Measurement of serum concentrations of free triiodothyronine (FT3) is considered to be an accurate index of thyroid function in the patient. In this study, we measured serum concentrations of FT3, free thyroxine (FT4) and reverse triiodothyronine (rT3) by radioimmunoassay in blood samples taken from the navel cord of 20 newborns as well as 20 nonpregnant women, 20 pregnant women, 10 patients with liver diseases, 25 patients with diabetes mellitus, 65 patients with hyperthyroidism, 30 patients with primary hypothyroidism and 29 normal subjects. In pregnant women, serum FT3 and FT4 levels gradually decreased as the pregnancy progressed. In cord blood, FT3 levels were less than a quarter of the values found during the first trimester of pregnancy or that of non-pregnant women, whereas serum rT3 levels were drastically increased. In chronic hepatitis, liver cirrhosis and diabetes mellitus, serum FT3 and FT4 levels were significantly lower than that in the controls. In thyroid diseases, serum FT3 levels varied parallel to other thyroid hormone levels. In primary hypothyroidism, however, serum FT3 levels were still lower than these in the controls after treatment with 1-thyroxine, whereas other thyroid hormone levels and TSH levels returned to control levels.(ABSTRACT TRUNCATED AT 250 WORDS)

The precision of CA 19-9 RIA kit was evaluated by recovery, reproducibility and dilution test with very satisfactory results. The CA 19-9 value in sera from 52 healthy individuals and from 224 patients with gastric intestinal cancer and other benign disease, showed an increased positive rate in several cases of gastric intestinal cancer. For example, the positive rate in pancreatic cancer, bile duct cancer, colo-rectal cancer, gastric cancer, esophagus cancer, primary biliary cirrhosis diabetes mellitus, liver cirrhosis and chronic hepatitis was 60%, 75%, 55.6%, 45.6%, 20%, 28.6%, 22.7%, 13.7% and 1.7% respectively. By contrast, values from patients with acute hepatitis, fulminant hepatitis, fatty liver, gastric duodenal ulcer, pancreatitis, and primary liver cancer were within the normal range. In this study, CA 19-9 RIA were found to be significant as an adjunct in the management of patients with gastrointestinal cancer, especially pancreatic cancer, and bile duct cancer.

Investigations on the T and B lymphocyte surface architecture were carried out by means of scanning electron microscopy (SEM) in 102 patients with various chronic internal diseases such as chronic hepatitis, diabetes mellitus, systemic lupus erythematosus, chronic lymphocytic leukemia, a.o. The clinical and biological examination performed in these cases reveled also some cytochemical, biochemical and immune disturbances. Emphasis is laid on the superiority of SEM, as compared with the E and EAC rosette tests, for the identification of T and B lymphocytes. Moreover, by detecting certain cellular anomalies, SEM can provide useful indications regarding the diagnosis, prognosis and therapy of the above mentioned diseases. Association of SEM with other clinical and laboratory investigations can either confirm, disapprove or even enhance the value of the morphological findings by electron microscopy.

Some peculiar aspects of T and B lymphocytes from patients with chronic internal diseases with immune component such as: chronic hepatitis (CH), rheumatoid arthritis (RA), diabetes mellitus (DM), systemic lupus erythematosus (SLE), chronic lymphocytic leukemia (CLL) and others, could be detected using transmission electron microscopy (TEM) or scanning electron microscopy (SEM) in association with cytomorphologic, cytoenzymochemical and/or cytoimmunologic methods. Among the more frequent aspects observed were the change of the T and B lymphocyte balance and the presence of giant nucleoli associated with RNA hyposynthesis. This latter aspect was proved to be a sign of unfavourable prognosis and of resistance to immunosuppressive treatments in diseases such as: RA, SLE and CLL.

Aspects of cytotoxicity followed by cytolysis have been described by many authors in neoplasias. Using scanning electron microscopy, the authors have investigated the cellular population from 179 patients with various chronic internal diseases such as: chronic hepatitis, diabetes mellitus, systemic lupus erythematosus, scleroderma, chronic lymphocytic leukemia. In some of the forms of disease with severe clinical and biological phenomena, they have detected the presence of "killer" type phenomena, having as target cells either the monocytomacrophage or the T or B lymphocytes. Iconographic aspects are demonstrating the presence of this phenomenon, observed particularly in the peripheral blood. Detection of such phenomena is assumed to provide information on prognosis of the chronic disease investigated and on the effects of therapy at cellular level.

We studied the relation between admission after check-up and check-up frequencies of AMHTS examinees in our hospital in order to evaluate the usefulness of serial check-ups. Follow-up studies were performed on a total of 16,630 examinees and continued until March 1982. There were 558 inpatients, 3.4% of the total. The main reason for their admission was cancer, followed by diabetes mellitus and chronic hepatitis. It was shown that the admission rates of examinees decreased as the check-up frequencies increased the examinees under 60 years old, but not for those over 60 years of age. In addition, both admission and cancer rates of AMHTS examinees decreased on the whole as their check-up frequencies increased.

In 228 patients with diabetes mellitus (130 diabetics without and 98 diabetics with hyperlipoproteinaemia) percutaneous liver punctures after Menghini as well as biopsies of the subcutaneous fatty tissue were carried out. From the biopsy specimens and from serum the fatty acid pattern of triglycerides was estimated. In 87 patients with chronic aggressive hepatitis and 37 patients with fibrosis no differences in the fatty acid composition could be found. According to the present findings there was no evidence of alterations in the supply of individual fatty acids caused by chronic hepatitis per se. However, it is to be taken into consideration that simultaneous liver steatosis can provoke marked changes in the fatty acid pattern of liver triglycerides. The best reference seems to be the size of the fat droplet in the hepatocytes. Its rise is associated with an increase of palmitic and oleic acid, whereas the percentage of arachidonic and eicosapentaenoic acid is decreased. It must be clarified by further studies, whether this reveals a general pathophysiological phenomenon or is restricted to diabetic subjects.

To determine a possible relation of hepatic oxidative activity to glucose metabolism, the rates of oxygen consumption of liver slices from patients with chronic liver diseases were polarographically measured. The livers from patients with chronic (persistent and aggressive) hepatitis and with normal glucose tolerance showed almost the same respiratory activity as those from patients with normal livers and normal glucose tolerance, whereas the livers from patients with chronic hepatitis and with diabetic glucose tolerance (ie, diabetes mellitus secondary to chronic hepatitis) showed only a half the normal level. The decreased rate of respiration was also observed in liver slices from cirrhotics with glucose intolerance. The decrease in respiration was found in patients with normal or hyperinsulinemia as well as hypoinsulinemia responding to oral glucose load. No liver tests so far examined, except the oral glucose tolerance test, correlated with hepatic respiratory activity. It is concluded that in patients with chronic liver diseases the defect of liver respiration has a close relation to the glucose metabolism and is not necessarily associated with histological change of the liver.

The levels of serum secretory component (SC) were measured in 147 patients with digestive disease. Decreased levels were found patients with acute hepatitis, HBs-antigen associated chronic hepatitis, HBs-antigen associated liver cirrhosis and hepatoma. Normal levels were observed in patients with diabetes mellitus, gastric cancer and colonic carcinoma. Elevated levels were found in patients with cholecystitis, obstructive jaundice and acute pancreatitis. The serum SC level in almost all disease groups showed no correlation with immunoglobulin levels.

We investigated the side effects of interferon (IFN) on the endocrine and respiratory system in 545 cases of chronic hepatitis C. Eleven of 494 (2.2%) patients with chronic hepatitis C who were treated with natural or recombinant interferon (IFN) developed thyroid disease while on treatment. Eight patients developed hyperthyroidism and 3 patients developed hypothyroidism. All 11 patients required definitive therapy, who became euthyroid after the therapy. Two patients received nIFN alpha and one patient received rIFN alpha 2b developed diabetes mellitus. Two patients received rIFN alpha 2a and rIFN alpha 2b, respectively, developed interstitial pneumonia 12 weeks and 24 weeks later, respectively. One patient showed positive reaction for RA test and LE factor and positive LE cell, and complained of fever, arthralgia and dry cough. These phenomenon disappeared after the cessation of IFN therapy.
