Diabetic cheiroarthropathy
(
DCA
) or pseudosclerodermatous hand of the diabetic is characterized by nonpainful limited extension of the proximal metacarpophalangeal and/or interphalangeal joints with spontaneous flexum of the fingers. The mechanism of lesion formation is poorly known but apparently associates neurogenic, vascular and cutaneous phenomena. Fifteen patients with
DCA
(9 men, 6 women; range 20-74 years) were studied by capillaroscopy, photoplethysmography and skin biopsy. Eleven had type 1 diabetes and 4 type 2 over periods ranging from 1 to 42 years (mean 19.9 years). Diabetic retinopathy was noted 10/15 times, nephropathy 5/15 times and neuropathy of the lower limbs 13/15 times. All patients had at least one of these abnormalities. In capillaroscopy, "Shoal of fish" features of diabetic microangiopathy were found only 4 times, but minor dystrophy was noted in 12 cases. In digital photoplethysmography, a drop in digital systolic pressure or an increase in pulse time was noted in 5 cases. The Hillestad test was less than or equal to 2 in 8 patients. Histological study showed constant dermal collagenous fibrosis in diseased skin, which was also found in normal skin in 6/13 patients. PAS staining showed a thickening of vascular basal membrane 14/15 times in diseased skin and 11/13 times in normal skin. The relation between

Diabetic cheiropathy
is often not diagnosed because of a paucity of symptoms. In the present study, 100 type I diabetic patients and 20 controls were investigated. The age of patients ranged from seven to 26 years, the duration of
diabetes
was one to 23 years. Extension of hands by "prayer position", a hand-print and angle determinations of joints were considered for the determination of diabetic cheiropathy. The presence of cheiropathy was unrelated to the duration of
diabetes
. However, it was well correlated with the quality of blood sugar control and it was often associated with diabetic retinopathy:
Diabetic cheiropathy
was detected in 53 (53%) of the diabetic patients, and in 26 (49%) of them diabetic retinopathy was also diagnosed. Only three patients had a retinopathy without cheiropathy. On the basis of our results, diabetic cheiropathy is a surprisingly common feature in young type I-diabetics. It is not to be considered as a late complication but rather a sequel of badly controlled
diabetes mellitus

Diabetic hand syndrome
is a condition affecting about 30% of patients with insulin-requiring juvenile
diabetes
. Characteristic findings in this syndrome are mild- to- moderately severe joint contractures of the fingers, particularly at the proximal interphalangeal joints in the ring and small fingers, and thickening of the skin of the dorsum of the hand. There is no evidence of palmar fascial thickening or Dupuytren's contracture. Occasionally other joints may be involved, such as the wrists, elbows, hips, knees, and toes. In the case presented in this report light and electron microscopic studies showed that the disease presents aspects similar to those of other "fibrotic diseases" as described by Kisher and Speer. Surgical treatment was only partially beneficial in this patient.
