186 patients with periarthritis of the shoulder have been studied. The sex ratio was female:male, 1-52:1. The peak age of onset was 54-59 years in both sexes. Over 40% of the patients were referred to the clinic after 6 months had elapsed from the time of onset of the disease. The right shoulder was more frequently involved than the left, particularly in the men. One shoulder only was affected in 75% of patients. There was frequently a previous history of '
rheumatism
' before the episode of periarthritis. In one-third of the women 'nonspecific
rheumatism
' had occurred. Cervicobrachial pain and a previous episode of shoulder pain had occurred more often in the women. There were a number of associated diseases, ischaemic heart disease, thyroid disease among women,

among women, hemiplegia, pulmonary tuberculosis, chronic bronchitis, and epilepsy. Acute trauma was rarely a precipitating factor. Manual workers were more frequently seen than sedentary workers in the sample, and there were more in the sample than in the general population of Leeds. The general psychological background was no different from a control group. The Maudsley Personality Inventory gave no different results among patients with periarthritis of the shoulder than among a control group and among the general population. It is suggested that there is no evidence in this study for a 'periarthritic personality'. It is suggested that the cause of periarthritis of the shoulder is likely to be related to chronic trauma occurring in an age range when changes in connective tissue are occurring. Certain associated diseases may predispose the patient to this disorder.

In the Tri-State Leukemia Survey, the history of diseases in 605 adult male leukemia cases 15 years and older and in 668 adult male population controls was examined. These diseases occurred at least 1 year before leukemia was diagnosed. The data were based on respondents' answers that the disease was diagnosed by a physician; the respondent was either the subject or his spouse. Of 30 diseases studied, 7 showed an excess among the patients with leukemia: infectious hepatitis, eczema, psoriasis,

, arthritis and

, heart disease, and ankylosing spondylitis. Mumps had a lower reported occurrence among the cases, whereas pneumonia was less frequent in acute lymphatic cases than in population controls. Three diseases occurred significantly less in controls than in persons with specific histologic types of leukemia. Our data revealed a more frequent history of herpes zoster (shingles) in chronic lymphatic leukemia, more hives in acute chronic myeloid cases, and meningitis in acute myeloid leukemia. When we only considered the patients' responses, more of them admitted having had acne than did our controls. The remaining diseases--childhood viral diseases, infectious mononucleosis, smallpox, typhoid fever, dysentery, scarlet fever, tuberculosis, asthma, hay fever, and goiter did not occur more frequently in cases than in controls. The findings were consistent with evidence from previous laboratory and clinical studies. The increased occurrence of infectious hepatitis in our case series is consistent with the findings of other studies showing an increased frequency of Australia antigen in patients with hepatitis, leukemia, and Down's syndrome.

Between September 1983 and July 1985, a case-control study was performed of carpal tunnel syndrome risk factors in the general population of Maastricht, The Netherlands, and some surrounding villages. Twenty-eight of the 501 participants were found to suffer from carpal tunnel syndrome. These 28 were added to a series of 128 consecutive carpal tunnel syndrome patients from the same area. The 156 (131 women and 25 men) subjects in whom carpal tunnel syndrome had been diagnosed on the basis of clinical history and neurophysiologic testing were compared with the remaining 473 (310 women and 163 men) subjects. After adjustment for age and sex, the following carpal tunnel syndrome risk factors could be identified: activities with a flexed wrist or with an extended wrist (exposure-related increased risk), hysterectomy without oophorectomy, last menstrual period in menopausal women 6-12 months ago, height, weight, Quetelet index, slimming courses, and in men, varicosis. Teucrium polium is widely used by the folk-medicine practitioners in Saudi Arabia for the treatment of inflammations,
rheumatism

diabetes
and ulcers. Scientific reports are lacking to confirm these activities. The present study reports the effect of ethanolic extract of T. polium on carrageenan-induced acute inflammation, cotton-pellet granuloma and some of the biochemical parameters. The ethanolic extract of Teucrium polium at a dose of 500 mg/kg body weight produced significant inhibition of carrageenan-induced inflammation and cotton-pellet granuloma. Biochemical studies showed a significant decrease in glucose level. The presence of flavonoids and sterols might be responsible for the anti-inflammatory activity of this plant. Further studies on the fractionation of the phytoconstituents, and on their mechanism of action are in progress.

Federal recognition of the tribal status of the Klamath Indians of Oregon was terminated by Congress in 1954, along with all health, education, and welfare services. In the winter and spring of 1985 a health status and health care needs assessment was conducted among 202 Klamath Indians ages 40 years and older with the use of a shortened version of the Older Americans Resources and Services (OARS) instrument. Twenty percent of the Klamaths surveyed reported having

, and more than 30 percent reported having arthritis,

, or hypertension, or having had their gallbladder removed. The data were compared with those of national surveys of Indian and non-Indian elders that also used the OARS instrument. Even though the Klamaths surveyed were younger than the comparison groups, their health status was no better than that of other Indians and was worse than that of the non-Indian population. Moreover, among these Klamath adults, health insurance coverage was lower, and perceived unmet needs for medical care were higher than in either of the comparison groups.

By the end of March, 1987, 550 couples had been enrolled in the Habitual Miscarriage Outpatient Department at Keio University Hospital in Japan. The average rate of miscarriages during the past 8 years at this hospital was 16.12%. Average number of miscarriages suffered by those couples was 3.09 +- 1.13. Average age of the husband was 34.5 +- 0.3, and that of the wife was 32.8 +- 0.3. All of those who miscarried 2 or 3 times underwent thorough clinical tests. The results showed organic disorders such as uterine myoma (22), arched uterus (16), cervical incompetency (14), double uterus (9), endocrine abnormalities such as hyperthyroidism (6) and hypothyroidism (7), metabolic disorders such as

Testings of reproductive wastages both at fertilization and nidation did not yield significant results because of defective methods. Surgical removal of uterine myoma, plastic unification of double uterus, and immunological therapy were among the methods used for treating infertility. Immunotherapy used at Keio University Hospital is as follows: 30 ml venum is taken from the husband aseptically; lymphocyte is isolated from the venum via Ficoll conray; its concentration is adjusted to 3.0-4.0X107/ml; intracutaneous injection of it is given to his wife every 2 - 4 weeks. This therapy resulted in 96 cases of pregnancy among 150 couples. 3 year follow-up study on 42 births show that those babies are healthy and normal in every respect.

The possibility of bilateral femoral neuropathy after microsurgical tuboplasty for the reversal of sterilization is possible. There seems to be little awareness of this condition by gynecologists and fertility surgeons. This type of femoral neuropathy has an excellent prognosis and only physiotherapy is necessary to aid muscular function. Some cases have been reported where recovery has been extremely slow, normal functions had taken months, and some disability lasted years. The femoral nerve is not included in the pelvis, therefore injury through operative procedures are unlikely. The self-retaining retractors were used in all reported cases and verified through clinical experience. There are 2 types of injury to the femoral nerve: Direct pressure on the nerve itself by retractor blades, and impingement of the psoas muscle and the nerve against the lateral pelvic muscle.
