We report the case of a 71-year-old man presenting a histologically proven liver cirrhosis with compensated disseminated intravascular coagulation and calcified aortic stenosis. This coagulation disorder became clinically and biologically severe during or soon after percutaneous balloon aortic valvuloplasty. In a cirrhotic patient with compensated disseminated intravascular coagulation, the procedure can induce life-threatening bleeding disorders.

Endocarditis has been traditionally classified as infective and non-bacterial thrombotic endocarditis (NBTE). NBTE has been associated with connective tissue diseases, chronic inflammatory processes and malignancy. During the pre-echocardiography era, the diagnosis of NBTE was most commonly determined at postmortem examination. A 63-year-old female patient with transient cerebral ischemic attacks and weight loss, who subsequently presented with aortic stenosis, is reported. She was treated for infective endocarditis, but developed a clinical picture of sepsis with disseminated intravascular coagulation. Despite aortic valve replacement, she died early postoperatively. Autopsy found valvular NBTE, multiple vascular thromboses and associated metastatic adenocarcinoma of unknown primary site. NBTE associated with malignancy April clinically mimic sepsis and infective endocarditis.

During the past few years, a new and attractive approach--transcatheter aortic valve implantation (TAVI)--has been developed for patients who are symptomatic of aortic stenosis and, due to the high expected operative risk, would not be otherwise treated. Unfortunately, TAVI can result in endocarditis of the percutaneously implanted valve that may present atypically and cause delays in diagnosis and treatment. Herein, the case is described of a 79-year-old female affected by endocarditis on aortic valve percutaneously implanted caused by Enterococcus faecium, complicated by iatrogenic pancytopenia and subacute disseminated intravascular coagulation, that proved fatal at six months after TAVI.
