During 1992, 53 women were admitted to Lady Hardinge Medical College and Smt Sucheta Kriplani Hospital in New
Delhi
, India, for septic induced abortion and 1855 were admitted for medical termination of pregnancy (MTP). Most septic induced abortion cases lived in semi-urban/urban slums (64.15%), were Hindus (98.11%) and married (94.34%), and had a parity of less than 2 (56.6%). The leading reason for abortion was unwanted pregnancy among septic abortion cases (81%) and contraceptive failure among MTP cases (98.3%). No septic abortion case had used contraception. Among septic abortion cases, termination methods included instrumentation by untrained midwives (62%), foreign body insertion (7.5%), and dilatation and curettage or suction by unqualified personnel (7.5%). About 33% of septic abortion cases presented with generalized peritonitis, septicemia, septic shock, acute renal failure, or
disseminated intravascular coagulation
(
DIC
). All septic abortion cases had pelvic inflammatory disease compared to 3.55% among MTP cases. 94.35% had anemia. About 34% needed a blood transfusion compared to 0.16% among MTP cases. MTP cases were significantly less likely to suffer uterine perforation than septic abortion cases. None of the MTP cases had septicemia. The need for laparotomy was more common among septic abortion cases than MTP cases (26.4% vs. 0.43%). A hospital stay of more than one week was also more common (72% vs. 0.43%). On discharge, MTP cases were more likely to be in satisfactory condition than septic abortion cases (100% vs. 75.7%). No one in the MTP group died, while 13.2% died in the septic abortion group. The causes of death were septic shock (7 cases), hepato-renal failure (2 cases), and
DIC
(1 case). The abortion ratio was 312/1000 births. The abortion mortality ratio was 1.7/1000 abortions. Illegally induced abortions were responsible for 20% of all maternal deaths at this institution in 1992. These findings suggest that family planning education, contraceptive use, and safe pregnancy termination facilities would prevent abortion-related morbidity and mortality.

The association between coagulation abnormalities and pregnancy outcome was investigated in a case-control study conducted at Lady Hardings Medical College and Associated Hospitals in New
Delhi
, India, in 1991-92. Enrolled were 30 neonates born to mothers with pregnancy-induced hypertension (PIH) and 30 infants of normotensive mothers. Compared with control infants, infants of mothers with PIH had significantly elevated levels of prothrombin time (PT), partial thromboplastin time with kaolin (PTTK), thrombin time (TT), and fibrinogen and fibrinogen degradation products (FDP) and significantly reduced platelet counts and fibrinogen. PTTK and TT were significantly higher in neonates born to mothers with hypertension of more than 1 month's duration, but the difference in PT, fibrinogen, and platelet count was not significant. Also observed was a significant correlation between decreasing gestational age and derangement in all coagulation parameters and between decreasing birth weight and an increase in FDP level. disseminated intravascular coagulation
was higher in preterm than term neonates. 43.3% of neonates required admission to the neonatal intensive care unit and the perinatal mortality rate was 3.3%.

A dengue outbreak has recently hit the Indian capital. We studied the clinical profile of adult patients. Five hundred and sixty patients of dengue infection were admitted in a specially created ward according to the criteria laid down by WHO. Haematemesis (28.28%), epistaxis (26.78%) and malena (14.28%) were some of the common presentations. Similarly lymphadenopathy, especially cervical (30.89%), palatal rashes (26.96%) and hepatomegaly (23.75%) were the most commonly encountered findings on physical examination. Most of the cases were of dengue fever with haemorrhage and only 2.5% cases were classified under dengue haemorrhagic fever or dengue shock syndrome. The average hospital stay was 3.4 days but only 9.8 hours in the eleven patients who died, suggesting their late arrival in preterminal situation giving little time for resuscitation. Thrombocytopenia was not a feature and only 12.85% patients had platelet count less than 70,000/cmm. Most of the patients who were admitted with thrombocytopenia, showed normalization in their platelet counts in next few days. Serological examination demonstrated evidence of recent dengue infection in 41.17% patients. Few patients required blood or platelet concentrate transfusion. Eleven patients died, three due to

, one of intracranial haemorrhage and seven due to massive gastric haemorrhage. Rest of the patients recovered completely. Thus we can conclude that recent outbreak in
Delhi
was of dengue fever with haemorrhage and mortality was very low in patients who came early to the hospital.
