Inflammation has been shown to be a risk factor for several chronic diseases. Few epidemiologic studies have examined the relationship between markers of inflammation and cancer. The current study included 7,674 Second National Health and Nutrition Examination Survey (NHANES II) participants, 30 to 74 years of age, between 1976 and 1980. Mortality follow-up through December 31, 1992 was assessed using the National Death Index and Social Security Administration Death Master File. A graded association between higher WBC and higher risk of total cancer mortality was observed [highest versus lowest quartile (relative risk [RR] 2.23; 95% confidence interval [CI], 1.53-3.23)] after adjusting for age, sex, and race. After further adjustment for smoking, physical activity, body mass index, alcohol intake, education, hematocrit, and
diabetes
, WBC remained significantly associated (P trend = 0.03) with total cancer mortality [highest versus lowest quartile (RR 1.66; 95% CI, 1.08-2.56)]. In stratified analyses, increased WBC was associated with higher risk of non-
lung cancer
(P trend = 0.04), but not
lung cancer
(P trend = 0.18). Among never smokers, a 1 SD increase in WBC (2.2 x 10(9) cells/L) was associated with greater risk of total (RR 1.32; 95% CI, 1.05-1.67) and non-lung (RR 1.30; 95% CI, 1.03-1.63) cancer mortality. These findings support the hypothesis that inflammation is an independent risk factor for cancer mortality. Additional studies are needed to determine whether circulating levels of inflammatory markers are associated with increased risk of incident cancer.

Apart from differences in lifestyle factors between urban and rural, access to medical resources may also be relevant to the differences within the province in chronic disease rates and in life expectancy.

To investigate the relationship between exposure to polycyclic aromatic hydrocarbons (PAHs) and mortality for specifc cancer sites, 1152 men, employed for at least 1 year at a prebake aluminium smelter, were followed-up from 1972 until 31 December 2001. Exposure to PAHs was estimated from a detailed reconstruction of the working history experienced in the plant by each cohort member and from several environmental and personal shift-sampling measurements available, by task and working department, since 1979. Furthermore, information on smoking habits, previous jobs before engagement in the smelter and main clinical findings observed during the follow-up were collected from the personal medical files. This study showed no increased mortality for
lung cancer
or bladder cancer associated to exposure to PAHs. Mortality for pancreatic cancer, based on 6 observed deaths, was significantly higher than expected in the whole cohort (SMR 2.4; 95%CI 1.1-5.2) and particularly among workers employed in the anodes factory of the plant (SMR 5.0, 95%CI 2.1-12.1), where a relatively consistent exposure to PAHs has been estimated. The nested case-control study planned for pancreatic cancer cases, confirmed that, also after controlling for cigarette smoking, PAH exposure experienced in the anodes factory was associated with a significant increased risk of pancreatic cancer. diabetes mellitus
and a potential occupational exposure to pesticides experienced in previous agricultural jobs were found as concurrent significant covariates increasing the risk. In conclusion, the relatively high exposure to PAHs, experienced in the anodes factory and particularly in the green-mill department of this prebake aluminium reduction plant, cannot be ruled out as one of the main factors in the multifactorial aetiology of the pancreatic cancers observed in this study.

It has been estimated that 30-40 percent of all cancers can be prevented by lifestyle and dietary measures alone. Obesity, nutrient sparse foods such as concentrated sugars and refined flour products that contribute to impaired glucose metabolism (which leads to
diabetes
), low fiber intake, consumption of red meat, and imbalance of omega 3 and omega 6 fats all contribute to excess cancer risk. Intake of flax seed, especially its lignan fraction, and abundant portions of fruits and vegetables will lower cancer risk. Allium and cruciferous vegetables are especially beneficial, with broccoli sprouts being the densest source of sulforophane. Protective elements in a cancer prevention diet include selenium, folic acid, vitamin B-12, vitamin D, chlorophyll, and antioxidants such as the carotenoids (alpha-carotene, beta-carotene, lycopene, lutein, cryptoxanthin). Ascorbic acid has limited benefits orally, but could be very beneficial intravenously. Supplementary use of oral digestive enzymes and probiotics also has merit as anticancer dietary measures. When a diet is compiled according to the guidelines here it is likely that there would be at least a 60-70 percent decrease in breast, colorectal, and prostate cancers, and even a 40-50 percent decrease in

Idiopathic pulmonary fibrosis (IPF) is a chronic fibrosing lung disease limited to the lungs and associated with the histologic appearance of usual interstitial pneumonia (UIP) on surgical lung biopsy. The estimated prevalence in the United States is between 35,000 and 55,000 cases,and evidence suggests that the prevalence is increasing for IPF. Risk factors associated with pulmonary fibrosis include smoking, environmental exposures, gastroesophageal reflux dis-ease, commonly prescribed drugs,

The diagnosis requires a careful history and physical examination, characteristic physiological and radiological studies, and, in some cases, a surgical lung biopsy. The natural history of IPF is not known, but evidence supports the concept of a continuum of idiopathic interstitial pneumonias that may overlap in time. , pulmonary embolism, infection, and other health problems. The median survival time for patients with IPF is less than 3 yr. In light of the poor prognosis and lack of response to available anti-inflammatory therapy, alternative approaches to therapy are being pursued. Emerging strategies to treat patients with IPF include agents that inhibit epithelial injury or enhance repair, anti-cytokine approaches, agents that inhibit fibroblast proliferation or induce fibroblast apoptosis, and other novel approaches.
