The case of a young shepherd with Brucella melitensis aortic valve endocarditis is presented.
His
illness ran an afebrile course and was also complicated by
disseminated intravascular coagulation
(
DIC
), nephritis, hepatitis and peritonitis, all of which responded well to supportive measures and a combination of tetracycline, trimethoprim-sulphamethoxazole and amikacin sulphate. The fact that even the most severe case of brucellosis can present without fever is stressed.

A new born infant who died when he was 40 days old, after urinary infection, septicemia and
disseminated intravascular coagulation
(
DIC
) is reported. The concentration of fibronectin (FN) was undetectable (less than 1.1 mg/dl).
His
mother and one sister had also decreased levels (19 and 19.5 mg/dl), although inside normal limits, when they were compared to simultaneously studied normal controls (27 +/- 8). There were not infections, coagulation disturbs neither keloid scars in the family. A patient cousin also died at the first days of life. The deficiency of our case was much more important that FN levels found in 23 children with sepsis and
DIC
(range 6-38 mg/dl), therefore it is possible he had a primary deficiency. The investigation of FN levels in all newborns with severe infections or other disturbs is recommended, since these patients could be benefit from purified FN or cryoprecipitate therapy.

This is a case report of a 61-year-old man, suffering from infective endocarditis of mitral valve in association with

and intracerebral hemorrhage. He was treated symptomatically and conservatively for 80 days after the onset of the
DIC
.
His
multiple cerebral hematomas gradually disappeared 2 months after conservative management. Then the diseased mitral valve was replaced with a Duromedics valve. He is doing well at present. In a review of the literature it appears that there is some room for surgical intervention in endocarditis despite the complication of coagulopathy. We feel that one of the most serious complications of infective endocarditis is
DIC
. Operative management of endocarditis with this complication is recommended to be followed by medical management.

A 58-year-old man, born in Nagasaki prefecture, was admitted to our hospital because of anorexia and general fatigue on November 22, 1984. Hepatosplenomegaly was found without skin eruption. The blood examination on admission revealed leukocytosis (50,800/microliter) and atypical lymphocytes with hyperlobulated nuclei. He had hypercalcemia, and hepatic and renal damage. A diagnosis of adult T cell leukemia (ATL) in the acute stage was made. Treatment with KM2210, a conjugate of chlorambucil and estradiol, was started, and his peripheral leukocytes decreased gradually reaching, 19,700/microliter by the end of this medication.
His
leukocyte count continued to decrease after discontinuation of KM2210 and reached a nadir of 4,700/microliter. Hepatosplenomegaly and hypercalcemia also improved. About one month later, recurrence of the disease occurred and he was again treated with KM2210. Although the second course of the KM2210 therapy was also successful in relieving hepatosplenomegaly and leukocytosis, it proved impossible to ameliorate his poor condition and he died of
DIC
. Our case suggests that KM2210 has a remarkable cytotoxic effect against ATL cells even in the acute stage but the optimal schedule of treatment with this new drug should be established in order to obtain more satisfactory therapeutic results.

We reported three patients who developed
disseminated intravascular coagulation
(
DIC
) accompanying thoracic and abdominal aortic aneurysm. The first case, a 26-year-old man with dissecting aortic aneurysm developed
DIC
with clinical bleeding after operating on glaucoma. The administration of fibrinogen concentrates and antifibrinolytic agent made his
DIC
improve. The second case, a 70-year-old man with abdominal aortic aneurysm developed
DIC
showing large ecchymosis after angiography.
His
DIC
disappeared after operation on aneurysm. The third case, a 73-year-old woman with thoracic and abdominal aneurysm developed laboratory-
DIC
without severe hemorrhagic diathesis. During antifibrinolytic therapy, platelet count, fibrinogen and fibrinogen degradation product (FDP) level improved. Since the treatment of the coagulopathy might be varied in the situation of the cause and clinical course, it is noted that anti-fibrinolytic therapy was effective in our two cases.

).
His
red blood cells (RBC) were shown to be T- and Tk-activated; serial testing showed his mature RBCs as well as neocytes remained T-activated at 40 days. Anti-T was detected in his serum, with only one of two T-activated RBC samples. T-activating enzyme was shown to be present in his serum.

Within a period of 6 years 8 patients with 12 bites by potentially hemotoxic snakes (crotalids, viperids) were hospitalised in various clinics of the Basle area. All the patients were snake owners. One patient was hospitalized 3 times, 2 patients twice and the others once. In two cases no signs of envenomation were noted after an observation period of 12 hours. Ten bites were followed by local cytotoxic symptoms e.g. oedema and suffusions. Among this group only 6 cases had further signs of systemic, hemotoxic envenomation with decrease of the activity of various coagulation factors. Complete
defibrination
was seen in 4 patients, but only in 2 cases with additional thrombopenia did overt clinical symptoms of bleeding complications (urological bleeding) occur. Bleeding complications of the central nervous system or of the gastrointestinal tract were not seen. Antivenin was used in three patients and coagulation mechanisms recovered within 24 hours. One patient with complete
defibrination
, thrombopenia and overt bleeding (macrohematuria) refused antivenin.
His
coagulation parameters started to improve after 7 days and became normal on day 11.

The interaction of the actin-binding protein hisactophilin from Dictyostelium discoideum amoebae to partially charged lipid membranes composed of mixtures of L-alpha-dimyristoylphosphatidylcholine (DMPC) with L-alpha-dimyristoylphosphatidylglycerol (DMPG) and L-alpha-phosphatidylinositol 4,5-bisphosphate (PIP2) is studied by film balance experiments, microfluorescence, and lateral diffusion measurements at low ionic strengths (approximately 20 mM). Excess surface concentrations and adhesion energies of the protein are evaluated by the application of Gibbs law of surface excess as a function of charged lipid content. ) and natural protein with a fatty acid (

) isolated from Dictyostelium cells are compared. For mixtures of DMPG and DMPC, protein binding leads to an increase in lateral pressure of the monolayer (at constant area) and causes strong lipid immobilization pointing to partial penetration of the protein into the lipid layer. Falciparum malaria is the most hazardous form of malaria. Its high degree of parasitemia interferes with vital functions of most organs and is directly responsible for its high rate of mortality and morbidity. Quinine and other antimalarial drugs are relatively slow acting and not always effective due to the growing resistance developed by Plasmodium toward these drugs. Another emergency modality, which would remove the parasitic burden quickly and effectively, is thus much needed. We present a case of a 51-year-old sailor, who was admitted to the hospital because of complicated falciparum malaria.
His
situation deteriorated rapidly into a desparate stage, despite the various intensive treatments and quinine. He soon developed a systemic inflammatory response syndrome manifested as cerebral malaria, renal failure, acute respiratory distress syndrome and
disseminated intravascular coagulation
. An emergency blood exchange reversed the situation dramatically, and the patient recovered completely. It is recommended that any doctor, both in endemic and in non endemic areas, dealing with blood transfusions or infectious diseases, should be acquainted with this lifesaving modality, regardless of the controversy still surrounding this subject.

We describe a 39 years-old male hemophilia A patient with acquired immunodeficiency syndrome (AIDS) developing to
disseminated intravascular coagulation
(
DIC
) due to gastric carcinoma. He had been diagnosed as human immunodeficiency virus (HIV) sero-positive in 1990. Since then, he has been treated by the oral administration of zidovudine (AZT), dideoxyinosine (ddI) and intravenous administration of glycyrrhizin. In September 1990, he suddenly complained abdominal pain with bloody stool.
His
condition deteriorated in spite of our intensive treatment for
DIC
. He died of multiple organ failure (MOF) due to
DIC
. The autopsy findings showed gastric carcinoma, defined of signet ring cell carcinoma histopathologically. But neither opportunistic infection nor other cause of
DIC
were observed.
