The urinary bladder and urethral content of substance P and vasoactive intestinal polypeptide and the in vitro effects of the peptides on the bladder were studied at 6 weeks and 6 months of streptozotocin-induced
diabetes
in the rat. The results were compared with those obtained in age-matched control animals. Both short-term and long-term streptozotocin treatment induced a clearcut increase in bladder weight. Bladder substance P content was increased in both groups of diabetic animals but substance P concentration was similar in control and diabetic animals. Vasoactive intestinal polypeptide content was slightly higher in diabetic animals than in controls but vasoactive intestinal polypeptide concentration was significantly lower in the bladders from both short-term and long-term diabetic animals. The bladder contractile response to substance P was similar in all groups of animals and vasoactive intestinal polypeptide was found to be devoid of contractile or relaxatory effects in the rat bladder. No change in urethral weight was seen with
diabetes
. There were no clear-cut changes in the urethral contents or concentrations of substance P and vasoactive intestinal polypeptide. The study also enabled comparisons between younger (3 months) and older (9 months) rats. This comparison showed a decrease in the concentrations and contents of substance P and vasoactive intestinal polypeptide between young and older rats. The changes were seen in both the bladder and the
urethra
and were similar in diabetic and normal animals.

The effect of double Malecot type polyurethane intraurethral catheter (IUC) was examined in 17 benign prostatic hypertrophy patients who were unfit for operation and suffered from urinary retention. Patients were aged 68 to 90 (mean 80.5) years old and the causes of IUC insertion were cardiac, cerebrovascular, respiratory and gastrointestinal diseases,
diabetes mellitus
and aging. IUC was selected among three types (55, 60, 65 mm) according to the length of prostatic
urethra
. Insertion of IUC was carried out easily under fluoroscopic guidance without endoscopy. All patients could void by themselves just after insertion of IUC and the longest indwelling period was 10 months. The length of IUC need not be longer than that of prostatic
urethra
and patients with normal or hypertonic bladder could void better than those with atonic bladder. Urinary tract infection did not get worse in any patients with indwelling IUC. Double Malecot type polyurethane IUC is a safe and an effective alternative method in place of urethral balloon catheter for inoperable prostatic hypertrophy patients in urinary retention.

The impairment of sympathetic innervation of the urinary bladder neck was supposed to be a cause of the syndrome of diabetic retrograde ejaculation (DRE). The performance of the sphincter system had not been studied in this category of patients. The liquid profilometric technique was used to examine 3 groups of males: 8 patients with the DRE syndrome; 5 patients with
diabetes mellitus
(DM) without ejaculation disorders; and 7 apparently healthy subjects. All the examinees showed no organic changes in the prostate and
urethra
. The groups were matched by age, type, duration and severity of DM. The DRE syndrome patients had no elevation of intraureteral pressure in the area the inner sphincter of the urinary bladder, which evidenced its atony. In health the elevation of vesical pressure is usually accompanied by an increase in ureteral resistance (r = 0.96; p less than 0.001), thus maintaining the stability of the positive pressure gradient and preventing the escape of urine. Correlation analysis revealed alterations of interrelations between intravesical and sphincter pressures in
diabetes mellitus
patients. This phenomenon evidenced the disorders of somatic innervation of the outer ureteral sphincter more pronounced in patients with the DRE syndrome. In this retrospective study on 226 patients with bladder cancer before radiotherapy following diagnostic or conservative surgery, bacterial infection of the urine was evaluated. The incidence of urine infection was 65.5%. 12.3% of the patients had sterile urine and in 22.1% of patients the investigation for urine infection was not performed. 77% of infections were caused by Gram-negative bacilli. No dependence of infection incidence on the tumor-T-stage, incidence of
diabetes mellitus
, prostatic gland size and tumor location near the internal ostium of the
urethra
was found. Thus, the cancer itself, transurethral diagnostic and/or transurethral or abdominal operation and the hospital stay appear to be of primary importance for the development of infection. To prevent serious late postradiation reactions it is important to manage infection before the start of radiotherapy. Some features of infection therapy in bladder cancer patients are discussed.

, aspermatism (51.2, 27.9, 20.9% of patients, respectively). The leading pathogenetic factors are peripheral neuropathy, surgical injury to the bladder internal sphincter, psychosexual disorders. The authors propose classification and algorithm of the patients' examination to facilitate and enhance diagnosis. Special attention should be given to the presence or absence of orgasm, after-orgasm occurrence of spermatozoa in urine. Anejaculation treatment outcomes remain unsatisfactory. Good responses were achieved with selective adrenomimetic gutron, laser therapy and electrovibration. Prevention of anejaculation should be given more attention.

The present study deals with the diabetic neuropathies prevailing in the male population. In this investigation 100 insulin-dependent
diabetes mellitus
(IDDM) and 314 non-insulin-dependent
diabetes mellitus
(NIDDM) patients with and without an objective evidence of neuropathy, having an age span of 15 to 80 years and the duration of
diabetes
distributed over 1-33 years were included along with age-matched nondiabetic controls. The diabetic subjects were evaluated for semen analysis. Results of semen analysis showed a highly significant increase (p > .0005) in total sperm output and sperm concentration in both IDDM and NIDDM neuropathic diabetic men. On the other hand, sperm motility and semen volume were found to be about 30 and 60% less, respectively, in IDDM and NIDDM patients, where as sperm morphology and quality of sperm motility remained unaffected. A comparison between IDDM and NIDDM neuropathic and non-neuropathic diabetic groups further indicated a nonsignificant difference in the parameters of semen analysis, thus suggesting an endocrine basis for the sexual disturbances of diabetic neuropathy. A significant rise in total sperm output in both IDDM and NIDDM neuropathic diabetic patients and a significant decrease in semen volume in both types of diabetic patients thus suggests some kind of Leydig cell hyperplasia, which in turn may stimulate spermatogenesis and atonia of the bladder and
urethra
, resulting in retrograde ejaculation.
