Continuous measurement of lactate is potentially useful for detecting
physical exhaustion
and for monitoring critical care conditions characterized by hypoperfusion, such as heart failure. In some conditions, it may be desirable to monitor more than one metabolic parameter concurrently. For this reason, we designed and fabricated twisted wire-based microelectrodes that can measure both lactate and glucose. These dual-analyte sensors were characterized in vitro by measuring their response to the analyte of interest and to assess whether they were susceptible to interference from the other analyte. When measured in stirred aqueous buffer, lactate sensors detected a very small amount of crosstalk from glucose in vitro, although this signal was less than 3% of the response to lactate. Glucose sensors did not detect crosstalk from lactate. Sensors were implanted subcutaneously in rats and tested during infusions of lactate and glucose. Each sensing electrode responded rapidly to changes in its analyte concentration, and there was no evidence of in vivo crosstalk. This study constitutes proof of the concept that oxidase-based, amperometric wire microsensors can detect changes in glucose and lactate during subcutaneous implantation in rats.

The main purpose of the study was to examine if 3 months of bicycle endurance training alters the red cell deformability in non insulin dependent type 2 diabetes mellitus men.The red cell deformability was measured with the Laser assisted optical rotational cell analyzer. The maximal elongation index and the semimaximal shear stress were measured with the Lineweaver Burke model.At the beginning and the end of the intervention the patients passed a bicycle ergometry test. As a reference group, 13 males without
diabetes
passed the same testing procedure. Blood samplings were taken before testing, immediately after
physical exhaustion
and after a 30 min recovery phase.After the training period diabetic patients could significantly reduce BMI, fasting glucose and HbA1c. The reference group had significantly higher elongation indices than the
diabetes
patients independent from training status. After the training period the basal values of the maximal elongation index did not change significantly. However, maximal elongation indices were significantly reduced after physical examination and in resting time.The semimaximal shear stress of
diabetes
patients did not alter during the training period. In comparison to the reference group semimaximal shear stress was significantly reduced at all measurement times.This pilot study proves that the maximal elongation index is significantly decreased in
diabetes mellitus
patients. After 3 months endurance training the red cells become more rigid while the semimaximal shear stress remains constant. Further interventions are required to analyze the exact cause of the presented findings.

Pathogenic variants in the
MT-ATP6
are a well-known cause for maternally inherited mitochondrial disorders associated with a wide range of clinical phenotypes. Here, we present a 31- year old female with insulin-dependent
diabetes mellitus
, recurrent lactic acidosis and ketoacidosis recurrent infections with suspected immunodeficiency with T cell lymphopenia and hypogammaglobulinemia as well as proximal tetraparesis with severe muscle and limb pain and rapid
physical exhaustion
. Muscle biopsy and respiratory chain activities were normal. Single-exome sequencing revealed a variant in the
MT-ATP6
gene: m.9143T>C. Analysis of further specimen of the index and mother (segregation studies) revealed the highest mutation load in muscle (99% level of mtDNA heteroplasmy) of the index patient. Interestingly, acute metabolic and physical decompensation during recurrent illness was documented to be a common clinical feature in patients with
MT-ATP6
variants. However, it was not mentioned as a key symptom. Thus, we suggest that the clinical spectrum might be expanded in
ATP6
-associated diseases.
