A 49-year-old woman was admitted to our hospital because of
fever of unknown origin
. The patient had long-lasting spiking fever, hepatosplenomegaly, pleural effusion, and skin rash. Laboratory tests showed marked leukocytosis and an extremely high serum ferritin level (240 000 ng/ml) accompanied by
disseminated intravascular coagulation
and hemophagocytic syndrome. Most of the patient's features were compatible with a diagnosis of adult-onset Still's disease (AOSD), the rash, however, was not a typical rheumatoid rash but multiforme erythema. Biopsy of a breast nodule revealed breast cancer, leading us to a diagnosis of paraneoplastic syndrome mimicking AOSD. Although this is a rare disorder, cases resembling the present one have been reported, indicating the importance of including paraneoplastic syndrome in the differential diagnosis of AOSD.

Myxomas, as other primary cardiac tumors, appear rarely. The most common symptoms are typical of mitral stenosis or peripheral embolism. The rarity of infected cardiac myxomas leads to numerous diagnostic and therapeutic difficulties. We present a case of a 67-year-old man with
fever of unknown origin
. Laboratory results disclosed signs of sepsis,
disseminated intravascular coagulation
, acute renal failure, and neuroinfection. Blood cultures were positive for methicillin-resistant Staphylococcus aureus. Transthoracic echocardiogram revealed enlarged left atrium filled with tumor with mobile structure protruding into the left ventricle during diastole. Abnormal intra-atrial mass were also apparent in chest computed tomography (CT). Because of sepsis complicated by coagulation disorders, surgical treatment was postponed. On the 11th day, a large retroperitoneal hematoma occurred, and it was punctured under ultrasonography (USG) control. After 37 days of intensive medical therapy, a stable patient was operated. A giant tumor fixed to the atrial wall between the pulmonary veins' orifices was excised and confirmed as myxoma in histological tests. The patient was discharged in good condition and was followed up for several months with no further complications.

A 78-year-old woman was admitted to our hospital with lumbago. Her activities of daily living had previously been completely independent. However, she developed temporary chills in January 2009, that improved without treatment, but recurred on February 7, 2009 in association with left lumbago and loss of appetite. She was then referred to our hospital with a disturbance of consciousness and high fever on February 14. A blood test performed on admission revealed an elevated inflammatory response, coagulation disorder and low platelet count, and abdominal computed tomography demonstrated findings suggestive of pyogenic spondylitis. The patient was therefore admitted and treated with antibiotic therapy; however, she died on day 8 due to complications of
disseminated intravascular coagulation
. An autopsy showed isolated pulmonary valve endocarditis. The patient's history was later found to include regular dental treatment, and the same Streptococcus group G was detected in cultures of the sputum, blood and vegetation.
