An open study was carried out to determine the clinical efficacy of secnidazole in the treatment of acute and chronic cases of
amoebiasis
. Patients were given 2 g of secnidazole in a single dose or in two divided doses within a 4 h interval. Details were recorded daily by patients of complaints such as fever, lumbar pain, LBM,
dizziness
, body weakness, bloody mucoid stools, epigastric pains and other symptoms, before and after treatment. Results were analysed for 45 chronic and five acute cases and showed clinical cure of 98% in chronic and 80% in acute cases. The acute group showed a non-significant change (P greater than 0.05) which could be attributed to the small sample size. The treatment regimen proved effective for the eradication of E. histolytica and only two (4%) remained positive after treatment out of the 50 patients initially positive.

Go.10213, a new nitroimidazole, was studied in 12 male volunteers for tolerability and in 20 patients with intestinal
amoebiasis
for antiamoebic activity. Go.10213 was well-tolerated by volunteers up to a dose of 400 mg X 3. Patients also tolerated well the dose of 100-150 mg X 3 for 7 days. In two patients, there were mild and transient side-effects like headache,
dizziness
, fatigue, etc. No neurological side-effects were observed. There were no significant changes in blood pressure, pulse rate and ECG. The organ function tests did not show any adverse effects of Go.10213 on bone-marrow, kidney, liver, etc. Go.10213, at a dose of 150 mg X 3, showed potent antiamoebic activity in 10 patients with intestinal
amoebiasis
, as judged by the clinical relief, the eradication of the trophozoites and cysts of Entamoeba histolytica from stools and the healing of colonic ulcers. Go.10213, a novel nitroimidazole, may prove to be the most potent and safe agent against the protozoal infections, e.g.
amoebiasis
, giardiasis and trichomoniasis.

Subjective adverse reactions to metronidazole were analyzed in 111 patients with
amebiasis
. Metronidazole was administered to 36 patients at a daily dose of 2250 mg and 75 patients at daily doses lower than 2250 mg. The reactions reported included nausea without vomiting in 11 (9.9%) patients, nausea with vomiting in 2 (1.8%), dysgeusia in 2 (1.8%), diarrhea in 1 (0.9%), headache in 1 (0.9%), numbness in 1 (0.9%),
dizziness
in 1 (0.9%), urticaria in 1 (0.9%), exanthema in 1 (0.9%), and discomfort in 1 (0.9%). Nausea was reported by 28% (10/36) of the patients receiving metronidazole at a daily dose of 2250 mg and 4% (3/75) of the patients receiving lower daily doses. The duration of the metronidazole administration in days was not associated with the appearance of nausea. No life-threatening adverse reactions were identified, and good clinical therapeutic effects were observed in 96% (107/111) of the patients. While metronidazole appears to be a safe anti-protozoal agent for patients with
amebiasis
, our results indicate that a daily metronidazole dose of 2250 mg is excessive for
amebiasis
, as it often induces nausea.
