Patients sustaining acute myocardial infarction (AMI) often require urgent percutaneous revascularization within the first 24 h from onset of the infarction due to continuous ischemia and hemodynamic instability. Upon arrival to the cardiac catheterization, the electrocardiogram of AMI patients may exhibit acute ST-elevation (STEMI) with or without accompanying Q-wave or depression of the ST segment (non-STEMI or non-Q-wave infarction). Data comparing acute outcome of device application in patients presenting for urgent revascularization with established Q-wave myocardial infarction (QWMI) versus those with non-STEMI (NQMI) are sparse. Excimer laser is a revascularization modality applied for debulking of atherosclerotic plaque and vaporization of associated thrombus in the setting of AMI. One hundred fifty-one AMI patients with continuous chest pain and ischemia who enrolled into a multicenter study and underwent urgent revascularization were divided for the purpose of a retrospective analysis into two groups. One group presented with established electrocardiographic Q-wave, whereas the other had ST-depression (NQMI). In comparison with the NQMI group, the QWMI patients had a higher incidence of failed thrombolytic therapy (17% vs 3, p = 0.006), cardiogenic shock (20 vs 6%, p = 0.01), left anterior descending as a culprit infarct-related vessel (46 vs 14%, p < 0.0001), a higher incidence of TIMI 0 flow (48 vs 24%, p = 0.04), a heavier thrombus burden (grade 4 TIMI thrombus, 58 vs 23%; p = 0.0001), and higher CPK (1272 +/- 2180 vs 404 +/- 577, p = 0.001) and troponin levels (62 +/- 95 vs 14 +/- 48, p = 0.0003). Both groups underwent laser angioplasty and stenting for relief of continuous chest pain and ischemia within 24 h of infarction onset. Quantitative coronary arteriography in an independent core laboratory measured similar improvement in baseline minimal luminal diameter and percent diameter stenosis by application of laser energy in both groups. Among the QWMI patients, a significantly higher acute gain was recorded with the laser treatment in lesions containing a large/extensive thrombus burden as compared with lesions containing only a small clot burden (1.2 +/- 0.7 vs 0.8 +/- 0.5, p = 0.01). Such a phenomenon was not detected among the NQMI patients (1.0 +/- 0.5 vs 0.8 +/- 0.6, p=ns). Baseline TIMI flow grade (0.9 +/- 1.0 for QWMI vs 1.5 +/- 1.2 for NQMI, p = 0.0001) increased with laser emission to 2.8 +/- 0.5 and subsequently reached a final level of TIMI 3 in both groups. In comparison with the QWMI patients, there was a trend toward a reduced rate of major adverse coronary events among the NQMI patients (12% QWMI vs 4% NQMI, p = 0.09). Significant differences in baseline clinical characteristics, extent of myocardial damage, location of infarct related vessel, thrombus burden, and TIMI flow exist between QWMI and NQMI patients who require urgent intervention. However, application of excimer laser results in similar high procedural success and low complication rates in both groups. Maximal acute laser gain is achieved among QWMI patients whose lesions are laden with a heavy thrombus burden.

This retrospective study was carried out from January 2005 to December 2006 to ascertain the profile of patients admitted for treatment of myocardial infarction (MI) in a modern emergency reception facility (ERF) in Sub-Saharan Africa. Diagnosis of MI was based on clinical presentation, electrocardiography (persistent ST-segment depression), and laboratory findings (measurement of troponine T and CPK MB). Study data included epidemiological parameters (including risk factors and interval between onset of symptoms and admission), clinical and paraclinical findings, therapeutic modalities (including any prehospital management), complications at the time of admission, and mortality within the first five days. Men accounted for 77% of the 52 consecutive patients hospitalized for MI during the study period. Mean age was 59 years. Risk factors included hypertension in 46% of cases, tobacco use in 40%, and diabetes in 21%. The mean interval for management was approximately 29 hours with only 5 patients receiving care within the first 6 hours. Chest pain was the main reason for coming to the ERF (86%). The location of pain was anterior in 58% of cases and inferior in 37%. Diagnosis was confirmed by laboratory findings in 96% of patients. Ten patients benefited from transportation by ambulance and four patients underwent thrombolysis before hospitalization. Complications at the time of admissions included cardiovascular collapse (n=3), acute pulmonary edema (n=13), and arrhythmia (n=6). Eleven patients (21%) died within the first five days. These findings confirm the need to educate the population in an effort to reduce the interval for management, to develop prehospital medical care, and to increase the availability of coronary artery revascularization modalities to improve the prognosis of MI in the acute phase.

Direct, dose dependent effects of the nose-horned vipers (Vipera ammodytes ammodytes) venom on various parameters of cardiac action in isolated rat hearts were examined. Biochemical (protein content, SDS polyacrylamide gel electrophoresis) and biological (minimum haemorrhagic and necrotizing dose and lethal dose (LD(50))) characterization of the venom was performed before testing. The hearts were infused with venom doses of 30, 90 and 150 microg/mL for 10 min followed by 30 min of wash out period. Left ventricular pressure, coronary flow, heart rate, atrioventricular conduction, myocardial oxygen consumption, incidence and duration of arrhythmias were measured and relative cardiac efficiency was calculated. Cardiac CPK, LDH, AST and troponin I were measured as biochemical markers of myocardial damage. The venom caused dose dependent electrophysiological instability and depression of contractility and coronary flow. Effects on the heart rate were biphasic; transient increase followed by significant slowing of the frequency. Relative cardiac efficiency decreased as oxygen consumption remained high relative to the heart rate-contractility product, indicating purposeless expenditure of oxygen and energy. Effects by the dose of 30 microg/mL were highly reversible while the dose of 90 mug/mL caused damages that were mostly irreversible. The dose of 150 mug/mL induced irreversible asystolic cardiac arrest.

Tramadol, an analgesic with a low affinity to opioid receptors, inhibits the reuptake of norepinephrin and serotonin. It is also abused by opioid addicts. Tramadol overdose can induce CNS and respiratory depression, tachycardia, and seizures. In this report, a 19 years male was admitted due to suicidal attempt of ingestion of 4000 mg of Tramadol. He experienced frequent seizures, confusion, myosis, and dramaticrise of CPK, LDH and Creatinine. Improvement was had in the following days by administering fluids, NaHCO3 and chlordiasopoxide and routine management. He was discharged with no further sequelae.

Pyrethroids are the most frequently used pesticides in agriculture, forestry, horticulture, hospitals public health, homes and textile industry. Cypermethrin, a composite pyrethroid is moderately toxic to mammals. Exposure to the pyrethroids occurs by inhalation, dermal and oral routes both accidentally as well as from the environment. Cypermethrin and DDT have been detected in human breast milk from malaria endemic area in South Africa. The WHO has recommended that the level of permethrin in drinking water not exceed 20 micrograms per liter (microg/L). The effects of exposure to any hazardous substance depend on the dose, the duration, how you are exposed, personal traits, habits and whether other chemicals are present. Pyrethroids are often combined commercially with other chemicals called synergists, which enhance the insecticidal activity of the pyrethrins and pyrethroids. The synergists prevent some enzymes from breaking down the pyrethrins and pyrethroids, thus increasing their toxicity. Because these compounds are broken down in the body quickly, there are several ways to measure the metabolites of these chemicals in human blood and urine. In this study the pyrethroid cypermethrin Sherpa 25% (active substance 250 g/l cypermethrin) was used, rabbits (1 kg weight), were gavaged by 1/20 LD50 for 3 weeks (one dose every week). Blood was collected before dosing and after 24, 72, 144 hours after the treatment. Enzyme activities were assayed in the plasma samples obtained. GOT, GPT, ALPH, CREA, GGT, Glucose and Total Pro were measured. Rabbits showed depression, decrease in feed intake, body weight and loose faeces. Livers exhibited fatty change, necrosis, lesions in kidney included tubular necrosis and pink homogeneous tubular casts. Serum ALT and creatinine concentrations increased while those of total proteins, albumin, serum cholesterol and triglycerides decreased.The results showed a decrease in RBC; WBC and Hb. This probably explained by the effect of cypermethrin on the erythropoiesis. An increase of plasma enzyme activities in GOT, GPT and CPK were recorded, explain a high energy-generating product. An increase, in the plasma enzyme activity in Alkaline phosphatase, related to their role in the cell permeability. The histopathological results showed lesions and morphological changes of hepato-cellular, fibrosis and appearance of inflammatory infiltrate, confirmed disturbances of the biochemical parameters. These changes were much underlines during the animal toxicity.
