The effects of isosorbide dinitrate single dose 120 mg daily and nifedipine 20 mg twice daily were studied in 17 patients with variant angina pectoris due to coronary artery spasm. After a placebo phase the patients were randomized to treatment with either isosorbide dinitrate or nifedipine. After six weeks the patients were crossovered for another six weeks period of treatment. There was significant decrease of number of angina attacks during both treatment regimens. Using 24 hours Holter monitoring we also proved significant decrease of number of ST segment elevation or depression, either symptomatic or asymptomatic. There was increase of performed work during exercise tests after both treatment periods. The efficacy of Isoket 120 mg and Adalat Retard 2 x 20 mg daily in the treatment of patients with active variant angina pectoris was comparable in our study. 3 patients suffered untolerable headache during isosorbide dinitrate phase and had to terminate treatment after first day only.

Bioelectric activity of the heart was measured in 39 healthy men, aged 25 to 45 years, during their exposure to head-down tilt. The first experimental study which continued for 182 days (-4.5 degrees) was performed on 18 test subjects, and the second study which lasted for 120 days (-4.5 degrees) on 21 test subjects. Both studies included controls (15 subjects) and subjects who exercised during head-down tilt (16 subjects). In addition, the second study also included 4 test subjects who took drugs to modify metabolic shifts and 4 test subjects who were in the drugs + exercise regiment. Prolonged hypokinesia led to a heart rate increase, A-V conductance decrease, and repolarization phase (T-wave) changes. Except for heart rate variations, these changes developed beginning with the first weeks of exposure, plateaued during 1.5 to 2 months and then varied in a sinusoidal manner, coinciding with similar changes in blood neurotransmitters and electrolytes. T-wave changes correlated well with K, Mg and Ca variations in blood. Exercises performed during head-down tilt decreased significantly (1.5-2 times) the level of T-depression and accelerated the restoration of ECG parameters during readaptation. Corinfar taken for two weeks had a noticeably beneficial effect on electrophysiological processes of the heart. The above electrophysiological changes that developed in healthy men during prolonged hypokinesia were of functional, reversible character and disappeared within the 2-month recovery period.

In 10 patients with stress stenocardia pharmacodynamics of Isoptin was studied with the aid of identical physical loads on the treadmill repeated with an hour's interval for 5-6 hours. A clearcut antianginal effect was recorded which consisted in the reduction of ST segment depression and of the pain syndrome for 5 hours after administration of 3 tablets over 5 hours and for 6 hrs 20 min after administration of 5 tablets of both drugs. The degree of effect increased considerably after the dose of isoptin was enhanced and was not changed after the dose of corinfar was enhanced. Difference in the haemodynamic effect of the drugs was found in their effect on the cardiac rate. Corinfar increased the cardiac rate, isoptin decreased it.

The microcirculatory state (by the method of conjunctival biomicroscopy) and ADP-induced platelet aggregation were examined in 42 patients with atherosclerotic and 34 patients with involutional depression both before and after the treatment which included vasoactive preparations and calcium antagonists. It was determined vascular and intravascular alterations predominance and increase of platelet aggregation in patients with atherosclerotic depression independently of leading syndrome, in patients with involutional depression with anxiety prevalence as well as in patients in stuporous state. The tendency was revealed of such disturbances increasing as for as psychosis was extended. Trental and Cavinton antiplatelet and spasmolytic properties were potentiated by inclusion of Corinfar in complex therapy and that promoted earlier microcirculatory alterations removal as well as aggregatograms and patient mental state normalization.

In 25 patients with transmural myocardial infarction a mean of 16 days after the onset of infarction an exercise stress test was performed 2 hours after the administration of Corinfar Retard. We evaluated ECG, blood pressure, heart rate, echocardiograms and spirographic parameters before and after drug administration. In the majority of patients ST segment depressions were reduced and duration of the exercise test increased or remained unchanged. In 5 patients the magnitude of ST segment depression was markedly decreased and associated with prolonged duration of exercise and substantial patient comfort. In these patients the drug is continued, as its withdrawal resulted in decreased exercise tolerance with accompanying chess discomfort. Corinfar Retard significantly decreased weigh blood pressure, both systolic and diastolic, whereas its effect on low blood pressure was only slight or none. Heart rate was higher before exercise, during and after exercise it did not differ from that measured without the drug. Ejection fraction increased by a mean of 4%; VC and FEV1 increased or remained unchanged. Exercise testing was safe, did not produce and any side effects and seems to be useful in deciding about the choice of drug after myocardial infarction, especially in patients with counterindications to beta blockers.
