Second, addition of Mg2+, which abolishes the oscillations, has a much more marked inhibitory effect on the process of cation efflux than on cation influx. Conversely, addition of A23187, which removes membrane-bound Mg2+, promotes cation efflux and thus the oscillations. It is suggested that, in the present system, stretching of the inner membrane and Mg2+ depletion result in activation of an electroneutral H+/organic cation exchange, and that cyclic activation of this reaction results in damped oscillations.

Infections and chronic liver injury are common causes of morbidity and mortality in alcoholics, and both of these may be related to an altered immune response. This study describes a guinea pig model of chronic ethanolism designed to selectively study the cellular immune system in a setting free from the malnutrition, socioeconomic deprivation, and severe underlying hepatic dysfunction seen in human disease. Animals were given 2.5 g/kg/day of ethanol as a 15% solution in 0.9% NaCl or isocaloric-dextrose-saline control solution intraperitoneally in 2 divided doses for 5 weeks. At 2 weeks, the mean serum ethanol level 1 hr after treatment was 20.4 mM (range 8.9-30.6) while the mean serum acetaldehyde level was 55.1 microM (range 17.0-111). At 5 weeks the serum levels for ethanol and acetaldehyde were 20.1 mM (13.3-32.9) and 41.5 microM (2.4-87.6), respectively. Weight gain was persistent throughout the study and did not differ significantly between ethanol and control groups. After 5 weeks of treatment, lymphocyte response to the mitogens, phytohemagglutinin, and concanavalin A was significantly decreased in the ethanol treated group (p less than 0.05). The effect of lysine and tryptophan addition to an all-corn diet fortified with vitamins and minerals fed to gestating gilts and sows was studied in a series of five trials. The experiments were designed to establish the effect of the addition of the two amino acids on: 1) N balance, 2) reproductive performance over two consecutive parities and 3) the immune response of the gestating gilt and transfer of immune proteins to the nursing pig. Nitrogen retention by gravid gilts fed an all-corn gestation diet increased (P less than .05) in response to lysine addition, but was not affected by subsequent addition of tryptophan. Daily N retention of gravid gilts fed the corn or corn and amino acid-supplemented diets was lower than that of gilts fed a 12% crude protein (CP) diet. Reproductive performance for two parities, as evaluated by gestation and lactation weight gain and, number and weight of pigs at birth and at 28 d was similar among treatments. In carcinoma of the esophagus, response to in vivo sensitization with recall antigens and DNCB was markedly depressed with 13% and 16% positivity respectively. Levels of IgA did show a downward trend following enteral hyperalimentation. Circulating immune complexes and serum CEA level were elevated in almost 50% of patients. These data confirm the influence of tumor-related impairment of cell-mediated immunity while nutrition appears to affect IgA levels.

A nutritional support team was used in the assessment and management of patients on a general urological service. Indications for nutritional evaluation included history of weight loss, anorexia, significant infection, chronic neoplastic disease, trauma or major surgery. The fat and protein status of the patient was assessed by anthropomorphic and laboratory determinations. The patient then was categorized as having mild, moderate or severe degrees of nutritional depletion. Deficiencies in vitamins, trace elements or essential fatty acids were not noted. Caloric and protein needs were calculated by multiplication of the basal energy expenditure by a metabolic activity factor, which was derived from the degree of illness or stress. Nutritional support was provided by enteral feedings via oral, nasogastric or jejunal feeding tubes and/or intravenous hyperalimentation via peripheral or central venous nutrient lines. During a 6-month interval nutritional consultation was requested for 50 patients, who represented 7 per cent of the urological admissions. Nutritional support was provided for patients who had obstructive uropathy with or without neoplasms, radiation cystitis, sepsis, urinary fistulas, mental

of delayed cutaneous hypersensitivity (DCH) to seven common recall antigens was found in 50 patients with rheumatoid arthritis (RA) compared with 50 matched controls. Complete skin anergy was seen in 12 (24%) of the RA patients but in none of the control subjects. In addition, a nutritional assessment of all subjects tested showed significantly lower

and body mass indices in the RA compared with the control group. Analysis of these 100 subjects showed a significant correlation (r = 0.53, p less than 0.001) between the number of positive skin reactions and the concentration of

suggesting an association between nutritional status and DCH. No single factor could be identified to distinguish the reactive and anergic RA patients with the possible exception of drug treatment in that a higher proportion of patients on sodium aurothiomalate or immunosuppressive therapy than D-penicillamine was found in the anergic group. It is likely that the cause of impaired DCH in RA is multifactorial and our findings suggest that drug treatment and nutritional status may be an important contributor to it.
