A 4-year evaluation of the chronic toxicity of megestrol acetate in dogs is reported. .01, .1 or .25 mg of megestrol acetate/kg/day or .25 mg of chlormadinone acetate/kg/day was administered orally for 4 years t o female beagle dogs. The hormone-treated dogs tended to gain more weig ht than did the controls (controls vs. .25 mg megestrol acetate every month after the 3rd p less than .01). All treated dogs revealed decreased evidence of estrus. Mucoid vaginal discharges were more prevalent among the middle and high dose groups. Mean hemoglobin, packed cell volume and total erythrocyte values were slightly decreased while mean total leucocyte count and erythrocyte sedimentation rates were slightly increased in the middle and high dose groups. Clotting me chanism did not reveal any disturbances. Evidence of
diabetes
consistin g of
bilateral cataracts
, elevated serum glucose concentrations and glycosuria after 4 years in 2 of 16 high-dose megestrol acetate and in 6 of 15 chlormadinone acetate-treated dogs was revealed. It is concluded that the effects of megestrol acetate were similar but less severe than those of chlormadinone acetate.

The regional density of perfused cerebral capillaries (rDPC) and regional cerebral blood flow (rCBF) were measured in 12 selected brain regions in rats after 3 and 20 weeks of streptozotocin-induced
diabetes
and in control groups. After 3 weeks of

, both rCBF and rDPC were unchanged in the diabetic group compared to the control group. A

duration of 20 weeks causing
bilateral cataracts
induced a significant (p less than 0.05) reduction in rCBF in two structures in the visual system compared to the control group (visual cortex: 105 versus 129 ml 100 g-1 min-1; lateral geniculate body: 106 versus 128 ml 100 g-1 min-1) and in the pontine reticular nucleus (82 versus 128 ml 100 g min-1), whereas rDPC remained unchanged. A highly significant correlation between rCBF and rDPC was found in both control groups (r = 0.8, p less than 0.005) whereas the correlation was more scattered in the diabetic groups (r = 0.6, p less than 0.05). , a reduction of rCBF does not affect the number of perfused capillaries.

A 41-year-old woman was diagnosed as having Werner's syndrome associated with bladder cancer. The patient noticed sudden, total gross hematuria in September, 1985. Cystoscopy revealed a papillary tumor with a stalk that was accompanied by a daughter tumor and concealed the left ureteral orifice. The tumors were 25 X 20 X 10 mm and 15 X 10 X 5 mm. Double contrast cystograms, computed tomography and transurethral echo showed no invasion of muscle layer. Intravesical instillation of mitomycin (10 mg), cylocide (300 mg) and adriacin (30 mg) was carried out 3 times per week for 4 weeks. Tumor size was reduced, and then TUR was performed. High power section of the removed bladder tumor showed pathologically PNT, TCC, grade II, INF alpha, pTla, lyo and v(-). The patient had such clinical manifestations as short stature with low body weight, thin limbs and stocky trunk, senile face, early graying hair, highpitched voice,
bilateral cataracts
, osteoporosis, sclerodermia-like signs, flat feet, tendency toward
diabetes mellitus
and parental consanguinity. Hyaluronic acid was not detected in the urine. To the best of our knowledge, this seems to be the 30th report describing the association of malignancy with Werner's syndrome in Japan; besides, only one other case of bladder cancer in Werner's syndrome has been reported to date in the world.

Increased hemoglobin A1 (HbA1) occurs in
diabetes mellitus
as a direct consequence of hyperglycemia, which causes postsynthetic modification of hemoglobin proportional to blood glucose concentration. Glycosylation of hemoglobin is demonstrable also in vitro, not only by glucose, but with other reducing sugars such as galactose. The purpose of this study was to show that increased HbA1 could be induced by feeding rats a diet enriched either by galactose or sucrose. A third experimental group consisted of rats made diabetic by injection of streptozotocin. Exposure to a diet of 40% galactose resulted in a
diabetes
-like state with polyuria, weight loss, muscle atrophy, fat depletion,
bilateral cataracts
, peripheral neuropathy and significantly elevated HbA1. Blood glucose levels were not elevated and after four months of galactose feeding there was hypoglycemia. Total HbA1 was determined by a microcolumn chromatographic procedure, recently made available for clinical use. Increased HbA1 was also found in rats which received a diet of 68% sucrose for several months. The highest values of HbA1 were determined in streptozotocin diabetic rats and there was significant correlation with plasma glucose concentration. Development of experimental models for increased HbA1 by dietary exposure makes it possible to relate increases in HbA1 to measured amounts of sugar included in the diet. Increased HbA1 in galactosemic and diabetic rats provides a biochemical marker which may be correlated with the evolving morphologic changes such as basement membrane thickening occurring in certain tissues in

. In nine other paediatric CF clinics in the Republic of Ireland (total: 420 patients), three patients have DM, two receiving Insulin. Abnormal glucose tolerance is becoming more common in CF as patients survive longer. The possible role of corticosteroid treatment and intensive carbohydrate feeding regimes in development of glucose intolerance must be considered. DM in CF differs from the usual childhood DM. Regular screening and early Insulin supplementation may be beneficial.
