A 62-year-old man was admitted because of paresis of the legs and a bleeding tendency. He was diagnosed as metastatic bone cancer with
disseminated intravascular coagulation
(
DIC
). In spite of treatment, his general condition progressively deteriorated and he died of respiratory failure 13 days later. Autopsy revealed a carcinoma in adenoma in the rectum. Although the depth of cancer invasion was confined to the submucosal layer, disseminated
carcinomatosis
of the bone marrow and tumor emboli in blood vessels of the lung were present.

The authors report four cases of intracranial hemorrhage associated with nontraumatic
disseminated intravascular coagulation
(
DIC
). Two cases demonstrated a sudden onset of intracerebral hemorrhage. The other two showed chronic subdural hematoma initially, followed by acute multiple intracranial hemorrhages or general hemorrhagic diathesis. The underlying disorders were glioblastoma multiforme, thoracoabdominal aortic aneurysm, acute promyelocytic leukemia, and stomach cancer associated with disseminated
carcinomatosis
of the bone marrow. All patients died eventually. When the underlying disorder has a rare incidence of
DIC
as in glioblastoma multiforme or thoracoabdominal aortic aneurysm, the possibility of
DIC
and the need for immediate initiation of replacement therapy should be recognized, although the mortality is very high because the underlying disorder cannot be eliminated quickly. When the underlying disorder has a high incidence of
DIC
as in acute promyelocytic leukemia or disseminated
carcinomatosis
of the bone marrow, it is mandatory to start replacement therapy and treatment for the underlying disorder simultaneously.
DIC
can be controlled when the treatment for the underlying disorder is effective.

Reported are 2 cases of an advanced gastric cancer with a "diffuse
carcinomatosis
of the bone marrow" (DCMB) showing a widespread osteoplastic bone metastasis. In a DCBM, a widespread invasion of cancer cells to the bone marrow occurs, causing leukoerythroblastosis, anemia, and
disseminated intravascular coagulation
. The local reaction in the bone, namely being osteoplastic or osteolytic, has not been discussed in previous papers. Our cases were characterized by an extremely high serum alkaline phosphatase, a "superscan" view of the bone scintigram, and a diffuse osteoplastic bone metastasis with an osteoid increase. Thus we think there are DCBM subtypes with a diffuse osteoplastic bone metastasis.

Five cases of gastric carcinoma (one woman) were investigated. The average age of the patients was 44 and the clinical manifestation were lumbago, melena and fatigue. Two cases were Borrmann III, and other were Borrmann IV, IIc and IIc advanced, respectively. Three were signet-ring cell carcinoma and the other two were moderately differentiated and well-differentiated adenocarcinoma, respectively. All patients showed leukoerythroblastosis. Bone marrow specimens revealed myelofibrosis in three cases, while adenocarcinoma cells were also found in three. MHA or
DIC
was found in three patients, all of whom died of bleeding tendency. Mean survival time was 2.6 months. Thus, bone marrow
carcinomatosis
proved to be very severe.

A case of advanced gastric cancer associated with metastatic
carcinomatosis
of the bone marrow, microangiopathic hemolytic anemia (MAHA) and
disseminated intravascular coagulation
(
DIC
) was reported. A 71-year-old woman complained of lumbago and general fatigue. At the time of admission, besides anemia, jaundice and a tendency to bleeding, the laboratory data showed, hyperbilirubinemia, elevated FDP and hemolytic anemia with fragmented red cells. The bone marrow was infiltrated by carcinoma, and an upper GI series examination showed Borrmann II type gastric cancer on the greater curvature of the antrum. After remission of both MAHA and
DIC
by mitomycin C, neothramycin and FOY, the patient successfully underwent subtotal gastrectomy. However, she died of cerebral hemorrhage on the 96th postoperative day.

. Such subdural hemasma is supposed to be caused by tumor cell embolism in the dural vessels and

. Most cases show acute clinical symptoms such as consciousness disturbances, and their prognosis is extremely poor.

A 35-year-old woman tuffering from gastric cancer associated with disseminated
carcinomatosis
of the bone marrow is reported. Total gastrectomy combined with splenectomy and distal pancreatectomy was performed. The patient was treated with mitomycin C, FT-207, OK-432, and PSK. But serum ALP (alkaline phosphatase) and CEA (carcinoembryonic antigen) values showed gradual elevations followed by deterioration of the patient's general condition. Consequently, chemotherapy program consisting of 5-fluorouracil, Adriamycin (intra-arterially), and cisplatin (intravenously) was initiated. Serum CEA and ALP values were considerably improved, and patient was restored to a better condition. She survived 17 months and died of
disseminated intravascular coagulation

Twelve autopsy cases of
carcinomatosis
of the bone marrow were examined clinicopathologically. Among them, 7 were gastric adenocarcinoma, and the other 5 were a rectal carcinoid and carcinomas of the lung, prostate, maxilla and kidney, respectively. The gastric cancers were almost all poorly differentiated adenocarcinoma with mucin production and presented poorer prognoses than the other cancers. Leukoerythroblastic anemia, microangiopathic hemolytic anemia and
DIC
were found more frequently in the gastric cancers than in the others. It is concluded that the evolution of these critical hematologic disorders may be dependent on differences of histologic type, original focus and cancer-host interactions as well as wide-spread skeletal metastases of cancer cells.

A 63-year-old woman was in hospital for persistent backache. Four months prior to admission she had been pointed out as having hypertension for the first time. On admission, she had anemia (hemoglobin 7.0 g/dl) with reticulocytosis, and a blood smear showed fragmented erythrocytes. A bone marrow aspirate disclosed erythroid hyperplasia and invasion of cancer cells. The chest roentgenogram showed a coin-lesion of the right lung and left pleural effusion. A diagnosis of microangiopathic hemolytic anemia (MAHA) associated with
carcinomatosis
was made, but the primary site of the cancer was unknown. Respiratory failure developed and the patient died a month later. Surprisingly, the autopsy revealed a malignant pheochromocytoma arising from the right adrenal gland with massive metastases to the lungs, liver, lymph nodes and systemic bones, and also
disseminated intravascular coagulation
(
DIC
). The
DIC
would probably account for the MAHA in this case. To our knowledge, this is the first reported case of malignant pheochromocytoma accompanied by MAHA.

A 44-year-old woman with an unusual form of pulmonary metastasis is described. She presented with pulmonary thrombosis and clinical signs of
disseminated intravascular coagulation
(
DIC
) and died of cerebral hemorrhage. The autopsy study revealed transitional cell carcinoma of the left renal pelvis with pulmonary thrombosis in the large arteries. The intima of the vessels were intact on gross inspection except where the thrombi adhered to. The thrombi contained no tumor cells. However, microscopic examination identified that the metastatic carcinoma diffusely replaced the endothelium and proliferated on to the intimal surface without invasion of the wall and metastatic nodules in the parenchyma. Other examined organs had neither primary nor metastatic tumors, except for microscopic metastasis to the inferior vena cava. To date, this pattern of metastasis has not been noted in previous literature. This condition was designated as being vascular intimal
carcinomatosis
because of its characteristic manner of tumor proliferation on vascular intima.
