Diabetes mellitus
, steatorrhea, cholelithiasis and a tumor distorting the duodenum prompted a work-up for somatostatinoma in a 52-year-old man. The responses of pancreatic B-cells but not of A-cells to nutrient stimuli were inhibited, and growth-hormone release was suppressed, suggesting somatostatin resistance in some target tissues. Plasma somatostatin-like immunoreactivity ranged from 9000 to 13,000 pg per milliliter (normal: 88+/-8, mean +/- S.E.M.) and was distributed in four molecular forms, including free somatostatin. The
primary tumor
contained 5 microgram of somatostatin-like immunoreactivity per milligram of wet tissue, distributed in three of the molecular forms noted in plasma. Plasma calcitonin was also elevated (4650 pg per milliliter; normal: less than 120). Immunocytochemical studies showed that cells of the
primary tumor
contained somatostatin and calcitonin but no other peptide hormones. Only somatostatin was present in the metastases. Somatostatin was localized electron microscopically in all secretory granules, irrespective of size and shape, whereas calcitonin was present only within a single subpopulation of small granules in the same cells.

Clinical effect of 5-fluorouracil or chromomycin-A3 alone, 5-fluorouracil + chromomycin-A3, and of the first two plus prednisolone on gastrointestinal and other solid tumors was evaluated. Out of 133 cases acceptable for evaluation, the number of responders was as follows: 3 (18.8%) of 6 cases treated with 5-fluorouracil alone, 1 (9.1%) of 11 cases treated with chromomycin-A3 or chromomycin-A3 hemisuccinate, 13 (21.7%) of 60 cases on the two-drug regimen, and 21 (45.7%) of 46 cases on the three-drug regimen. In cases of stomach carcinoma, response rate to the three-drug regimen was 54.2% (13/24), significantly higher than that of other regimens. At least 25% regression in the size of
primary tumor
was observed in 2 (7.1%) of 28 cases on the two-drug regimen and in 6 (33.3%) of 18 cases on the three-drug regimen. Of 51 cases on the three-drug regimen, steroid
diabetes
developed in 5 cases, moon face in 4 cases, and gastric ulcer in 1 case. However, toxic effect of these regimens (especially appearance of leucopenia) was less than those of previously tried combined regimens. The duration of response, on an average, was 10.8 weeks in 13 cases on the two-drug regimen and 11.7 weeks in 21 cases on the three-drug regimen. It was concluded from these results that a better response is obtained by the three-drug regimen than other regimens, and that prednisolone in combination, in addition to its favorable effect in improving the general condition of the patients, might enhance the anticancer effect of the drugs used in combination.

A rare case of renal cell carcinoma metastasizing to the choroid plexus of the lateral ventricle is reported. A 59-year-old woman was admitted to our institution on November 26, 1987 complaining of left-half headache for one month. She had a past history of right nephrectomy due to renal cell carcinoma 4 years before admission, and of right radical mastectomy due to breast cancer 10 years before admission. She had no abnormal neurological findings and laboratory results were normal. CT scan revealed a well-circumscribed, apparently homogeneously enhancing mass in the left lateral ventricle with mild ventricular dilatation. Left vertebral angiogram showed a tumor stain fed by the left posterior choroidal artery. The most likely diagnosis was thought to be metastasis of renal cell carcinoma. The patient underwent the operation for tumor removal via the posterior interhemispheric transcallosal approach 14 days after admission. A histological examination of the tumor determined the diagnosis of clear-cell type renal cell carcinoma. The operation was uneventful and the patient was discharged 20 days after operation without neurological deficit. But she had recent-memory disturbance, low activity, and gait disturbance in May 1989. CT scan revealed ventricular dilatation and tumor recurrence at the same site. She also suffered from
diabetes
due to the regrowth and invasion of
primary tumor
to the pancreas. She was discharged free of neurological defects after ventriculo-peritoneal shunt. In December 1989, she gradually deteriorated due to the regrowth of the intraventricular metastatic lesion, and now she is bedridden. Choroid plexus metastasis is quite rare, and, to our knowledge, only three cases have been described.

Central retinal artery occlusion (CRAO) is uncommon in those younger than 50. We present a 42-year-old man who complained of sudden loss of vision in the left eye. Fundus examination showed typical findings of CRAO. Extensive medical work up showed no evidence of
diabetes
or of cardiovascular, hematologic or collagen disease. However, a malignant tumor of the right testis was found, with metastases in the lung, retroperitoneal lymph nodes and brain. The right testis was resected and chemotherapy and brain irradiation resulted in remission during a follow-up of 2.5 years. The differential diagnosis of CRAO in younger patients is difficult. We believe that metastases from the
primary tumor
were the cause of CRAO in our patient. The possibility of malignancy with metastatic spread causing CRAO should be considered, particularly in relatively young patients with no other evidence of systemic disease.

The influence of
diabetes mellitus
on the course of breast cancer was investigated retrospectively in 752 patients. Possible unfavourable prognostic factors like overweight, lipid disorders, age and menopausal status were considered as confounders in a Cochran-Mantel-Haensel analysis. There was no difference in
primary tumor
status and lymph node involvement between patients with
diabetes mellitus
and nondiabetic patients. Diabetic patients had more often overweight, lipid disorders and were older than nondiabetic patients. Metastatic disease was highly significant correlated with
primary tumor
status (p less than 10(-6)) lymph node involvement (p less than 10(-10)) and
diabetes mellitus
(p less than 10(-5)). Overweight, lipid disorders, age and menopausal status were not correlated with metastatic disease. A possible explanation of the correlation between
diabetes mellitus
and metastatic disease could be hyperinsulinism in type IIB
diabetes

Seven patients with clinical features of the glucagonoma syndrome, including the characteristic rash,
diabetes mellitus
, and weight loss, were examined by CT. Computed tomography demonstrated a primary pancreatic tumor in all patients, and, in four, hepatic metastases were identified. The primary tumors, relatively large and solid in nature, varied in size from 2.5 to 6 cm in maximum diameter. The tumor was found in the tail of the pancreas in three patients and in the head of the pancreas in four. No obliteration of adjacent perivascular or peripancreatic fat planes was observed in any patient. Calcification was present in the
primary tumor
in three patients and in the hepatic metastases in one. All tumors that were studied angiographically were found to be hypervascular. In contrast to insulinomas, which are frequently quite small when clinically diagnosed, glucagonomas appear to attain considerable size prior to being clinically apparent. Thus, we conclude that CT ought to become the mainstay in the identification, localization, and staging of these tumors.

Early diagnosis of endometrial carcinoma enables one to achieve a cure rate of 80%. The purpose of this study is to characterize the patients who are prone to develop adenocarcinoma by simple epidemiologic and clinical data. The data of 109 consecutive patients who developed adenocarcinoma were compared with those of 146 control hysterectomy patients using logistic regression analysis. The following characteristics of patients who developed adenocarcinoma were identified: parity (P less than 0.0001),

(P less than 0.003), hypertension (P less than 0.0001), obesity (P less than 0.0006), treatment with exogenous estrogen (P less than 0.001), and second

. The logistic regression formula classified correctly 77% of all patients to their actual group. The study showed that the relative risk of a patient to develop adenocarcinoma can be estimated from simple and readily available epidemiologic and clinical data.

The relationship of clinically defined menstrual categories and an independent measure of hormonal stimulation, maturation index of vaginal smear cytology, was studied. Analysis of 596 smears obtained at the time of breast cancer diagnosis revealed a statistically significant association between menstrual status and maturation index. However, within each menstrual group varying levels of maturation were noted. Estrogenic effect in the absence of exogenous hormone administration was found in 11% of patients following bilateral oophorectomy and among 24% of women whose natural menopause occurred 20 years or longer prior to diagnosis. Endogenous estrogen production appears to continue for many years among some women. Clinical factors such as obesity,
diabetes
and/or hypertension may stimulate high squamous maturation in some patients. Others of the same age and with similar clinical histories were found to have atrophic smears. The differences in maturation index may be due to individual variations in: endogenous hormone levels; sensitivity of the vaginal mucosa to similar hormonal stimuli; use of certain medications; or unidentified exogenous factors. The maturation index was found to be significantly associated with the following prognostic factors: weight relative to height, tumor size and estrogen receptor content of the
primary tumor
. These findings indicate that vaginal smear cytology may define specific subsets within menstrual categories which may be relevant to therapy and prognosis in breast cancer.

A 64-year-old man with a six-month history of visual loss was found to have multiple darkly pigmented lesions scattered throughout the choroid of both eyes. Some lesions, up to several disc diameters in size, were flat, but others were elevated one to two millimeters. The right eye also contained a darkly pigmented ciliary body mass. Impairment of light transmission was striking. Ultrasonography of the elevated lesions demonstrated solid masses with high internal reflectivity. The initial diagnostic differential included the possibility of pigmented metastases from a
primary tumor
elsewhere, as well as multiple primary nevi. A general medical examination demonstrated arteriosclerotic heart disease, hypertension,
diabetes mellitus
, areas of patchy pigmentation of the oral mucosa, and a hilar prominence. A mediastinal lymph node biopsy specimen showed a grade 3 undifferentiated carcinoma compatible with a lung primary carcinoma. Enlargement of the choroidal lesions was observed until the patient's death 15 months later. Autopsy showed an undifferentiated carcinoma of the lung with widespread metastases. Each eye showed multiple discrete benign melanocytomas within a diffuse nevus involving the entire uveal tract. The nature of the relationship between multiple uveal melanocytomas and the systemic carcinoma is uncertain, but recognition of multiple uveal melanocytomas warrants a general medical examination to exclude primary malignancy elsewhere.
