The authors compare the results of the analogous self-rating scale of affect with self report of affect (Hamilton Scale) in 57 patients with
depression
during the course of affective disorder. The ASRA seems to be particularly useful in the assessment of improvement in patients with slight or moderate
depression
.

On the basis of medical records of 353 patients with affective disorder from the years 1919-1938 and 1947-1990 it was found that some demographic and clinical changes occurred. Amongst others there was observed a rise in the percentage of patients with
depression
from 1/3 to 2/3 and a fall in the number of patients suffering from mania.

Carbamazepine treatment was used with 54 patients suffering from endogenous depression and organic
depression
. The anti-depressive effect was found to be the best in those patients with organic
depression
, while in endogenous depression it was better with bipolar affective disorder and with irregular EEG recordings.

Dexamethasone Suppression Test was performed during an acute phase in 81 patients with endogenous depression and 105 schizophrenic patients. The lack of suppression of cortisol was found in 1/3 if those ill with
depression
and 1/3 of those ill with schizophrenia. A relationship between those results and age was shown in female schizophrenic patients. In both groups a yearly rhythm was observed, however female patients differed from male patients.

The effect of intra-accumbens injections of drugs changing the function of GABA-A and GABA-B receptor systems on stressor-induced motor
depression
, was studied in rats. Local injections of picrotoxin and baclofen, but not of midazolam and muscimol, attenuated the inhibitory effect of inescapable footshock on locomotor activity in the open field test, examined 24 h after a single exposure of rats to the stressful event. The results obtained with picrotoxin may be related to the general disinhibitory properties of the convulsant on brain neuronal activity, in a period of time important for consolidation of central processes evoked by inescapable shock. The lack of effects of muscimol and midazolam, further underlines the minor and/or indirect role of accumbens GABA-A receptor-related innervation in the neural processes generated by stressful event. On the other hand, the results obtained with baclofen confirm the reports indicating an inverse relationship between the number of GABA-B receptors in the frontal cortex and the development of helpless behavior in rats. It is also noteworthy that most antidepressant drugs which have been shown to prevent or reverse behavioral deficits after inescapable shock, upregulate GABA-B receptors in the frontal cortex. Hence, it appears that GABA-B receptor-related systems within the nucleus accumbens, may contribute to the footshock-induced behavioral
depression
, including locomotor inhibition. The reduction of stress effect by baclofen does not seem to reflect changes in fear and anxiety, since the drug was given after the stress session, and the anxiolytic midazolam appeared to be ineffective in this test.

Exercise testing has been shown to be predictive for future cardiac events in patients with established diagnosis of coronary heart disease. Exercise test parameters associated with poor prognosis may be unreliable if patient is receiving beta adrenergic agents. The purpose of this study was: 1) to compare the results of exercise testing performed before and during beta blocking therapy, and 2) to determine the role of beta blockers in the prognostic significance of the ST-segment response recorded during exercise testing. The study population consisted of 518 patients (mean age 52 +/- 7 years) with coronary heart disease. The diagnosis was based on the presence of one of the following three criteria: 1) typical history and significant ST-segment
depression
on resting or exercise electrocardiogram, 2) history of myocardial infarction, 3) significant coronary angiographic abnormalities. In all patients symptom-limited exercise test was performed before and two weeks after the onset of beta blocker therapy. The data from the first and second tests were estimated for significance of differences between the mean values with following results: maximal heart rate--135 +/- 21 and 123 +/- 19 bpm (p less than 0.001), maximal work load achieved--98 +/- 43 and 109 +/- 44 W (p less than 0.001), maximal systolic blood pressure--171 +/- 28 and 163 +/- 26 mmHg (p less than 0.001). Occurrence of characteristic ST-segment
depression
was more frequent during the first than during the second test (p less than 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

In severe bronchial asthma reversible electrocardiographic abnormalities are not rare. It is usually sinus tachycardia, right axis deviation, atrial enlargement and right bundle branch block. Transient ST-segment
depression
or elevation in inferior leads in severe acute asthma has been observed since long. Adrenergic stimulation, hyperventilation, hyperinflation and primary or secondary coronary insufficiency were as a causes. Severity of ECG signs correlated with the degree of airway obstruction. Our study was aimed at investigation of electrocardiographic abnormalities in chronic pulmonary obstructive disease and asthma and to assess the relationship of the extent of airway obstruction to the frequency of ECG changes. Correlation was found of ECG manifestation of sinus tachycardia, right ventricle hypertrophy. ventricular premature complex, right bundle branch block with the degree of airway obstruction.

Experimental thermal brain injury leads to significant reduction of glucose utilization in the damaged hemisphere particularly evident in the cortex 3 days after the injury. The rate of development of these changes is not parallel with the observed damage to the blood-brain barrier, coexistent brain oedema and slight disturbances of cerebral blood flow. In a series of experiments it was possible to demonstrate significant accumulation of glucose, high-energy phosphate compounds and their metabolites in the areas of the brain near the damaged part. The authors think that this is an evidence of reduced glucose uptake by the brain resulting from reduced energy needs of the damaged brain tissue despite sufficient supply of energy-yielding substances. Since cerebral metabolism and functions are in close interrelationship reduced glucose metabolism in the damaged tissue leads to reduced activity of the cortex, which contributes to transient (or permanent) functional neurological deficits observed after cranio-cerebral trauma in humans. The knowledge and understanding of these processes regulating the development of local
depression
of cerebral metabolic processes may help in better results of treatment in such cases.

Early and late results of surgical treatment of cranial vault defects with Codubix prosthesis in 46 cases are presented. Early after the operation purulent complications were not observed. The main problem of normal healing-in of the prostheses was at that time accumulation of subgaleal haematoma which was observed in five patients (10.8%). Late complications necessitating removal of Codubix prosthesis occurred in four cases (8.7%). In one patient Codubix treatment led to meningitis due to late cerebrospinal fluid rhinorrhoea and in the second case staphylococcal infection was diagnosed 4 month after the operation. In 2 remaining cases prosthesis became movable 5 and 21 months after operation and became depressed by several millimeters. The inquiry sent to the patients for obtaining information on their present health was answered by 70% of them, and four reported slight, stable
depression
of the plate. The remaining ones reported no complications. In 91% of cases the patients regarded the cosmetic effect of the operation as good and 9% as unsatisfactory. Codubix is good material for filling of bone defects. It heals in quickly, produces rarely purulent complications, but requires very careful fixation.

Diagnostic and therapeutic problems in 14 year old patient with concealed WPW syndrome were presented. Paroxysms of atrio-ventricular reentrant tachycardia 180-220/min were frequently recurring, usually with normal QRS pattern. Tachycardias often had to be terminated by intravenous administration of antiarrhythmic drugs. Long term treatment with various antiarrhythmic agents did not prevent recurrence of tachycardias but they became sustained and were recurring more often. Their other side effects manifested with sinus node disfunction and
depression
of the heart muscle. The electrophysiologic study revealed right anterior septal accessory pathway. Epicardial dissection of the accessory pathway was urgently performed. The control electrophysiologic study revealed no evidence of conduction through the accessory pathway. The patient did not require antiarrhythmic treatment. During the 12 months follow up no tachycardia occurred.
