Blood coagulation tests were performed on admission to the hospital and on consecutive days after severe and moderate head injury in 34 patients. Platelet counts and fibrinogen were normal at admission and raised thereafter. The partial thromboplastin time was shortened at admission and lengthened in the following days. Fibrinolytic activity was enhanced at admission. The
ethanol
gelation test was negative in all patients during the post-traumatic time course. It was concluded that, in the first 24 hours after injury, activated coagulation was present after head injury. In contrast with data of other authors,
disseminated intravascular coagulation
did not occur in these series.

Investigations with the Nomarski
DIC
(differential interferece contrast) microscope and the electron microscope on the nature of hyaline zones in the cytoplasm of Amoeba proteus revealed that these regions represent pure ground cytoplasm. Differences between specimens 1) treated with 2%
ethanol
, 2) released from high hydrostatic pressure or 3) preincubated at 35 degrees C for 30 minutes could not be observed. Only dying cells undergoing lysis contained a watery solution within the zones of hyaline appearance. The existence of a so-called cell surface complex composed of the plasma membrane and an electron dense filamentous layer of groundplasm was demonstrated by the electron microscopical analysis of narcotized and pre-heated amoebae. This complex corresponds morphologically to the cell surface complexes in tissue cells. Hence it seems possible that the cell surface complex of amoebae is also responsible for changes of the cell shape and movements of the cell membrane. Observations with the
DIC
microscope also revealed the existence of two types of hyaline caps in A. proteus: in pseudopodia extending during normal locomotion the hyaline cap consists of pure ground cytoplasm, whereas in specimens showing fountain-like streaming the cap is built up by a large vacuole containing a watery fluid.

In 753 consequtive blood samples (from the same number of patients) routinely examined for coagulation abnormalities, a positive
ethanol
gelation test was found in 160. In 7 samples with plasma fibrinogen below 150 mg/100 ml and a positive test, other laboratory signs of
consumption coagulopathy
(low thrombocyte count, low factor V, low antithrombin III, large amounts of FDP in serum and great discrepancies between Normotest and Thrombotest) were regularly observed. Also at normal or high fibrinogen levels, a positive
ethanol
gelation test was more frequently associated with low thrombocyte counts, low factor V and Normotest/Thrombotest discrepancy than was a negative test. In 46% of the samples with a positive test, fibrinogen was higher than 500 mg/100 ml (versus 4% in those with a negative test). In one-third of these samples, all other parameters gave normal results.

Hemocoagulation was studied in 116 patients with prostatic adenoma, stage II, before and 1, 4-7, 8-10 and 11-15 days after surgical intervention. Out of 77 patients exposed to routine adenomectomy, 55 persons were treated 12-14 days prior the surgery with therapeutic dosages of a vitamin A, E, C, P complex. After the operation they were given nicotinic acid in presence of the double load of vitamins A and E. Out of 39 persons exposed to urgent adenomectomy 19 patients were administered the double dosages of the same vitamins before the operation. The rest of the both groups were postsurgically given acetylsalicylic acid instead of vitamins. It was stated that the aforementioned vitamin therapy significantly decreased postsurgical blood coagulation disorders (in those who were prepared for the operation beforehand it happened before adenomectomy), improved the clinical course and outcomes of the postsurgical period. Presurgical investigations reveal an accelerated intravascular coagulation and mild manifestations of the
DIC syndrome
in patients with prostatic adenoma. After the operation the syndrome manifestations increase but rarely develop into stages II and III that are usually accompanied by
consumption coagulopathy
. To follow the syndrome progression one could use only 5 out of 13 tests defined by the authors: activated time of recalcification, prothrombin index, fibrinolytic activity, products of fibrin degradation and an
ethanol
test.

Studies of coagulation hemostasis in 108 patients with rheumatoid arthritis have revealed that high blood coagulation is characteristic of this disease, and a number of coagulation parameters (antithrombin III, fibrinogen, fibrinolytic activity,
ethanol
test) point to intravascular microthrombi formation (
disseminated intravascular coagulation
). Along with hemostasis investigations, Willebrand's factor levels were measured in the blood plasma of 44 patients. Enhancement of the inflammatory process activity was found to be associated with elevation of this factor level and augmentation of the blood coagulation activity. These results permit considering Willebrand's factor as one of the diagnostic criteria of thrombophilic conditions in patients with rheumatoid arthritis.

Therapy with vincristine (2 mg i.v. weekly) and prednisolone (100 mg p.o. daily) caused a decrease in fibrinogen levels in nine patients treated for lymphoid blast crisis LBC) of chronic myeloid leukemia (CML). During the first days of treatment
disseminated intravascular coagulation
(
DIC
), evidence by a positive
ethanol
gelation test, markedly increased thrombin-antithrombin III complex and fibrin-split product D-dimer levels, and a rapid fall in fibrinogen levels was observed in two patients. The induction of
DIC
in these two patients caused profuse bleeding in one and necessitated substitution therapy with fibrinogen and platelet concentrates. The remaining seven patients revealed no signs of
DIC
; nevertheless, four of them showed a moderate increase in D-dimer levels after initiation of therapy. In these patients a well-known side effect of long-term steroid therapy, namely a decrease of fibrinogen levels, was observed within the first week of treatment. Fibrinogen levels did not fall below 150 mg/dl and increased after dose reduction from 100 mg/day to 50 mg/day. We conclude from our results that two types of disturbances in fibrinogen metabolism can be observed during vincristine/prednisolone therapy of LBC of CML: (a) a decrease of fibrinogen levels due to a steroid-mediated impairment of liver synthesis, and (b) a rapid fall in fibrinogen levels in the course of
DIC
, most likely induced by the release of procoagulants from deteriorating blast cells, leading to severe bleeding in selected cases.

The coagulation system has been examined in 30 women with late toxemia before and after heparin and trental therapy. Pretreatment fibrinogen levels were elevated and antithrombin III levels were reduced. A hemolysate-calcium mixture showed an increase in coagulation at 8 and 10 minutes of incubation. Staphylococcus agglutination and
ethanol
tests revealed a significant increase in early PDF and other constituents of the fibrinogen pool. These changes were indicative of prolonged-type
disseminated intravascular coagulation
in late toxemia. Comprehensive therapy comprising heparin and trental leads to a significant reduction in fibrinogen concentration, alleviation of the hypercoagulatory shift and increase in antithrombin III level. Early PDF and fibrin monomer complexes showed a decline. In addition, the therapy had beneficial effects on the maternal and fetal clinical status.

The intravenous infusion of calcium 2-amino
ethanol
phosphate was coincidental with cardiopulmonary arrest in a 53-year-old woman with a history of multiple sclerosis. Resuscitation was followed by massive hemolysis, renal failure, adult respiratory distress syndrome, shock liver, and
disseminated intravascular coagulation
. This agent, in use by at least one practitioner in West Germany for the treatment of inflammatory and autoimmune disorders is not FDA approved for use in the United States, nor is clinical investigation underway. It is currently thought to be in use by about 200 practitioners throughout this country as treatment for multiple sclerosis. It is apparently obtained in West Germany and brought illegally into the United States. This is the first known report of an adverse drug reaction associated with the use of this product.

Changes in blood coagulation and fibrinolysis were investigated in twenty seven patients with esophageal varices, who underwent endoscopic injection sclerotherapy (EIS) with 5% ethanolamine oleate with meglumine amidotrioate (EOMA), 1% Aethoxyskererol (AS) and pure
ethanol
. Changes in platelet aggregation between before and just after EIS were also investigated. Results obtained were as follows. 1) Remarkable changes in blood coagulation and fibrinolysis were observed in twenty patients, who underwent EIS by intravariceal injection combined with paravariceal injection. The patients showed significant change in factor XIa-alpha 1 antitrypsin complex as a result of destruction of the endothelium of varices, caused by the sclerosants. On the other hand, no significant changes were observed in seven patients, treated by paravariceal injection alone. 2) These changes were different from those observed in
DIC
because they were transient without bleeding tendency or multiple organ failures. 3) Paravariceal injection was suitable for EIS in the patients with very poor liver function. 4) Platelet aggregation was not elevated by EIS.

Although formalin ingestions have previously been reported in the literature, technology has only recently been developed to measure both formaldehyde and formate levels in plasma. Methanol, formaldehyde, and formate levels were followed in the case reported here until the patient's death approximately 13 h after the ingestion. The clinical course was marked by an initial profound CNS depression followed by an apparent clinically quiescent period. Severe abdominal pain and retching preceded the development of seizures,
DIC
, severe hypotension, and cardiac arrest. Methanol levels rose throughout this 13-h course. Formate and formaldehyde levels increased until bicarbonate and
ethanol
therapy were instituted. The "fixing" of the stomach by formaldehyde may have produced delayed absorption following formalin ingestion. Therapeutic implications are discussed.
