In this paper we review methods of cluster analysis in the context of classifying patients on the basis of clinical and/or laboratory type observations. Both hierarchical and non-hierarchical methods of clustering are considered, although the emphasis is on the latter type, with particular attention devoted to the mixture likelihood-based approach. For the purposes of dividing a given data set into g clusters, this approach fits a mixture model of g components, using the method of maximum likelihood. It thus provides a sound statistical basis for clustering. The important but difficult question of how many clusters are there in the data can be addressed within the framework of standard statistical theory, although theoretical and computational difficulties still remain. Two case studies, involving the cluster analysis of some
haemophilia
and
diabetes
data respectively, are reported to demonstrate the mixture likelihood-based approach to clustering.

Overall 995 children with different somatic chronic diseases were examined for viral hepatitis B markers demonstration using up-to-date highly sensitive methods (hemagglutination inhibition test, EIA). In the control group (children with acute intestinal infections), HBV-infection markers were discovered in 4.3%. Children with
diabetes mellitus
(13.1%), chronic renal diseases (18.9%), pulmonary diseases (32.8%), bronchial asthma (33.3%) and
hemophilia
(85.2%) are attributed to the group at greater risk for HBV infection. As a rule, the rate of HBV-infection markers demonstration in chronic somatic diseases was higher in considerable duration of the underlying illness. The overwhelming majority of the children examined had suffered subclinical forms of HBV-infection as shown by the disease history, whereupon they manifested antibodies against HBV antigens. HBs-antigenemia, that persisted for a long time (chronic HBV-infection) was demonstrable far less frequently. The authors provide evidence for the necessity of carrying out a broad-scale screening of HBV-infection markers in the indicated risk groups and vaccination against hepatitis B in children without HBsAG and without immunity to viral hepatitis B. The importance of measures aimed at preventing infections transmitted via blood is emphasized.

Eight hundred and fifty two cases, who were introduced from the Dental Hospital to Department of Internal medicine, Medical Hospital at Niigata, The Nippon Dental University, were examined for medical complications. About one hundred and fifty cases were introduced per year from the Dental Hospital, which consisted of about 15% of the total outpatients in the Department of Internal medicine. The age distribution showed a high incidence of patients in their 50's. The number of patients over 60 years old was 220 (25.8%), and the number of patients over 65 years old was 160 (18.8%). There was then a high percentage of elder patients. The reason for the initial medical examinations was the need for a pre-operative medical examination in 42.6% of the total cases. Concerning medical complications among the patients, the highest incidence was hypertension in 116 cases (13.6%); the second highest was hepato-biliary and pancreatic diseases in 114 cases (13.4%); and heart diseases in 81 cases (9.5%). Eighteen cases of HBs antigen positive (2.1%) were seen among the patients with liver diseases. Among those with blood diseases, anemia showed the highest incidence, and a small number had leukemia and

This study explored the hypothesis that Gilles de la Tourette's syndrome involves a disturbance in arousal modulation. The experimental group consisted of 20 unmedicated subjects with the Gilles de la Tourette's syndrome, and the control group of 20 subjects with chronic medical illnesses (

, von Willebrandt's disease and
diabetes
). There were differences between groups in the manner in which log conductance level changed over time during sound and light habituation experiments involving mild levels of stimulation with the Gilles de la Tourette group showing less change in arousal level over trials than the control group. No group differences were found in measures of phasic arousal, rate of spontaneous fluctuations and performance on two tasks that have been related to cortical arousal. It is suggested that the slower change in log conductance level in the Gilles de la Tourette group during the sound and light habituation experiments indicates that reticular activity is more persistent in these patients.
