Diffuse erosive "gastritis" was discovered as early as five hours postinjury in 45 of 54 burn patients (83.5%) evaluated by gastroduodenoscopy. Acute ulcers were identified in 14 patients (26%); concomitant duodenal disease was present in 34 patients (76%). Microvascular fibrin thrombi were not demonstrated even though five patients had disseminated intravascular coagulation. Seven patients were examined before nasogastric intubation; four, with a mean burn size of 59.6% total body surface, had diffuse "gastritis." Low total serum protein levels were measured in 81% of tested patients, but were not predictive of mucosal disease. Hemorrhage followed the clinical deterioration of six patients (11.1%); one ulcer perforated. Whereas coagulation abnormalities, nasogastric intubation, and hypoproteinemia may augment mucosal injury, the morphologic and histologic examinations of the lesions suggested a primary ischemic cause resulting from the opening of submucosal shunts or local vasoconstriction.

A case of cerebral astrocytoma associated with a systematic multiple myeloma is reported. A 76-year-old man was admitted to our hospital because of left hemiparesis. The CT scan revealed a cystic tumor at the right parietal lobe. A serum protein electrophoresis showed a monoclonal peak in the region of gamma-globulin, and examination of bone marrow revealed 15% of atypical plasma cells. After irradiation and chemotherapy, the patient suffered from DIC syndrome and died. Autopsy findings revealed a cerebral astrocytoma and bone marrow infiltration by myeloma cells in the sternum, vertebrae, costae, pelvis, and so on. So we finally diagnosed this case a cerebral astrocytoma with systematic multiple myeloma. In spite of the fact that systematic multiple myeloma is frequently accompanied with cancers at other sites (so called multiple primary cancers), the coexistence of a cerebral astrocytoma and systematic multiple myeloma is extremely rare. Complications of multiple myeloma with other tumors are reviewed.

We evaluated the roles of plasma endothelin-1 and plasma thrombomodulin in the development of disseminated intravascular coagulation (DIC) in patients with sepsis. Plasma endothelin-1 was measured by radioimmunoassay (RIA). Plasma thrombomodulin and tumor necrosis factor-alpha (TNF-alpha) were measured by enzyme-linked immunosorbent assay (ELISA), and serum protein C (protein C) was measured by the synthetic substrate method. Endotoxin was measured by the Endospecy test, a synthetic substrate method. A new perchloric acid method was used for the pretreatment of plasma. Blood levels of endothelin-1 and thrombomodulin were significantly higher in patients with DIC than in those without DIC (p < 0.0001). Endothelin-1 and thrombomodulin levels were positively correlated (r = 0.8645, p = 0.0001), as were endothelin-1 and TNF-alpha levels (r = 0.5441, p = 0.0002). Thrombomodulin and protein C levels were negatively correlated (r = -0.5627, p = 0.0001). Endotoxin was elevated above the normal level 14.3% (6/42) for these patients. TNF-alpha is involved in the production of endothelin-1 and thrombomodulin, which play a role in the pathogenesis of DIC and whose blood levels reflect its severity.
