A female infant was delivered at term after biophysical and biochemical monitoring during the intrapartum period. At birth excessive bleeding was noted from the scalp incision sites, leading to severe anemia. The infant was also found to have congenital syphilis. The clinical course was complicated by the development of disseminated intravascular coagulation leading to death despite intensive management. Identification and appropriate management of excessive pre- and postnatal bleeding from a sampling incision is emphasized.

Congenital syphilis is a systemic infectious disease affecting and damaging many organs. It can be treated simply and effectively by penicillin. Our patient presented with sepsis and DIC, which is a rare manifestation, and to our knowledge this is the first reported case at the age of six weeks. We also review the symptoms of the disease focusing on the hematological manifestations of early congenital syphilis, diagnosis and treatment.

Disseminated intravascular coagulation (DIC) although a well known complication in neonatal sepsis is extremely rare in congenital syphilis and there are scanty reports of this entity in the literature. Intracranial bleeding following DIC in neonatal congenital syphilis is even rarer, and has been reported only once earlier. We are reporting the second case of neonatal DIC with intracranial haematoma due to congenital syphilis in a newborn. Our patient also had clinical and biochemical evidence of hepatitis which predisposes to DIC. Extensive investigations and emergent use of imaging modalities including ultrasound and CT scan led to early diagnosis and treatment in our patient, who could therefore be salvaged from an otherwise life threatening disease.
