The birth and mortality rates in the Democratic Republic of Congo (DRC), a former Belgian colony, are high, i.e., 48.9/1000 and 17/1000 respectively. The DRC also has one of the highest maternal death rates in the world, i.e., 1289/100,000 live births. Health conditions have not improved since independence. Access to drinking water is limited, living conditions are poor, and food availability in households is low. The mean health services utilization rate in the DRC is estimated to be 0.15 visits/inhabitant/year. The incidence of transmissible diseases is rising. This increase is observed even for illnesses that were under control before independence such as sleeping sickness, onchocerciasis,

, and tuberculosis. One the main causes of mortality and morbidity in the population is malaria that is responsible for the deaths of 150,000 to 250,000 children under the age of 5 every year. The HIV prevalence rate is 4.5% with 1.19 million persons with AIDS and 930,000 orphans whose parents died of AIDS. Other potentially epidemic diseases including bubonic plaque and Ebola hemorrhagic fever are serious threats. Non-transmissible diseases are also on the rise including

, systemic arterial hypertension, cancer and neglected diseases such as sickle cell anemia. To meet these challenges, the country's health authorities have established a program called the Strategy for Reinforcement of the Health System (SRHS). One goal of the SRHS is to develop health zones in order to improve access to quality health care for the whole population.

Critical analysis of the Latin American bibliographical production over the last 10 years regarding the adherence/non-adherence to treatment of people with chronic health problems:
leprosy
, tuberculosis, hypertension,
diabetes
and AIDS. Thirty six articles were analyzed identifying the variables: year of publication, publication area and kind of study. Most of the articles (27) were produced by professionals of the medical area, in epidemiological studies, and of the nursing area (7) in qualitative and quanti-qualitative studies. The scientific production on the subject increased until 2002, when it began to drop. The authors repeatedly defended the idea that the role of the patient is to follow the recommendations of the health professional and that the patient is free to follow or not the treatment, the professional however being exempt from the responsibility for the consequences of this decision. The greater part of factors pointed out by the authors as contributing to non-adherence relates to the patient, showing that the major responsibility for the adherence / non-adherence to treatment is conferred upon him. The measures the authors indicate for solving the problem point to responsibility of professionals, health services, governments and teaching institutions.

Insulin-like growth factor binding protein-5 (IGFBP-5) mediates mesangial cell migration through activation of cdc42, and laminin421 binding to alpha(6)beta(1)-integrin (Berfield AK,
Hansen
KM, Abrass CK. Am J Physiol Cell Physiol 291: C589-C599, 2006). Because glomerular expression of laminin beta(2) is reduced in diabetic rats (Abrass CK, Spicer D, Berfield AK, St. John PL, Abrahamson DR. Am J Pathol 151: 1131-1140, 1997), we directly examined the effect of hyperglycemia on mesangial cell migration and laminin beta2 expression. Migration mediated by IGFBP-5 is impaired in the presence of 25 mM glucose. This reduction in migration was found to result from a loss in mesangial cell synthesis of laminin421, and IGFBP-5-induced migration could be restored by replacing laminin421. Additional studies showed that there was selective reduction in mRNA translation of laminin beta2 in the presence of high glucose. Preserved synthesis of laminin beta1 indicates that not all proteins are reduced by high glucose and confirms prior data showing that laminin411 cannot substitute for laminin421 in IGFBP-5-mediated migration. Results showed significant increase of the vascular net (p < 0.05) compared to the negative control (H2O). The histological analysis was in accordance with the results obtained. In conclusion, our data indicate that S. umbellatum latex, under the conditions of this research, presented angiogenic effect.

Kavirajes or traditional medicinal practitioners form the primary healthcare providers of the predominantly rural population of Bangladesh. Kavirajes use a variety of medicinal plants for treatment of different ailments. The formulations prepared from medicinal plants vary considerably between Kavirajes of different regions of the country. The objective of this study was to conduct an ethnomedicinal survey amongst the Kavirajes of Chalna area, Khulna district, Bangladesh. That area is known to contain a diversity of medicinal plants. Information on 50 plant species was obtained. These medicinal plants belonged to 49 genera and 33 families. Twenty five plants were used to treat skin diseases and twenty three plants for treatment of intestinal tract disorders, which included constipation, indigestion, stomachache, diarrhea, and dysentery. Fourteen plants were also used by the Kavirajes to treat cancer or tumor. Nine plants were used as insecticide, eight for rheumatoid arthritis, and seven for wounds. Five plants were used to treat jaundice. Five plants were also utilized to treat animal and snake bites, which included tiger bites. Six plants were used to treat
diabetes
, and two each for the treatment of
leprosy
, and sexually transmitted diseases like gonorrhea. Five plants were used to treat impotency, while one plant was used as an abortifacient. Three plants were used to treat helminthiasis, which we found to be quite common amongst the population, while four plants were used to treat heart disorders. Synadenium umbellatum Pax, popularly known as "cola-nota", is a medicinal plant that grows in tropical regions. The latex of this plant is used against various diseases, such as

, tripanosomiasis, leukemia, and several malignant tumors. The mutagenic, antimutagenic, and cytotoxic effects of the latex of this plant were investigated by measuring the frequency of micronuclei in mice bone marrow cells. To evaluate mutagenicity, the animals were treated with four doses of latex (10, 30, 50, and 100 mg/kg body weight). To study the antimutagenic activity, the animals were simultaneously treated with latex and mitomycin C (4 mg/kg). The cytotoxicity was evaluated by polychromatic and normochromatic erythrocytes ratio. Our results showed a significant increase of frequency of micronucleated polychromatic erythrocytes (MNPCE) compared to the negative control group (p < 0.05). Concerning antimutagenicity, the doses of 10 and 30 mg/kg co-administered with mitomycin C showed significant decrease in MNPCE frequency compared to the positive control group (p < 0.05). However, no significant reduction in MNPCE frequency (p > 0.05) was detected at the doses of 50 and 100 mg/kg. Under our experimental conditions, the results obtained indicate strong mutagenic and cytotoxic activity of S. umbellatum latex except the dose of 10 mg/kg and moderate antimutagenic effect at lower doses.
