In order to determine the psychopharmacological profile of tiapride, the authors carried out 3 studies. The first study of ten resistant psychiatric cases permitted us to detect the direction of later studies. The object of the second was to analyse the neurovisceral properties and the action of tiapride on somatic symptoms due to anxiety in 25 ambulatory patients. The last study was a therapeutic approach. Tiapride appears to be one of the best drugs for anxiety in one of the 3 following circumstances: a paroxysmal or chronic somatic expression, depression, or behaviour suggesting latent and masked psychosis or early psychosis. Tiapride is tolerated in the same way as a neuroleptic for even in low dosage (150-300 mg/day) may appear undesirable side effects of extrapyramidal type. On the other hand, and contrary to sulpiride, neuro-endocrine effects are exceptional.

If the different forms of depression of humour are nowdays well known and easily surrounded and treated, it's for from being the case for the forms of excitation of humour, though they are as numerous. In these forms, indeed, whether they are atypical maniac attacks of hypomania, it may happen that the pathological nature of psychic excitation posses unnoticed, as well for the patient as for his familiars, or it may also happen that the excitation of humour desguises with "masks" suggesting other troubles, mental or not, which bad to delays in the setting of adapted treatments. These "masks" are essentially: -- hysteria and perturbations of character, in the neurosis register; -- delirious aspects, schizophrenical or confusional, in the psychosis register. In these states of hidden excitation, the most difficult thing, nevertheless, is to obtain from the patient a sincere claim for cares, contrarily to what can be noticed in the states of hidden deppression in which the somatical or psychological "complaints" of the patients are very easily exposed to the physician, a generalist as well as a specialist, and the treatments can be searched for.

Vectarion has a powerful respiratory stimulant effect in animals and in man which is manifested by a significant and lasting increase in ventilation, as shown by a marked lowering of arterial P. CO2 and an increase in blood pH. This activity is maintained in dogs with experimental alkalosis or acidosis as well as during morphine or oxygen-induced respiratory depression. This respiratory stimulating action originates at the level of the aortic and carotid chemo-receptors: this explains why Vectarion is devoid of any epileptogenic risk. The aim of the present study was to demonstrate the peak of activity and duration of action of an intravenous perfusion of Vectarion, in such a way as to be able to plan administration all through the day and night.

Passive hemagglutination tests (H.T.), involving the coating of a soluble B. abortus antigen onto sheep red blood cells through chromium chloride, were always negative in non-brucellic subjects. Positive H.T., even at low titers (1:50), were associated with positive specific lymphoblastic (T.T.L.) and inhibition of leucocyte migration (I.M.L.) tests in 13 patients. These 3 tests were negative in 17 control brucellosis-free individuals. Positive H.T. confirmed a clinical diagnosis of brucellosis in 27 patients with dubious or negative responses to classical tests. Low positive (1:50, 1:100) H.T. may correspond to Brucella primary-infection or to a quiescent chronic infection, and higher titers to clinically active brucellosis. There is a correlation between cellular immunity tests (T.T.L., I.M.L.) and passive hemagglutination test. Furthermore, 39/88 women hospitalized in psychiatric wards were positive to H.T., and 6 of them were also highly positive to complement fixation and tube agglutination tests. Three T.T.L. and I.M.L., performed on samples from 3 H.T. (1:50) positive patients, were positive. These 30 patients were classified as depression or severe anorexia.

Infective complications favoured by non-specific immuno-depression are common after renal transplantation. Often severe, they represent the main cause of mortality. Their prophylaxis is necessary at all stages of treatment. Before transplantation, the eradication of curable infective foci is essential, and certain patients must be excluded from the programme. The operation is simplified to a maximum. The conditioning is such that one does no longer expose to the risk of aplasia. These facts render useless the isolation of the patient on a sterile unit. In cases of poor tolerance of the transplant, with repeated rejection, the reduction and eventually the stopping of immunosuppression is necessary. This attitude implies often early return to treatment by repeated hemodialysis, but it preserves to a maximum the chances of a new transplantation.

Evidence has been found for the view that piribedil stimulates peripheral and central dopamine receptors. Piribedil stimulates the nigro-striatal pathway and has been shown to be an effective antiparkinsonian agent. The stimulation of dopamine receptors in the meso-limbic and/or meso-cortical pathways lead to propose its use in the treatment of affective disorders (maia and depression). Piribedil stimulates dopamine receptors located in the tubero-infundibular pathway and reduces the secretion of prolactine, producing ablactation ; it increases the secretion of STH. Recent studies suggest that the peripheral effects of piribedil, mainly the increases in femoral and cerebral blood flows are also due to stimulation of peripheral dopamine receptors.

Thirthy-three alcoholics, aged between 31 and 82 years, were treated for 7 to 30 days with tiapride. The dosage was 600 mg/day (200 mg 3 times daily) by mouth or 100 to 800 mg/day I.M. Out of 27 cases of tremor treated, there were 25 favourable results, one average result and one nil result. Insomnia and character disorders, e.g. anguish, depression, nightmares, hallucinations, were improved during the first few days of treatment in 27 cases out of 30. Out of 12 cases of algo-paresthesia of the lower limb treated, the were 9 good or excellent results, 2 average results and 1 nil result. A favourable result was observed in 7 cases out of nine in vomiting, water brash (3 cases out of 4), and in 16 cases out of 20 in anorexia. No clinical or laboratory disturbance attributable to tiapride was noted in our patients whose general health was often very poor.

A total of 24 patients (20 men and 4 women) were treated for varying degrees of alcoholism. Tiapride was administered orally as 300 to 600 mg/day to 22 patients, the other two cases receiving i.v. injections followed by oral doses. Tiapride was extremely well-tolerated and no side-effects were noted. Withdrawal symptoms were effectively reduced, and a significant action was noted against tremor. Tiapride was also effective against depression and anxiety, and hallucinations disappeared in two cases.

This clinical test, concerning the use of tiapride, was carried out for one year on 60 patients, with weekly or bi-mensual check-ups in a neuropsychiatric office, with no other associated medication capable of modifying either vigilance or humor. Tiapride, on top of its excellent tolerance (except for patients presenting a hysterical type structure), and apart from being a sedative for anxiety, turned out to be, on one hand, a disinhibitor for states of depression (even more so when accompanying psychosomatic symptomatology was apparent), and on the other hand a semeiological analyser inasmuch as it can help discern the underlying psychopathological structure.

Four types of combination therapy may be used in the treatment of depression: anxiolytics and antidepressants, neuroleptics and antidepressants, co-administration of two or more antidepressants, association of antidepressants with drugs correcting antidepressant-induced side-effects. First, the pros and cons of these combinations are described as they appear in daily practice. In the second part, the theoretical reasons for supporting or contraindicating such combinations are discussed in the light of pharmacological data. The value of recent molecules such as nomifensin, viloxazin, amineptin and mianserin which interact with a minimum number of neurotransmitters is stressed.
