Organic causes for nocturnal or recumbency leg cramps as venous, arterial, statical, nervous, rheumatical and metabolic diseases such as tetany,
diabetes
and gout must be excluded. This means also electrolyte disturbances during a medical treatment, for example with diuretics. In addition there is a very high portion of idiopathic cramps. In 22 patients with
nocturnal leg cramps
a four week double blind study with a combination of quinine sulphate and aminophylline (Limptar) was started. Unequivocally, Limptar significantly reduced the number and the intensity of nocturnal or recumbency leg cramps. There was no influence on angiological and biochemical parameters. Side effects appeared very seldom.

l-carnitine, a compound responsible for transportation of acyl groups across cell membranes and modulating intracellular acyl-coenzyme A levels, is reported to reduce muscle cramps in patients with liver cirrhosis and
diabetes
and those on dialysis. A 79-y-old man with right-sided paralysis was admitted to our hospital and diagnosed with cerebral infarction. Nocturnal leg cramps appeared in the affected side and caused sleep disturbance. Supplementation with l-carnitine reduced the number of
nocturnal leg cramps
and alleviated sleep disturbance. It also plays an important role in nerve protection and treatment for carnitine deficiency. Patients with stroke-induced paralysis experience muscle wasting, which might reduce pooled carnitine in the affected side. This case suggests that stroke may cause localized carnitine deficiency, and l-carnitine supplementation might be effective for muscle cramps induced by stroke. To the best of our knowledge, this is the first case of l-carnitine supplementation for muscle cramps triggered by cerebral infarction.

A significant percentage of costs in pharmaceutical markets is devoted to supplements, due to the confidence of consumers on the beneficial effects of these products. Magnesium is one of the supplements with enduring and increasing popularity. According to what is reported online this metal ion can cure, or prevent, almost all kind of diseases. This review aims at illustrating a series of scientifically demonstrated cases in which magnesium was used in clinical practice. Except for its ordinary use as antacid and laxative, other ascertained uses, reported in scientific literature, consist in helping to treat several diseases such as:
nocturnal leg cramps
, pre-eclampsia,
diabetes
, depression, Parkinson's and Alzheimer's disease, hypertension, some types of arrhythmias, asthma, migraine headaches, epilepsy, cerebral haemorrhage, stroke. However, many of these promising uses of magnesium require further studies to define, the involved molecular mechanisms which should help establishing its uses in relation to the prolonged use of supplements.
