Blood lipids, their alterations and methods of determination are reviewed. Serum values are compared in a series consisting of 20 cases of eye disease not caused by arteriosclerosis, 16 of chronic glaucoma, 14 of diabetes mellitus without retinopathy, 13 of occlusion of the central retinal artery, 16 of the retinal haemorrhage, 21 of retinal vein thrombosis, and 24 of sclerosis of the retinal fundus. Abnormal pictures, particularly with respect to the beta-alphaprotein ratio, were observed and it is suggested that their recognition and correction can serve as a guide in the prevention and treatment of arteriosclerosis and its ocular complications.

Forty-two patients with longstanding retinal vein thrombosis and 36 patients with diabetes had their blood viscosity levels measured. In both conditions blood viscosity, plasma viscosity, and plasma fibrinogen levels were significantly higher in those patients with capillary non-perfusion and/or new vessels on fluorescein angiography, compared to those patients without these complications. The possible role of abnormal blood viscosity in producing capillary non-perfusion and its sequelae in both conditions is discussed.

On the basis of literature review the role of fibrinolysis in the pathogenesis of central retinal vein thrombosis is presented. It has been proved by many authors that fibrinolytic activity is depressed in patients with vascular thrombosis, due to increased levels of fibrinolytic inhibitors mainly plasminogen activator inhibitor (PAI 1), and/or decreased levels of tissue plasmogen activator (t-PA). Low levels of t-PA and increased levels of PAI-1 have been found in states of special risk of thrombosis: obesity, diabetes mellitus, postoperative states, rheumatoid arthritis, malignancies and other diseases such as Behcet's syndrome. In pregnancy susceptibility to thrombosis follows the presence in blood of PAI-2, an inhibitor of plasminogen activators secreted by the placenta. Therapy is based on the use of anticoagulants (e.g. heparin) and thrombolytic agents (e.g. streptokinase).

A middle-aged lady presented with sudden onset of unilateral central retinal vein thrombosis after completing 6 months course of interferon and ribavirin for chronic hepatitis C infection. She had no risk factors and all her thrombophilia workup was normal, however, she was found to be dyslipidemic which may have contributed to atherosclerosis and predispose to thrombosis. Despite anticoagulation, her visual acuity deteriorated. This case illustrates the possibility of unpredictable visual complication of interferon. Frequent eye examination should be undertaken in patients having underlying risk factors like diabetes, hypertension or dyslipidemia undergoing interferon therapy.
