The clinical course of a patient poisoned with the 'superwarfarin' brodifacoum and a method for estimation of plasma levels is described. It was characterised by prolonged depression of Vitamin K-dependent clotting factors poorly responsive to Vitamin K administration.

Hypoprothrombinemic changes in blood coagulation parameters, such as prolongation of prothrombin time, increase in the level of plasma protein induced by vitamin K absence, and decrease in plasma prothrombin level, were detected in rats fed a vitamin K-deficient diet. These changes were enhanced by the administration of beta-lactam antibiotics containing N-methyltetrazolethiol, thiadiazolethiol or methyl-thiadiazolethiol. Microsomal vitamin K epoxide reductase activity was suppressed with the maximum effect at 1-2 days after the treatment and with recovery, thereafter, gradually to the normal level after 5-7 days. Hypoprothrombinemic alterations in blood coagulation parameters following a single administration of antibiotic to vitamin K-deficient rats were somewhat delayed compared with the change in the epoxide reductase activity, but the effects of the antibiotic on both blood coagulation parameters and the enzyme activity disappeared completely 7 days after the antibiotic treatment. Antibiotic-induced depression of the epoxide reductase activity was observed even in the vitamin K sufficient rats, although the hypoprothrombinemic changes in the blood coagulation parameters did not develop. Vitamin K administration could normalize the blood coagulation parameters in the hypoprothrombinemic rats caused by treatment with the antibiotics but without recovery of the decreased epoxide reductase activity. These results suggest that some antibiotics inhibit liver microsomal vitamin K epoxide reductase, which causes hypoprothrombinemia to develop under vitamin K-deficient conditions.

Three trials were conducted to identify the critical vitamins in the diets of broiler-strain chicks fed alfalfa juice protein concentrate (AJPC) corn-soy diets from 0 to 3 wk of age. Vitamin supplements were added to AJPC diets. Diets were formulated to contain, parts 30/121, 40/128, 50/135, and 60/142 parts AJPC/parts total diet. Parts were used to permit usage of wet materials and still maintain about 90% dry matter. All diets were formulated to contain 20% crude protein, .93% total sulfur-containing amino acids, and 2,940 kcal metabolizable energy/kg diet. Propionic acid was added at .2% to all diets. Feeds were refrigerated (7 C) and fed out daily. The addition of choline, riboflavin, vitamin B12, vitamin A or E, folic acid, or biotin did not increase weight gains. Addition of 3 mg/kg vitamin B6 completely overcame the growth depression caused by the 50-parts AJPC diet and significant (P less than .05) growth increases (13 to 29%) were achieved with vitamin-B6 supplementation to the 60-parts AJPC diet. Depressed immune responses were completely prevented by the addition of 3 mg/kg of vitamin B6. The significant (P less than .05) increases in leg deformities observed in birds fed the 60-parts AJPC diet were also brought back to more typical values in birds fed the diet supplemented with 3 mg/kg vitamin B6. Vitamin K supplementation (.53 mg/kg) to the 60-parts AJPC diet resulted in significant decreases from 15 min in blood clotting times to 3 to 5 min.(ABSTRACT TRUNCATED AT 250 WORDS)

World-wide blood demands are increasing at 18% per year. Around 50% of collected blood is used whole -- for surgery, accident victims, anaemias, malignancies, and haemoglobinopathies. Blood component therapy is placing extraordinary demands upon Blood Collection Services. This therapy uses multiple or single cell types, and a variety of plasma-derived components. Malignant over-growth depresses marrow function; and, active treatment, temporarily, similarly depresses cell production. Specific transfusion cells are available for the patient's protective mechanism. Platelets are required, fairly fresh, for depletion and for their primary clotting ability. Similarly, infused platelets are used to counter local or systemic drug sensitive platelet depression. From plasma components, specific coagulation products are available for health and during surgery for haemophilia, Christmas, and Von Willebrand's diseases. Prior to oral surgery, all possible coagulation abnormality problems need investigation and specific factors. Anti-coagulation therapy must be reduced or stopped as indicated by the need for therapy and the availability of the appropriate, previously depressed, Vitamin K dependent-clotting factors (II, VII, IX, X). Local measures, formerly the only treatment, are now of less importance. Some countries do not have these blood factors available.
