Fine structure of iris muscles from 16 iris specimens in patients with diabetes mellitus were investigated by electron microscopy, and the results were compared with those of 10 specimens of senile cataract eye without diabetes of the corresponding age groups. Significant ultrastructural changes were extensively found at the regions of the dilator, sphincter muscles and of the nerves endings adjacent to them. To cytoplasm of the muscle cells was partly occupied by membraneous structures with concentric lamellar appearance. Cytoplasmic vacuolations were noted in the muscle cells. An increased number of the lipid droplets associated with pigment granules in the cells was found in all diabetic iris specimens. These three kinds of pathological changes (lamellar structure, vacuole, and lipid droplet) increased depending on the duration as well as the control of the disease. Althought these findings were noted both in the sphincter and dilator muscles, the latter seemed to be more severely affected. Degenerative nerve endings containing lamellar membraneous structures were observed in a few moderate cases. Nerve fibers frequently diminished in diabetic patients, especially in severe cases with longer duration together with poorly controlled cases. These findings suggest that diabetes involves the iris muscle cells as well as the nerves, and these changes might be a reason to produce a pupillary abnormality.

Metabolic cataracts in diabetic patients usually develop within the first 3 years of the disease and cause little or no visual disability, but may progress to blindness. We report our findings in 3 patients who previously had normal vision and who presented with virtual blindness from cataract formation at the time of detection of their diabetes.

I have discussed five aspects of lens metabolism and their possible relationship to cataract in man, and this has left me with five fundamental questions to be answered. 1. A woman, aged 69, with latent diabetes was found to have retro-iridian pigment lines. Cataract extraction was followed by light microscopy and electron microscopy. Cross sections of the pigment lines showed these to consist of two main constituents, whose structures are described. The portion of the lines closest to the lens had a certain resemblance to the substance seen on the lens and in the iris in pseudo-exfoliation, while the remaining part of the pigment lines consisted of uniform microtubules.

Rates of hospital discharges for selected surgical procedures and some medical conditions were compared by socioeconomic status (SES) of patients for all 1972 hospital discharges from the 13 voluntary general hospitals located in Rhode Island. Socioeconomic status was determined by scoring the census tract of patient residence with such variables as median family income, housing status, etc. Many frequently performed surgical procedures had similar rates for the high, middle, low, and poverty socioeconomic groups. Procedures performed more frequently on the poverty than on the high SES group were tonsillectomy and upper gastrointestinal, heart, hemorrhoid, and cataract procedures. Dental procedures were performed more frequently on the high SES group. The over-all rate of hospital admissions showed a steady decline from the rate of 156 per 1,000 for the poverty census tracts to 104 for the high SES census tracts. This trend was reflected in the admission rates for several conditions requiring primary medical management, such as infections, diabetes, and skin problems. The population-based frame of reference used in this study can be a useful tool for raising pertinent questions for both professional standards review organizations and health planning groups.

It is not unusual to observe in the same eye a chronic glaucoma and a cataract. Surgery may be indicated for one of these diseases separately or for both of them. For simultaneous operation on glaucoma and cataract several attitudes could be discussed: a) cataract surgery and medical treatment of glaucoma, b) glaucoma surgery at first and then cataract surgery, c) cataract and glaucoma surgery at the same time. Since trabeculectomy is used for glaucoma surgery, the author performs the combined method, i.e. the simultaneous lens extraction and the operation of glaucoma through trabeculectomy. The advantages of trabec-lectomy over the other techniques for glaucoma surgery indicated that trabeculectomy over the other techniques for glaucoma surgery indicated that trabeculectomy is the best technique to be used in association with intracapsular cataract extraction. Our material consists of 65 patients, amongst which there are 11 patients with diabetes. 91 eyes have been operated upon during the last 2 years. Most of the postoperative complications, i.e. hyphaema, vitreous haemorrhage and prolapse, pupillary block, postoperatve rise of i.o. pressure are not severe and have no influence in the desired fall of i.o. pressure. It may be pointed out that a permanent regulation of i.o. pressure often occurs only 3 months postoperatively. Finally it can be said that the trabeculectomy associated with intracapsular cataract extraction appeared to be the best operation in case of glaucoma and cataract, provided that on the one hand the vascular status of these patients is controlled and on the other hand the operation microscope is exclusively used in surgery of the trabeculum.

The postoperative course of 141 eyes in 115 patients undergoing uncomplicated intracapsular cataract extraction was reviewed. Alpha-chymotrypsin as used in our institution did not increase the percentage of eyes developing significant postoperative ocular hypertension. Smaller amounts of enzyme and a shorter waiting period than have heretofore been advocated are adequate for clinically effective zonulysis and appear to be protective against "enzyme glaucoma." Indeed, we found that among non-diabetics, those receiving enzyme experienced a lower incidence of postoperative ocular hypertension than did those not receiving enzyme. The one factor in our series related to an increased percentage of postoperative hypertensive responses was diabetes mellitus.

A study of hospital discharge diagnoses from both national data and data from a local medical centre indicates that diabetes substantially increases the probability of cataract extraction at age 40-49, about doubles or triples the probability for age 50-69, and has little effect on risk at age 70 and over. Strengths and weaknesses of the data are discussed. Other reports, generally estimating a much stronger association between diabetes and probability of cataract extraction, at least at age 50 and above, are critically evaluated.

Onchocerca Control Project (OCP) was formed in 1974 under WHO for the control of oncocerciasis by the funds of developed countries. The control of vector (simulium) as well as new drug are giving the the project the prospect of success in eradicating the disease, thus preventing the blindness by diseases. The situation on blindness by trachoma, childhood blindness, glaucoma, diabetes will be discussed. The progress of molecular genetics of eye disease may open the gate for prevention of blindness by these disease in future.
