Sixty-four patients with active distal ulcerative colitis participated in a multicentre, randomized, investigator-blind trial to compare the effect of budesonide enema, 2 mg/100 ml, with prednisolone disodium phosphate enema, 31.25 mg/100 ml.
Budesonide
is a new potent corticosteroid with a rapid first-pass elimination. The patients were treated for 4 weeks, and the efficacy of the drugs were evaluated by sigmoidoscopy, histology, and subjective symptoms after 2 and 4 weeks. After 4 weeks of treatment 16 of 31 patients (52%) receiving budesonide enema had healed endoscopically, compared with 8 of 33 (24%) (p = 0.045) receiving prednisolone enema.
Budesonide
was superior to prednisolone in terms of both significantly improved sigmoidoscopic and histologic scores and subjective symptoms evaluated by visual analogue scales. The patients receiving prednisolone had a significant
depression
of endogenous cortisol levels during the treatment period, but not the patients receiving budesonide.
Budesonide
enema seems to be a promising therapy for active distal ulcerative colitis and causes no adverse reactions.

A recently synthesized, non-halogenated steroid, budesonide, with high local anti-inflammatory properties, but with low systemic effect, was tested as a nasal spray in the treatment of patients with perennial rhinitis. Used in a dose of 200 micrograms and 400 micrograms budesonide per day in a double-blind cross-over study in 36 patients it was found to have an excellent and dose-dependent effect on nasal blockage and discharge as well as on sneezing bouts.
Budesonide
caused a considerable decrease in the nasal symptoms in 21 patients with demonstrable cutaneous and RAST allergy, as well as in a group of 15 patients without. The same alleviation of symptoms was noted in 22 patients with nasal eosinophilia but not in a group of 14 patients without. To determine possible adverse reactions during long term treatment, 15 patients with perennial rhinitis used budesonide as a nasal spray for more than two years. No adverse reactions serious enough to discontinue the treatment were noted in any of the patients. Haematologic parameters as well as adrenal gland function were monitored throughout the first 1.5 years. No
depression
was noted. Biopsy specimens of the nasal mucosae did not show any significant evidence of increased metaplasia.

In order to study the adverse reaction of a new, inhaled steroid (Fluticasone) on the pituitary-adrenocortical axis in asthmatic children, we investigated 7 children (aged 7 to 15 years) before and during treatment with Fluticason (100-200 micrograms/day). For the dosage tested, we found no
depression
of adrenal function, neither in circadian cortisol secretion nor in hCRH-stimulation-test. Another 7 asthmatic children under treatment with
Budesonide
(800 micrograms/day) were examined by the same tests. They equally did not show an adrenocortical suppression. However, in 4 other children under therapy with oral prednisone (2.5 to 7.5 mg/day), there was a marked suppression on adrenocortical function, even with low doses of the steroid. We conclude that Fluticasone (as well as
Budesonide
) in the above dosages represent a safe therapy for bronchial asthma in children.
