A partial review of selected published case reports of AD-P associated with
malignancy
has been enhanced by the presentation of pertinent data on 15 unreported examples of the association. It is noteworthy that the first case in current literature of AD-P associated with a
malignancy
was described in 1916. The brief clinical report of a patient with proximal muscle weakness and skin lesions, with the obvious association with a
malignancy
(adenocarcinoma of the stomach), describes an example that has been repeated many times with different types of tumors but with essentially no variations in the clinical findings. In 1959 Williams identified 590 cases of AD with an overall tumor rate of 15%, and recently Barnes identified 258 cases of AD associated with a
malignancy
. The original designation,
dermatomyositis
or AD, has now been expanded to include proximal muscle polymyositis with systemic involvement, which syndrome at the current state of the art is indistinguishable clinically and pathologically from AD except for the lack of skin lesions. It may be that at some future time one or more immunologic features may differnetiate the clinical entity polymyositis from AD and further subdivide each of these entitites from similar clinical syndromes associated with a
malignancy
. However, the problem in management in either AD or polymyositis is similar. A number of patients with a
malignancy
and muscle weakness or neuropathy have been reported. These associations have been mentioned briefly, but insufficient data are available to determine whether these should be considered as a variant of AD-P or only casually related conditions with certain clinical features in common. Most of the patients described in the literature of AD-P with an associated
malignancy
have had skin lesions; a minority only have lacked this feature. However, unless a patient is followed carefully, it is possible for a transient or evanescent erythema or insignificant skin lesions to be present and not recorded in the case record.

The aim of this work was to develop a computer program (CANEST) to estimate the risk of
cancer
in patient populations and to use this program to investigate
cancer
risk associated with several dermatological disorders. Patients seen at the dermatology departments at the Karolinska Hospital and South Hospital were used for the study of chronic urticaria, condylomata acuminata, basal cell carcinoma, lichen planus and positive patch tests. The national Swedish In-Patient Register was used to find all patients hospitalized for
dermatomyositis
or polymyositis since 1964. From eleven large dermatological centers in Sweden, details of close to 5,000 PUVA-treated patients were obtained for study. The computer program CANEST was developed and used to calculate the expected number of malignant tumors in these patient populations, based on incidence data from the Swedish
Cancer
Register for the years 1958-1987. By matching the patients' records with the
Cancer
Register the actual number of cancers was obtained. Of 1,155 patients with chronic urticaria, a
malignancy
was diagnosed in 36, while the expected number was 41: clearly there is therefore no association between chronic urticaria and
malignancy
. In 3,260 patients with condylomata acuminata there was no increased risk of
cancer
in situ of the cervix (relative risk = 1.5; 95% confidence interval 0.9 to 2.5) and the number of genitourinary cancers in males was almost three times higher than expected (2.6; 1.2 to 5.0). These results indicate that the risk of developing cervical carcinoma in situ is less than previously thought, but the implications of the increase in genitourinary tumors in males are uncertain. Patients with basal cell carcinoma had an increased risk of
malignancy
in general. Melanoma risk was seven times greater in males (6.6; 3.0 to 12.5) after the basal cell carcinoma diagnosis. Risks of squamous cell carcinoma of the skin, lung cancer, thyroid cancer and
cancer
of the uterine cervix were also increased. No increased risk of cutaneous
malignancy
was found in 2,071 patients with lichen planus, but for oral cancer it was six times greater in males (5.9; 2.5 to 11.4). A slight general increase in
malignancy
risk was found in 2,183 males (1.3; 1.1 to 1.5) with positive patch tests, but not in 3,675 females. When individual sites were analyzed, cancers of the lung, larynx, uterine cervix and prostate were significantly increased. Temporary remission of scleroderma during the successful treatment of an associated malignant lymphoma by chemotherapy is reported in a patient with systemic sclerosis and mixed connective tissue disease. There is a well established relation between malignant disease and polymyositis/
dermatomyositis
but no overall association with systemic sclerosis or mixed connective tissue disease. Reports of the coexistence of
malignancy
and systemic sclerosis, however, emphasise a close temporal relation in their occurrence. A review of published work has identified several postulated mechanisms for this relation which may explain the response to chemotherapy.

A case of
dermatomyositis
(DM) associated with thymic cyst with lymphoid follicle formations has been reported. A 57-year-old man developed polyarthralgia, muscle weakness, heliotrope rash and Gottron's sign. Laboratory findings showed elevated values of creatine-phosphokinase, aldolase and transaminase. He was diagnosed as DM and was treated with prednisolone 50 mg/day. Muscle weakness was improved immediately, although rash persisted unchangeably. He was examined for coexistence of
malignant tumor
which had been reported frequently as associated with DM. Enlargement of thymus was found by computer tomography and then thymectomy was performed. In his thymus, lymphoid follicle formations, which are as often encountered in other autoimmune diseases, were found. So far, the beneficial effect of thymectomy on the improvement of immunological abnormalities not only in patients with myasthenia gravis but also in patients with other autoimmune diseases has been recognized. In this case, intractable rash in DM improved after thymectomy. Further study needs to clarify the relationships between DM and thymic abnormalities.
