Cognitive arousal has been associated with disrupted sleep in individuals with insomnia and may be one mechanism underlying sleep disturbances in
depression
. We evaluated the relationship of sleep to bereavement-related intrusive thoughts and avoidance behaviors in 40 men and women with major depression consequent to bereavement. Mean subject age was 65 years, 25% of the same was male, and median time since loss was 7.4 months. Levels of intrusive thoughts and avoidance behaviors were similar to samples seeking treatment for stress response syndromes, including post-traumatic stress disorder. After controlling for the effects of age, time since loss, and
depression
severity, greater frequency of bereavement-related intrusive thoughts and avoidance behaviors was associated with longer sleep latency and lower delta sleep ratio (P values < .01).
Intrusive thoughts
and avoidance behaviors were not strongly related to gender, time since loss,
depression
severity, or subjective sleep quality. These results suggest that intrusive thoughts and avoidance behaviors affect sleep in bereavement-related
depression
and may therefore be related to the clinical course of
depression
.

This report describes the reactivation of a posttraumatic stress disorder (PTSD) after a minor head injury in two young women who had recovered from extreme stress caused by sexual abuse during adolescence.
Intrusive thoughts
, images, dreams, and phobic avoidance bear a direct relationship to the specific circumstances of both head injury and sexual abuse, and were associated with obsessive-compulsive symptoms, generalized anxiety with panic, and
depression
. These findings suggest that in some individuals minor head injuries may induce not only extreme stress reactions, but also cause the reactivation of symptoms related to previous traumatic experiences.

Intrusive thoughts
(e.g., anxiety,
depression
, substance use) are among the most salient symptoms of clinical problems. Therefore, methods of thought suppression have received considerable attention. However, some studies have found that attempts to suppress thoughts precipitate an increase in thoughts. In the case of substance abuse, such thoughts could serve as cues for further use. This study examined the association between thought suppression and quitting smoking in a sample of current and ex-smokers. Using the White Bear Suppression Inventory (WBSI), scores were obtained for participants' level of thought suppression. Based on the idea that a greater tendency to suppress thoughts would make quitting smoking more difficult, it was hypothesized that unsuccessful quitters would have higher WBSI scores than smokers who had successfully quit. It was found that mean scores on the WBSI were significantly higher (P<.05) for smokers than ex-smokers. These findings suggest that a tendency toward thought suppression may increase the likelihood that attempts to quit smoking will be unsuccessful.

Intrusive thoughts
are one of the re-experiencing symptoms in posttraumatic stress disorder, and have been suggested as a predictor for the continuous presence of psychological distress in cancer survivors. The aim of this preliminary study was to examine the possibility of using cancer-related intrusive thoughts (CITs) as an indicator of psychological distress and adjustment after breast surgery. A consecutive series of ambulatory breast cancer survivors at 3 or more years after surgery were given the Structured Clinical Interview for DSM-IV (SCID) and self-report questionnaires, including Profile Of Mood States (POMS), Impact of Event Scale (IES) and Mental Adjustment to Cancer (MAC) scale, to measure psychological distress and/or adjustment. The SCID identified a history of CITs in 34 (46%) of the 74 participants. No significant differences were found in the POMS and IES psychological distress scores between subjects with and without a history of CITs. Subjects with a history of CITs showed significantly higher levels of anxious preoccupation, one of the MAC subscale scores. The association continued to be significant after controlling for potential confounders such as social support,
depression
, avoidance, arousal, and neuroticism. The results indicated that CITs might be useful for indicating poor psychological adjustment, but not distress, in patients at 3 or more years after breast surgery.

This study explored the mediation effect of metacognitive beliefs on the relationship between intrusive thoughts and emotional distress in schizophrenia (N=49) and obsessive-compulsive disorder (OCD) (N=35).
Intrusive thoughts
impact on anxiety and
depression
through beliefs about uncontrollability and danger of thoughts in schizophrenia. Negative beliefs in general mediated the effect of intrusive thoughts on anxiety in obsessive-compulsive disorder. The results suggest that metacognitive beliefs may be a vulnerability factor for emotional and psychological disorder.
