Dorsal column lesions in the high cervical region of the monkey result in severe defects of movements projected into space and contactual orienting reactions of the forelimbs. The hindlimbs are less affected provided a pathway through the lateral columns, Morin's tract, remains intact. Interruption of this pathway results in a defect of hindlimb function similar to that of the forelimbs. Cerebellar ablations in monkeys result in postural and movement disorders, including hypotonia of limb extensor muscles. An important mechanism underlying the hypotonia is a
depression
of the responses to muscle extension of spindle primary afferents owing to a decrease of fusimotor activity. In the decerebellate animal abnormalities of limb trajectory during active movements projected into space (cerebellar "dysmetria") appear to result principally from dysfunction of systems separate from the peripheral fusimotor efferent-spindle afferent reflex arc. Precentral cortical ablation results initially in a contralateral hypotonic hemiparesis, later in a hypertonic hemiparesis. A

of the responses of muscle spindle afferents occurs during the hypotonic phase, but during the hypertonic phase spindle function returns to normal levels. Accordingly a
depression
of fusimotor function appears to be important in the hypotonic phase of
hemiplegia
; however, there is no evidence that an enhancement of fusimotor function underlies the hypertonic phase. Bilateral section of the medullary pyramids results in an enduring hypotonic paresis. Abnormalities of contactual orienting responses of limbs are similar to those following dorsal column lesions. Responses of spindle primary afferents are depressed during the initial stages after acute pyramidotomy, then approach but do not reach normal levels. It is concluded that the dorsal columns constitute an afferent, and the pyramidal tracts an efferent, pathway important in oriented contactual reactions of the limbs. The hypotonia resulting from cerebellar lesions, precentral ablation, and pyramidal tract section stems, at least in part, from a
depression
of the fusimotor innervation of muscle spindle afferent activity.

and aphasia. After two weeks, he had hemiparesis, word-finding and naming problems and enuresis. A year later he demonstrated elective mutism at school, had attention and short-term memory impairments, occasional enuresis and an average IQ. He was shy and withdrawn; this is interesting, since
depression
is usually associated with left-hemispheric lesions. It is suggested that an early period of mutism should be included among the criteria for the study of crossed aphasia in children, as this is a common occurrence in such cases. Even after recovery of speech, impairments in attention and academic skills may persist.

Three cases of anterior choroidal artery territorial infarction, diagnosed by computerized tomography, with the triad of

, hemianaesthesia and hemianopia, pure motor stroke and ataxic hemiparesis are described. Major and minor (lacular) infarctions in the territory of the anterior choroidal artery involve almost exclusively the basal segment of the posterior limb of the internal capsule and manifest themselves by symptoms of long pathway lesion. Based on the published case reports and the authors' own observations, the complete capsular syndrome characterized by the triad of
hemiplegia
, hemianaesthesia and hemianopia was differentiated from partial capsular syndromes including the following forms: pure motor stroke, pure sensory stroke, sensorimotor stroke, sensory stroke with hemiataxia, ataxic hemiparesis, dysarthria and/or clumsy hand, and homonymous hemianopia (quadrantanopia or sectoranopia). The characteristic features of the above types of capsular syndromes were analyzed. Distant symptoms of territorial infarctions involving the anterior choroidal artery are transcortical sensory or motor aphasias and construction apraxia in the dominant hemisphere, left side perception failure and visual-construction apraxia in the non-dominant hemisphere, and cerebellar hemiataxia. These distant symptoms are a manifestation of distant cortical or cerebellar metabolic

The 159 patients with Parkinson's disease with onset after the age of 50 (mean: 62.3 years) were studied with reference to diagnosis and treatment. The results were as follows: 1. Other than the characteristic features, the symptoms at onset were
depression
(6% of the patients), lumbago (4%),
hemiplegia
-like (4%) and dizziness (3%), causing misdiagnoses in some of the patients. Among 159 patients studied, the severity most frequent was Yahr stage 3 (63%) at first examination, indicating the necessity of earlier diagnosis. 2. Magnetic resonance imaging (MRI) of the substantia nigra and striatum was investigated using a 1.5 Tesla field and T2-weighted images, which gave no specific results concerning diagnosis and severity. However, it was useful in differential diagnosis between this disease and parkinsonism caused by multiple system atrophy and cerebrovascular diseases. 3. With 123I-IMP SPECT, decrease in blood flow in the frontal and temporal lobes correlated with the severity (Yahr stage) of the disease. Regarding cognitive functions the scores of Hasegawa's Dementia Scale and Mini-Mental State showed a highly significant correlation with the amount of blood flow in frontal and parietal lobes, suggesting that dementia might be caused by dysfunction of these lobes. 4. In 98 patients treated with levodopa mixed with dopa-economizers for more than a year, the maximum improvement was small in severely disabled patients of Yahr stage 5 and 4 because none improved to stage 3A or below (3A is an arbitrary criterion meaning mild involvement in stage 3 with 3P meaning more severe cases).(ABSTRACT TRUNCATED AT 250 WORDS)

We report the pictorial evolution of a well-known French painter, before and after a left
hemiplegia
. Before the stroke he was famous not only for his drawings but also for his paintings. The evolution comprised 3 periods. Just after the stroke, the painter presented with severe exacerbation of a chronic
depression
. The second period was characterized by a rich pictorial production, with an excellent spatial organization of drawings; the style remained well recognizable and the favourite topics were unchanged: streets' scenes, country women, bunches of flowers. Shapes, drawings and volumes were well preserved. The neglect concerned not only the left hemispace but also the "microspaces" on the left side of which the painter's gaze attention had been fixed. Analysis of the production for this period supports the hypothesis according to which the neglect would be explained by a selective defect in automatic orientation of the attention on the left side. In the last period the aggravation appeared to bear more on the instrumental processes than on the esthetic capacities.

The purpose of this study was to describe and compare the gait of 20 patients with
hemiplegia
secondary to cerebrovascular accident (CVA) before and after a treatment regimen of resisted pelvic motions. Ten women and 10 men were studied, with a mean age of 48 years and a mean duration post-CVA of two months. Nine subjects (45%) were right hemiplegic, and 11 subjects (55%) were left hemiplegic. Treatment consisted of four sets of five repetitions each of manually resisted pelvic anterior-elevation and posterior-
depression
movements on the involved side. An insole footswitch system, knee electrogoniometer, and force walking aid were used in gait analysis performed before treatment, immediately after treatment (posttest 1), and 30 minutes after treatment (posttest 2). Results showed significant overall improvement in gait in posttest 1 (p less than .005) compared with the pretest. This improvement, however, was not maintained in posttest 2. Ten patients improved overall in posttest 1; only 4 patients also showed improvement in posttest 2. The major improvements seen immediately after treatment were observed in stance stability and limb advancement in the involved limb. More research is needed to identify an optimum treatment with carry-over using this technique.

Patients with ovarian carcinoma refractory to chemotherapy received a sequential combination of ethinyl estradiol and medroxyprogesterone acetate in two dose regimens. There was no difference in therapeutic activity of the two dose regimens. Of 65 patients, nine (14%) responded to treatment and 13 (20%) had stable disease. Vascular complications occurred in three patients;
hemiplegia
developed in one of those. Nine patients had significant nausea and vomiting, and one experienced severe
depression
that required treatment withdrawal. The sequential and combined use of ethinyl estradiol and medroxyprogesterone acetate may provide an alternative treatment for certain patients with ovarian carcinoma that does not respond to optimum chemotherapy. Additional studies are required to determine if synergism exists between this treatment and other modalities of therapy. Further investigation is required into the vascular disorders that complicate therapy to determine whether appropriate preventive measures are possible.

. On average, IQ was depressed and the magnitude of this

was highly correlated with lesion size, degree of hemiparesis and EEG abnormality, but not with location of lesion. There were no significant effects of lesion laterality on Verbal vs. Performance IQ on Wechsler tests. However, receptive vocabulary, as measured by the Peabody Picture Vocabulary Test, was differentially depressed by left-hemisphere damage. Further, on a variety of verbal tasks, patients with congenital lesions performed better than those with acquired lesions. In contrast, no significant differences were found between the two groups on spatial tasks.
