Recent concepts on the mechanisms of aging of extracellular matrix (EM) are reviewed as well as its involvement in age-associated diseases. Cell differentiation, histogenesis and organogenesis can be analyzed in terms of the program of the biosynthesis of EM macromolecules during development, maturation and aging. The most important biological role of EM is the integration of cells in tissues, of tissues in organs and of organs in the whole organism. EM can directly influence cell behavior through the contact between EM and the genome mediated by structural glycoproteins (fibronectin, laminin, elastonectin, etc.) interacting with other EM macromolecules (collagen, proteoglycans, elastin) and the cytoskeleton by trans-membrane receptors (integrins). Most age-associated diseases exhibit a deviation (qualitative or quantitative) from the normal program of EM biosynthesis. Three examples are analyzed in some detail: atherosclerosis, diabetes and malignant tumors. The degradation of elastic fibers catalyzed by cellular elastase-type enzymes is observed in atherosclerosis and also in emphysema and skin aging. Several of these enzymes were isolated and characterized from platelets, fibroblasts, smooth muscle cells and lipoproteins. The biosynthesis of some of them increases with age and facilitates cell migration. Plasma fibronectin increases with age exponentially. This increase is absent or strongly attenuated in diabetes and some cancers. Tissue fibronectin increases in diabetes, Werner syndrome and in the peritumoral desmoplastic reaction while most tumor cells can no more retain fibronectin on their membrane facilitating their movement in the organism. These examples demonstrate the importance of the study of cell matrix interactions for gerontology.

In order to test the hypothesis that Roman Catholic priests are at low risk for prostatic cancer because of their celibacy, a cohort of 10,026 men who were active or retired diocesan (parish) Roman Catholic priests in the United States on January 1, 1949 were followed until death, leaving the priesthood, or January 1, 1978. The overall standardized mortality ratio (SMR) was 103 and the SMR for cancer of the prostate was 81. Other interesting findings include increased SMRs for cancer of the larynx (147), cirrhosis of the liver (147), and diabetes (182) and decreased SMRs for lung cancer (59), emphysema (26), and suicide (13).

PMID:Retrospective cohort mortality study of Roman Catholic priests. 340 88

A retrospective cohort study was conducted to examine mortality among 18,811 male farm owners and operators in New York State from 1973-1984. Farm Bureau membership lists were used to identify the study population, and vital status was determined through record linkage with death certificate and motor vehicle files. The comparison group consisted of the 1980 United States Census population of men who resided in the same towns as did the farmers. The results indicated that the study cohort experienced fewer than the expected numbers of deaths overall and for each major cause category except accidents. Specific causes with significant mortality deficits included cancer of the lung (standardized mortality ratio [SMR] = 47.0); diabetes mellitus (SMR = 57.5); ischemic heart disease (SMR = 65.3); bronchitis, emphysema, and asthma (SMR = 26.7); and cirrhosis of the liver (SMR = 29.7). The only specific cause with a significantly elevated mortality was accidents other than motor vehicle (SMR = 146.5). The investigation differs from previous research in method, setting, and population, but the pattern of findings is generally consistent with that of other studies.

A proportionate mortality ratio (PMR) study was undertaken of 7,121 members and retirees of the United Association of Plumbers and Pipefitters in California who died in 1960-79. The PMR for all malignant neoplasms was 1.24, with a major contribution from lung cancers (PMR = 1.41). Lung cancer PMRs were consistently elevated, through the 20-year study period, across the pipe trades and within different birth cohorts. Sixteen mesothelioma deaths occurred, suggesting asbestos as a risk factor. PMRs for malignancies of the stomach, kidney, brain, and lymphopoietic system were also elevated, especially among plumbers. Chronic rheumatic heart disease, emphysema, liver cirrhosis, and all external causes of death were the major non-cancer causes with significantly elevated PMRs. There were significant deficits in diabetes mellitus, all pneumonia, chronic nephritis, and vascular lesions of the central nervous system (CNS). PMRs for successive birth cohorts among all study subjects revealed decreasing emphysema risk, suggesting previous reduction of a risk factor for this disease. Among plumbers, PMRs for death due to several non-respiratory malignancies showed an increasing trend with recency of birth cohort.

The basal lamina is an extracellular scaffold positioned between parenchymal cells and connective tissue. Parenchymal cells attach to one of its surfaces and the other is anchored to connective tissue. By its presence it defines the spatial relationships among similar and dissimilar types of cells and between these cells and the space occupied by connective and supportive tissues. Replenishment of cells which have died during normal functioning or have become damaged in course of injury occurs with new cells in an orderly way along the framework of the basal lamina scaffold. This process appears to be aided by the polarity of the basal lamina and by an apparent specificity for cell types, and it enables multicellular organisms to reconstitute histologic structures of most tissues and organs to what they were prior to loss of cells. If the basal lamina is destroyed, the healing in most tissues results in formation of scar and loss of function. The properties of the basal lamina concerned with maintenance of histologic order in organs and tissues offer new ways to interpret the pathogenesis of several common disorders, including emphysema, scars, adhesions, cirrhosis of liver and excessive accumulation of basal lamina material as, for example, it occurs in patients with diabetes mellitus.

Ceftizoxime (CZX), a parenteral cephalosporin derivative belonging to the so-called third generation cephalosporin is reported to have a broad antibacterial activity, particularly against Gram-negative aerobic bacilli and some anaerobes, such as Bacteroides fragilis and a good stability to beta-lactamases. Clinical study was performed on a total of 20 cases, 9 females (1 case had urinary tract infection 3 times) and 11 males, aged from 27 to 82 years. All patients had the underlying diseases. They were bronchial asthma in 3 cases, influenza in 1, chronic pulmonary emphysema in 1, pulmonary fibrosis in 1, chronic bronchitis with strongyloidiasis in 1, lung cancer in 3, esophagus cancer in 2, stomach cancer in 1, hepatoma with urolithiasis in 1, liver cirrhosis with diabetes mellitus in 1, alcoholism with strongyloidiasis in 1, cholelithiasis in 1 and congestive heart failure in 1, respectively. Clinical diagnoses for infections were 2-acute bronchitis, 2-exacerbation of chronic bronchitis, 2-broncho-pneumonia, 2-pneumonia including one suspected case, 1-obstructive pneumonia, 2-secondary pulmonary infection, 1-pulmonary infection, 3-urinary tract infection (UTI), 1-UTI with sepsis, 1-sepsis, 1-sepsis with purulent meningitis, 1-biliary tract infection and 1-infected bronchoesophageal fistula. CZX was given by intravenous drip infusion, at a dose of 1 to 2 g, twice daily for 3 to 15 days. Because of severity in infections and underlying diseases, some cases were treated either steroid, gamma-globulin preparations or other antibiotics in combination with CZX. Twelve out of 15 cases assessed clinically responded satisfactorily to the treatment and efficacy rate was 80.0%.(ABSTRACT TRUNCATED AT 250 WORDS)

Plasma cyclic AMP responses to adrenaline administration in normal volunteers, patients with spinocerebellar degeneration, bronchial asthma, pulmonary emphysema, and diabetes mellitus were studied. Intramuscular administration of low doses (0.1--0.4 mg/person) of adrenaline caused a dose-dependent increase in plasma cyclic AMP. The increase in cyclic AMP was completely prevented by propranolol, while it was not affected by phentolamine or atropine. In patients with spinocerebellar degeneration, the concentrations of plasma cyclic AMP both before and after adrenaline administration were lower than in normal subjects. In asthmatic patients, the plasma cyclic AMP increase after adrenaline administration was smaller than that of the healthy controls. The plasma concentration of cyclic AMP in patients with insulin-dependent diabetes reached the peak level more slowly than in diabetic patients with dietary control alone. Examining changes in the plasma cyclic AMP level after adrenaline administration appears to be a useful means for assessing the degree of beta-adrenergic dysfunction.

Elastic fibers are progressively lysed during maturation and aging and in an accelerated fashion in several aging diseases such as diabetes, arteriosclerosis, emphysema and several skin diseases. Several enzymes (elastase-type proteases) were isolated in recent years in our laboratory which appear to be involved in these processes. A cell membrane bound serine protease was isolated from arterial smooth muscle cells and was shown to increase with in vitro aging of the cells. A metallo-protease was isolated from skin fibroblasts and was shown to be capable of attacking the constituents of elastic fibers, mainly the microfibrillar glycoproteins and also the desmosine cross linked elastin in vivo. This partially purified fibroblast enzyme was shown to attack these elastic fibers when injected into the dermis. A new selective staining procedure was used to visualise and quantitate, by computerized image analysis, the skin elastic fibers in normal and pathological human or animal skin biopsies. This method, combined with the injection of elastase in rabbit skins, alone or together with inhibitors, enables the ex vivo/in vivo study of elastase action (and of its inhibition).

This study examined the effects of long-term experimental dysthyroidism on the enzymatic character of the costal diaphragm and selected respiratory parameters. Costal diaphragms from thyroidectomized (TX), euthyroid (EU), and hyperthyroid (HT) male albino rats were used. HT was induced by subcutaneous injections of triiodothyronine on alternate days for 6 wk. Minute ventilation was greater for the HT (70%) compared with the TX rats. The enzymatic potentials of glycolysis (28%), tricarboxylic acid cycle (30%), and fatty acid oxidation (16%) were significantly increased in the HT diaphragms, whereas the potentials were lower by a similar relative extent in the TX diaphragms. The proportion of alkali-labile fibers were greater in the TX and lower in the HT diaphragm. The shifts in heart and muscle lactate dehydrogenase isoenzyme activities were consistent with the fiber type changes. These findings show that dysthyroidism modifies the overall enzymatic capacity of the diaphragm (i.e., glycolysis, tricarboxylic acid cycle, and fatty acid oxidation) along with the proportion of alkali-labile to alkali-stable fiber types. These enzymatic changes are similar to those resulting from exercise training, tracheal banding, streptozotocin diabetes, and emphysema.

The incidence and treatment of empyema historically have fluctuated with the introduction of new antibiotics. As resistant strains of bacteria emerge, a return to aggressive surgical therapy becomes necessary. Empyemas are most likely to occur in patients with an underlying factor such as alcoholism, bronchitis, asthma, emphysema, diabetes, tuberculosis, carcinoma, heroin addiction, or steroid therapy. The bacteriology is constantly changing. Recently, the importance of anaerobic organisms--which are now involved in three out of four cases of empyema--has been recognized. Diagnosis is established and antibiotics chosen on the basis of Gram staining and culture of pleural fluid. Surgical procedures include thoracentesis, closed chest tube drainage, open drainage plus rib resection, decortication, thoracoplasty, and excision of the empyema sac with an extrapleural dissection.
