. A 64-year-old man complained of transient
dizziness
. MRI incidentally disclosed a 20-mm-diameter cystic lesion in the left cerebellopontine angle. Since the cyst was asymptomatic, follow-up MRIs were performed. One year later, sudden headache and left oculomotor palsy occurred. MRI showed niveau formation within the cyst and scattered fat droplets within the cerebrospinal fluid space, which indicated a spontaneous rupture of the
dermoid cyst
. Since hydrocephalus on MRI and gait disturbance appeared 2 months later, the tumor was resected, and a ventriculoperitoneal shunt was inserted. The patient's symptoms disappeared, and there were no postoperative neurological deficits. The pathological diagnosis was
dermoid cyst
. Only 48 cases of spontaneous rupture of a
dermoid cyst
have been reported. All were symptomatic, and MRI showed a large cyst. Of these cases, none was detected incidentally. Therefore, this is the first case report of an incidentally found
dermoid cyst
that ruptured spontaneously. Although asymptomatic, small dermoid cysts are usually followed up by MRI without surgical intervention. The possibility of spontaneous rupture, which may give rise to hydrocephalus, as in this case, should always be kept in mind.

Along with epidermoid cysts, dermoid cysts are uncommon, benign and slow-growing lesions. We described a case of diploic
dermoid cyst
of the occipital bone with an intracranial extension in the right posterior fossa diagnosed and subjected to pathological examination. A 63-year-old man presented with
dizziness
at the time of posture conversion. Magnetic resonance imaging(MRI)showed a heterogenous hypo-intensity area on T1-weighted image, hyper-intensity on T2-weighted image and remarkably-strong intensity on diffusion-weighted image. Gadolinium enhancement was partially seen in the tumor capsule. Bone density computed tomography (CT) and 3-dimensional CT using helical CT scan revealed the osteolytic range with destruction of the inner and outer table. From these findings, this tumor was diagnosed as diploic epidermoid cyst before surgery. The tumor was totally removed and underwent cranioplasty with a titanium plate. However, pathological examination confirmed
dermoid cyst
with existence of sebaceous gland in some cyst-walls. It was difficult to diagnose this case as dermoid tumor from radiological features before surgery.

at the lateral sphenoid ridge. A 53-year-old woman admitted because of progressive headache and

. The patient had no neurologic deficits, and magnetic resonance imaging with no contrast enhancement revealed a mass at the right sphenoid ridge. The mass was accompanied with sphenoid bone erosion visible on computed tomography. The patient underwent right pterional craniotomy, and the tumor including the capsule was totally resected. was confirmed with histopathological examination. The patient had no complications during the postoperative period. This study suggests that

Intrasellar dermoid cysts are extremely unusual lesions, with only four cases reported to date, and have not been previously reported in association with sudden-onset symptoms. Here, we present the case of an intrasellar
dermoid cyst
with sudden-onset symptoms mimicking pituitary apoplexy in an elderly woman. A 69year-old woman presented with sudden onset of headache,
dizziness
, and decreased visual acuity. Magnetic resonance imaging of the sellar region showed an intrasellar lesion, which showed mixed hyper- and hypointense signal on T1-weighted and T2-weighted images and enhanced peripherally. Endocrine workup showed pituitary hormones within normal levels. According to these findings, the initial diagnosis of nonsecreting pituitary macroadenoma apoplexy was made. Intraoperatively, a large amount of whitish-yellow purulent material was found in the mass and the lesion was partially removed, owing to tight adhesion between remanent mass and surrounding neurovascular structures.
