The relationships between coping strategies, goal adjustment, and symptoms of
depression
and anxiety were studied in 104 HIV-positive men who have sex with men, in December 2006. The mean age of the respondents was 50 years, and almost were of Dutch nationality. On average people had known about their HIV-positive status for 10 years and the majority was on HIV-medication. The Cognitive Emotion Regulation Questionnaire,
COPE
, the Goal Obstruction Questionnaire, and the Hospital Anxiety and
Depression
Scale were filled out at home. Pearson correlations and Hierarchical Regression Analyses were performed. The findings suggested that cognitive coping strategies had a stronger influence on well-being than the behavioral coping strategies: positive refocusing, positive reappraisal, putting into perspective, catastrophizing, and other-blame were all significantly related to symptoms of
depression
and anxiety. In addition, withdrawing effort and commitment from unattainable goals, and reengaging in alternative meaningful goals, in case that preexisting goals can no longer be reached, seemed to be a fruitful way to cope with being HIV positive. These findings suggest that intervention programs for people with HIV should pay attention to both cognitive coping strategies and goal adjustment.

The objective of this analysis was to describe psychological and cognitive variables in subjects with migraine and to identify those associated with chronicity. Data were collected from 10 000 subjects during face-to-face interview. Lower gastro-intestinal endoscopy is a common investigative procedure which can be distressing. The extent of this distress is not known, nor have the predictive factors been identified. The aim of the present study was to identify to what extent it is distressing, what factors might predict distress in this population and to identify coping strategies that may mediate such distress. Coping strategies of self-blame and self-distraction, threat appraisals of endoscopy procedure and control appraisals of results, low optimism, presence of symptoms, low social support, and gender were found to account for 56.3% variance in anxiety. Longer times on the waiting list increased the degree to which the procedure was appraised as a threat. High levels of distress are evident in this population. There is a clear need for better identification of distressed patients and the appropriate provision of psychological support.

Little is known about effects of community-based physiotherapeutic exercise programmes incorporated in COPD self-management programmes. In a randomised trial, the effect of such a programme (
COPE
-active) on exercise capacity and various secondary outcomes including daily activity as a marker of behaviour change was evaluated. All patients attended four 2-h self-management sessions. In addition the intervention group participated in the
COPE
-active programme offered by physiotherapists of private practices, consisting of a 6-month "compulsory" period (3 sessions/week) and subsequently a 5-month "optional" period (2 sessions/week). Because
COPE
-active was intended to change behaviour with regard to exercise, one session/week in both periods consisted of unsupervised home-based exercise training. Of 153 patients, 74 intervention and 68 control patients completed the one-year follow-up. Statistically significant between-group differences in incremental shuttle walk test-distance (35.1 m; 95% CI (8.4; 61.8)) and daily activity (1190 steps/day; 95% CI (256; 2125)) were found in favour of the intervention group. Over the 12-month period a significant difference of the chronic respiratory questionnaire (CRQ) dyspnoea-score (0.33 points; 95% CI (0.01; 0.64)) and a non-significant difference of the endurance shuttle walk test (135 m (95% CI (-29; 298)) was found. No differences were found in the other CRQ-components, anxiety and

scores and percentage of fat free mass. This study demonstrates that a community-based reactivation programme improves exercise capacity in patients with moderately to severe COPD. To be treated for cancer must be a frightening experience. Yet quality of life (QoL) of successfully treated cancer patients seems to be relatively similar in comparison with QoL of a general population, with psychological coping partly responsible for this finding. When measuring choice of coping, the nature of coping score levels constituting appropriate scores, and whether score levels rely on the context of the disease has not been settled. We have studied the
COPE
coping responses as related to disease in successfully treated head and neck squamous cell carcinoma (HNSCC) patient groups (general and laryngectomized), as well as compared to multiple sclerosis (MS) patients. The
COPE
response patterns have also been compared to the Beck
depression
inventory (BDI) scores. Age and gender of patients were not directly associated with choice of coping. Within the problem-focused coping indexes, the coping index "active coping" was reported to be most utilized among HNSCC patients, whereas "coping by suppression" and "coping by social support" were most utilized among MS patients. Emotional-focused coping was most prevalent among HNSCC patients and lowest among the MS patients. Level of avoidance coping was similar between the groups. The coping of the general HNSCC patients differed most from the MS patients. An association was shown between increased coping efforts and lowered mood. In particular, avoidance coping was associated with lowered mood. These associations were stronger among the MS patients than HNSCC patients. Drinking to cope was most prevalent among the laryngectomized group, and was correlated with BDI scores in all groups. Furthermore, adequate coping seems to be to limit avoidance coping and promote coping by acceptance. The response pattern of the
COPE
inventory seems to be valid among HNSCC and MS patients.
