An epidemiological survey focused on farmers engaged in vinyl-house culture of strawberry was carried out among the inhabitants of a farming village. For avoiding residential and habitual factors, all residents (851 in number) over 12 years of age in the area were inquired about symptoms, personal and family histories. There were no marked difference in age and sex among three groups, i.e. farmers with and without vinyl-house culture and people with no relation to farming in this area. Incidence of one of the inquired symptoms were 54% and 45% in farmers with and without strawberry culture in vinyl-house respectively, and 34% in the non-farming people. This difference became more larger up to 83% in the strawberry farmers of allergic constitution. Symptoms found more in the farmers than the other were headache, dizziness, conjunctivitis, rhinorrea, sneeze, palpitation and neuralgia, and the last four symptoms were more marked among farmers in charge of strawberry culture. Incidence of the allergic symptom consisting of conjunctivitis, rhinorrea and sneeze was more marked among the persons of allergic constitution . In conclusion, health effects of strawberry culture in vinyl-house in farmers can be look upon from three aspects; overwork, a particular stance during the work and allergic responses to the dust in the vinyl-house.

During the course of a study on the effects of chronic, heavy marijuana use, a placebo control subject reported dizziness and nausea, showed tachycardia and conjunctivitis, and was found to have urinary cannabinoid metabolites. Staff members observing the marijuana smokers also complained of similar subjective symptoms. The detection of cannabinoids in the urine of this nonsmoker documents the previously anecdotal concept of the "contact high" and has implications for marijuana research and for precautions that may be necessary should marijuana become legal.

Clinical and epidemiological features of 72 cases of boutonneuse fever, detected from 1979 to 1986 at the "Hospital del Insalud de Cartagena" (Murcia), are reviewed. The most important clinical features are commented on (fever, rash, tache noire), as well as the less important (dizziness, cough, conjunctivitis, sensorial changes). Several epidemiological factors were analyzed, temperature having been found to be the most significant in our area. The complications observed mainly affected patients with risk factors (senility, thrombocytopenia, hyponatremia, high blood pressure) being particularly severe in a patient who developed lethal DIC and in another 2 who developed pulmonary thromboembolism. The prophylactic measures suggested were the strict control of animals susceptible to parasites and organizing of disinfestation campaigns in the endemic areas.

For three months a 17-year-old boy had been suffering from laryngitis, pharyngitis and conjunctivitis, weight loss, nocturnal sweating and signs of changing cranial-nerve involvement (dizziness, nausea, nystagmus). The diagnosis of recurrent polychondritis was made only when, in addition to a definite inspiratory stridor there also developed a painful swelling of the left ear cartilage and a saddle nose due to loss of the cartilaginous portion of the nasal skeleton. Histological examination of the inflamed ear cartilage confirmed the diagnosis. After treatment with prednisone (100 mg daily, gradually reduced to 25 mg) and azathioprine (100 mg daily, increased to 150 mg after three weeks), there was clinical improvement, but the airway resistance (R = 6.2 cm H2O.s/l) rose further, requiring tracheostoma twelve months after onset of symptoms.

This is a report of the clinical analysis of 250 cases of dengue fever-like patients during the 1987-1988 epidemic in Southern Taiwan at Harn's clinic, Kaohsiung city. The results were: The peak age groups were between 20-40 years old. The ratio of male to female was 1:14. 98% patients presented with fever of mainly 3 to 6 days duration. 98% reported body pain, which included headache (82%), generalized soreness (65%), low back pain (50%). 95% patients had GI symptoms, such as anorexia (73%), nausea (60%), vomiting (45%). 97% patients complained of dizziness and weakness, and 22.4% patients had a bleeding tendency. The common bleeding sites were involving the subcutaneous (skin), gum, uterus, nasal cavity, GI tract, retina, and GU tract. The eruption rate was 78%. A majority of rashes appeared on the extremities, and then spread to the trunk. Less common symptoms were insomnia, chest tightness, urgency of urination, cough, running nose, palpitation, and shock. 45% patients were noted with hair loss. The hair loss was the latest presenting symptom, which tended to occur about two months after the recovery stage, and lasted for about one month. On physical examination, 62.6% patients had conjunctivitis, while only 1.3% had lymph node enlargement. Usually, the whole clinical course lasted for 7 to 14 days. Laboratory examination showed that leukopenia was noted from the second day and reached the lowest count on the fifth day, and recovered on 9th or 10th day. In 247 blood sera tested serologically for dengue fever, 84 were confirmed, while 157 were not confirmed yet due to patients' reluctance to accept a second blood drawing.(ABSTRACT TRUNCATED AT 250 WORDS)

Two adult patients with opsoclonus and tremor of the whole body associated with viral infections are reported. The first man presented with mumps (parotitis, orchitis and encephalitis). Paired serum mumps titers were both 1:80. The second patient had conjunctivitis and dizziness. Acute and convalescent sera showed significant rise of poliovirus type 3 titer. Clonazepam attenuated the symptoms in both patients.

Lambs were successfully infected with bovine strains of a respiratory syncytial virus. When the Nomi reference strain and the local Antonovo strain of the RSV were used i/v and via the trachea the animals reacted on the 3rd to 5th day with dizziness, dyspnea, conjunctivitis, nasal discharges, etc., more strongly manifested with the use of the Antonovo strain. The temperature reaction started on the 3rd day and receded on the 7th day, with a second peak on the 8h day. Serologically, RSV-antibodies were released on the 14th day, and that correlated in dependence on the severity of the clinical manifestations. Such observations made it possible to use lambs as a sensitive animal species to differentiate the virulence of the tested strains. Testing along these lines rendered is feasible to obtain attenuated strains and use various immunoprophylactic agents against the respiratory syncytial virus.

A 40-year-old man developed general malaise, dizziness and progressive headache 1 week after acute haemorrhagic conjunctivitis. Bizarre behaviour, confusion and adversive seizures occurred later. MRI showed haemorrhagic infarct in the bilateral basal ganglia and thalamus and abnormal signal density in the internal cerebral veins. Bilateral carotid angiography showed no filling of superior sagittal sinus and internal cerebral veins. With MRI, cerebral venous thrombosis can be suspected and diagnosed earlier than before, obviating any invasive investigation.

Breynia officinalis has the Chinese proprietary name, Chi R Yun, which means dizziness or vertigo for 7 d. In daily practice, it has been used to treat venereal diseases, contusion, heart failure, growth retardation and conjunctivitis in combination with other traditional Chinese medicines. Two hospital-based cases of Breynia officinalis poisoning have been reported to the Poison Control Center. Case 1 was a 43-y-old female who consumed a mixture of 1500 g lower stem and root of Ji Mu Ju in boiled water in a suicide attempt. Her AST reached 264 and ALT reached 2443. Case 2 was a 51-y-old female who consumed 20 pieces of lower stem and root of Ji Mu Ju stewed with meat and 100 ml of wine to treat chronic contact dermatitis. Her AST reached 3815 and ALT reached 6625. In both cases Breynia officinalis was identified as the cause of poisoning. Poisoning in humans involves the neurologic, gastrointestinal, hepatic, urinary and respiratory systems. Hepatotoxic effects have been reported for some Chinese herbal medicines, but not Breynia officinalis: Breynia officinalis poisoning causes hepatocellular liver injury rather than cholestatic liver injury.

A 64-year-old man developed cognitive impairment. A neurological examination showed hypereactive tendon reflex. A cerebrospinal fluid (CSF) examination showed a mild increase in both the number of mononuclear cells (14 cells/mm3) and the protein level (45 mg/dl). An electroencephalogram showed diffuse slow wave activity. Brain MR images revealed multifocal lesions in the limbic area and white matter regions. He was suspected to have autoimmune encephalitis and his symptoms improved after treated with methylprednisolone (1 g/day for 3 days). Despite this treatment, his mental disorders later relapsed. His condition improved after undergoing repeated treatment. Thereafter, he developed bacterial meningoencephalitis which had been caused by Listeria monocytogenes during the period of the internal administration of steroids. Antibiotics successfully cured his Listeria meningoencephalitis. He thereafter developed conjunctivitis, polyarthritis, hearing difficulties, dizziness and auricular chondritis. Based on these findings, we diagnosed the patient to have relapsing polychondritis (RP). It was thus conceivable that his cognitive impairment has been caused by CNS vasculitis associated with RP. RP therefore should be carefully taken into consideration and be included in the differential diagnosis as a possible cause for CNS vasculitis.
