Chronic subdural hematoma (CSDH) is one of the most common clinical entities in daily neurosurgical practice. The diagnosis and treatment are well established, but recurrence, complications, and factors related to these problems, especially in the elderly, are not completely understood. This study evaluated the clinical features, radiological findings, and surgical results in a large series of the patients treated at the same institution. 500 consecutive patients (359 men and 141 women) with CSDH were treated by burr
hole
craniostomy with closed system drainage from January 1987 through February 1999. Causes, clinical and computed tomographic findings, surgical results, re-expansion of brain after surgery, and hematoma recurrence were statistically analyzed to elucidate the potential risks of CSDH. Most patients (89.4%) had good recovery, 8.4% showed no change, and 2.2% worsened. Six patients (1.2%) died, three due to

. Recurrence of hematoma was recognized in 49 patients (9.8%), at 1 to 8 weeks (3.5 +/- 1.9 weeks) after the first operation. The brain re-expansion rate at one week after operation was 45.0 +/- 21.4% in patients with hematoma recurrence and significantly lower than 55.3 +/- 19.1% in patients without recurrence (p < 0.001). Old age, pre-existing cerebral infarction, and persistence of subdural air after surgery were significantly correlated with poor brain re-expansion (p < 0.001). Twenty-seven patients (5.4%) suffered postoperative complications, of which 13 cases were acute subdural hematoma caused by incomplete hemostasis of the scalp wound and four cases were tension pneumocephalus. Careful hemostasis and complete replacement of subdural hematoma by normal saline to prevent influx of air into the subdural space will further improve the surgical outcome for patients with CSDH.

We treated two cases of a subdural hematoma associated with dural metastasis of gastric cancer, from which both patients died. Case 1: A 60-year-old female patient was hospitalized with a diagnosis of type 4 gastric cancer of the antrum. The patient suddenly collapsed, and, subsequently, left hemiplegia and a depressed level of consciousness were noted. surgery was performed on the second day of hospitalization because of an enlargement of the hematoma. Laboratory data on admission showed the presence of a
disseminated intravascular coagulation
(DIC). Bone marrow aspiration revealed metastases of signet ring cell carcinoma, and abdominalCT showed gastric cancer. He was diagnosed as having DIC with bone marrow metastases of advanced gastric cancer. Despite anti-DIC therapy and blood transfusion, his systemic bleeding tendency was not improved. The neurosurgeon therefore consulted with a palliative care team. Since the patient was still young, we considered that he should be treated with anti-cancer drugs. At first, his family did not accept chemotherapy because they were pessimistic about his prognosis. However, after he regained his consciousness, we were able to perform sequential MTX and 5-FU therapy with the consent of the patient and his family. The therapy was successful, and he recovered from DIC and was discharged on the 57th hospital day.

) and a fixed camera has been fully investigated in many published works and widely used for various applications. However, in certain special cases (e.g., long-term deformation monitoring of engineering structures, or deformation measurement of a specimen subjected to uncommon loading), it is very difficult or impossible to carry out in situ deformation measurement using 2D-
DIC
, and necessitates ex situ 2D-
DIC
measurements for a repositioned specimen or using a relocated camera. To achieve accurate measurements, the error sources of ex situ 2D-
DIC
measurements should be identified and minimized, and the strain accuracy of ex situ 2D-
DIC
measurements should be quantified. In this work, the potential error sources of ex situ 2D-
DIC
measurements are first discussed in detail. Then, to mitigate the errors associated with these issues, an ultra-stable 2D-
DIC
system combining the idea of active imaging and a well-designed bilateral telecentric lens is established, which is invariant to the potential variations in ambient lighting and the possible small out-of-plane motions in object surface and/or image plane. The established ultra-stable 2D-
DIC
system is first compared with the regular 2D-
DIC
setup in determining the surface strains of an unstrained sample. Then, ex situ residual stress measurement using the
hole
-drilling technique and the established 2D-
DIC
system was conducted and compared with the applied ones. The results of this work confirm that the accuracy of ex situ 2D-
DIC
deformation measurements using the proposed system is of high fidelity, and can be used for accurate deformation measurement in practical ex situ tests.
