One hundred and eighty-three conservative amputations of some part of the foot in 161 patients with gangrene from
diabetes
or arteriosclerosis have been studied retrospectively. They constituted 48 per cent of all amputations in one orthopaedic service over a period of twelve years, during which the minimal feasible procedure was always chosen. Sixty per cent healed soundly, but in over a third of these cases at least one revision to a higher level on the foot had been required. Factors that significantly influenced the outcome of the initial operation were the level of amputation, the age of the patient, the interval between the onset of gangrene and operation,

and pyrexia.

Fourteen adults in whom
diabetes mellitus
and coeliac disease coexist, are described. In no patient was coeliac disease diagnosed (biopsy proven) before the age of 28 years.

was recognized before coeliac disease in all except one. Diabetic control was very unstable and hypoglycaemia particularly troublesome before treatment with a gluten free diet. The opportunity to diagnose coeliac disease in adult diabetics will usually fall to the diabetologist and wider use of jejunal biopsy in diabetics with chronic or recurrent diarrhoea is suggested.

The results of the study of 37 patients with gastric polyps are analysed. The different methods show as most frequent findings; solitary polyps, 73%; sessile, 60%; of antral localization, 70%; smaller than 2 cm., 78%; with chronic gastritis histology, 50%; and with hypochlorhydria, 77% (achlorhydria, 62%). The incidence of malignancy was of 10%. The fost frequent concomitant pathologies were cholecystopathy (33%) and

Gastric Vagotomies were performed on fifty four patients suffering from bleeding Duodenal Ulcers. Forty-three cases were operated on following a spontaneous cease in hemorrhaging, and necessary examinations. The remaining eleven patients were opereted on immediately as conservative measures would not be able to stop the bleeding. The procedure used following the admittance of a patient with a bleeding ulcer to the hospital was discussed. A detailed explanation of the technical procedure used in handing the bleeding duodenal ulcer was also presented. The authors believe that a Duodenotomy performed without cutting the pylorus has its advantages inasmuch as hemostasis has been successfully accomplished. All the cases had good clinical results, however, bleeding did recur in a few instances. There were no recidive ocurrences. Ever though many patients were chronically ill ie;

Ten episodes of Torulopsis glabrata fungemia occurring in nine patients with terminal illnesses are described. Eight patients had underlying malignancies and one patient had a plastic
anemia
. Two episodes of fungemia were considered transient since they were clearly related to the administration of intravenous hyperalimentation (IVH). Most patients were adult women and had solid tumors of the genitourinary tract. Contributory factors were: antibiotic therapy (100%), immunosuppressive drugs (75%), abdominal surgery (63%), IVH (50%), neutropenia (38%), and
diabetes mellitus
(13%). The clinical course was indistinguishable from a severe bacterial infection. However, endotoxic shock was not observed. The infection was rapidly fatal in four patients. In the remaining five patients, the infection was altered favorably by the discontinuation of infected intravenous hyperalimentation catheters. However, tissue invasion by T. glabrata was found in two of these patients who died shortly thereafter from tumor progression. At autopsy, T. glabrata was identified in tissue sections of the lungs, kidneys, and mucosas of the gastrointestinal and genitourinary tracts. In all cases there was tissue necrosis with a minor inflammatory response consisting of mononuclear cells. To our knowledge, this is the single largest series of T. glabrata fungemia ever reported.

A single tonometry observation is not very relevant due to considerable interobserver variations which are also found in applanation tonometry. Furthermore, the intraocular decreases by repeated applanation tonometry and intra-ocular pressure that change for as much as 8 mm Hg in 1 minute has been found in continous pressure recordings. There are in addition diurnal pressure variations and pressure alterations with the position of the body. Therefore, the diurnal pressure curve is much more important than the single tonometric value. Erythrocyte transketolase activity (ETKA) and the effect of adding thiamine pyrophosphate have been measured in a group of 27 healthy individuals and in 37 patients diagnosed as having
diabetes mellitus

, polyneuritis, or malnourishment secondary to vascular disease of the brain. The observed values for the malnourished group did not differ significantly from those for the control group. The low ETKA values in
diabetes mellitus
seem to be due to a reduced apoenzyme level resulting from the disease itself rather than thiamine deficiency. Polyneuritis patients had low values of ETKA. In the anemic group as a whole the values showed a difference of only marginal significance from those found in the control group, but the patients with pernicious anemia all had a highly significant elevation of the ETKA values. Although the absolute thiamine pyrophosphate effect differ, there are no significant differences in percentage of thiamine pyrophosphate effect between the groups. It appears that differences in the patient groups studied here reflect variations in apoenzyme levels rather than thiamine status.

Fifty-eight patients with chronic laryngitis, defined as diffuse laryngeal inflammation, of an average duration of approximately 1 year, were investigated. The sex incidence was equal and the average age 45 years. Occupational factors were of minor importance, and the smoking habits of the patients were the same as those of a similar matched population. Associations between previous induced abortion and demographic and health factors in pregnancy were measured in 9 874 women who gave birth and who had been interviewed during pregnancy. Previous abortion was most rare among women having their first baby and increased with increasing birth order up to the fourth, thereafter decreasing. It was positively correlated with maternal age and negatively with age at marriage. There was no effect of years of schooling, when other variables were taken into account, but there were significant differences between ethnic groups, abortion being commonest among Jewish women from North African countries and more prevalent in those from western and Asian countries than in the second-generation Israel-born or in Arab women.Women who reported abortions were less likely to be strict as regards religious observance and less likely to have had a previous stillbirth or child death, other variables being equal. They were more likely to be smokers or former smokers and to be delivered of their babies in certain obstetric units. They more often reported vomiting, bleeding, and medication in early pregnancy. On the other hand, there was no significant association with

, blood groups, or season of birth.The findings show that women reporting previous induced abortions differ significantly from other pregnant women in a wide range of demographic and health characteristics. Such women may also be biased for complications of pregnancy and outcome, particularly if selected from a clinic population. Observations that indicate a deleterious effect of induced abortions on subsequent pregnancy outcomes must therefore be interpreted with considerable caution.
