The conditioning problems of suicidal acts of aged people were described by demonstrating the results of investigations in 125 patients (over 65 years). The control group consisted of 3004 suicidal patients below 65 years.
Depression
has been the prevailing
psychiatric diagnosis
(50% in contrast to 33% in the control group). 24% of the patients alleged conflicts with partners or other relatives as a motivation for the suicidal act. Fear of somatic illness or social isolation follows next. It is remarkable that vocational or economical conflicts were not mentioned. At last prevention problems were discussed. The need for modifying the attitude toward the aged was stressed.

The relationship between diagnosis and abnormally high fasting blood sugar levels was reviewed for 4,994 patients in a mental hygiene clinic. Fifty-four patients exceeded the criterion level of fasting blood sugar abnormality, FBS greater than or equal to 145 mg%. There was no evidence of relationship between
psychiatric diagnosis
and the presence of an abnormally high fasting blood sugar level. This held true for the diagnosis of
depression
despite observations made elsewhere that a significant association exists between the depressions and diabetes.

Confidence in the assignment of lifetime
psychiatric diagnosis
is of great importance to genetic studies of psychiatric illness. To establish the credibility of a lifetime psychiatric history obtained via a structured interview, two paradigms were constructed to estimate reproducibility of the interview recording process. The first paradigm, simultaneous coding, was used to test comparability of four interviewers independently coding an interview form. Low variance/high reliability was demonstrated. The second paradigm, test-retest, provided for each subject to be interviewed twice, with a mean interim time of 6.7 months (SEM = .39). This paradigm demonstrated high reproducibility of
psychiatric diagnosis
over time. The overall k value for measurement of diagnostic agreement was .79. Only the diagnostic category of minor
depression
seemed to evade reliability. It was shown across both paradigms that an interviewer need not be blind (naive to previously held diagnosis) to obtain an unbiased interview. However, it is still recommended that the diagnosis of each interview should be determined by an independent diagnostician.

The evidence for disturbances of tryptophan and 5-hydroxytryptamine in
depression
and for various mechanisms by which such disturbances could occur is discussed. Two recent relevant studies in the author's laboratory are described: a) non-esterified fatty acid and total and free tryptophan were determined in plasmas of psychiatric patients unselected with respect to
psychiatric diagnosis
before and after a stress situation. Retarded patients had significantly low total and free tryptophan values which correlated negatively with agitation. Total tryptophan fell significantly after stress in the non-retarded subjects. The only biochemical abnormality significantly associated with a diagnosis of primary
depression
was the rise of plasma non-esterified fatty acid after stress. Thus tryptophan abnormalities were associated more with psychiatric rating scores than with diagnoses. b) Determinations on plasma and lumbar and ventricular CSF from psychiatric patients undergoing psychosurgery indicate that low plasma free tryptophan concentrations are associated with decreased 5HT turnover in the central nervous system.

Functional overlay is not a recognized
psychiatric diagnosis
. Evaluating functional overlay and differentiating between this concept and organic conditions is important in medicolegal areas in which financial values are placed on pain and disability. Functional overlay is not malingering: the former is based on preconscious or unconscious mechanisms, the latter is consciously induced. In considering psychologic reactions to pain and disability, a gradient of simulation, malingering, symptom exaggeration, overvaluation, functional overlay and hysteria is useful. The dynamics of overlay are a combination of anxiety from body-image distortion and

At least one
psychiatric diagnosis
was made for 205 of 220 children whose psychiatric evaluation had been requested by the medical service. Only 78 of 242 psychiatric diagnoses given the 205 patients were reflected correctly in the medical discharge diagnoses. In addition, seven of fifteen patients considered to be "normal" by the consulting psychiatrist had a psychiatric or mixed medical-psychiatric diagnoses included in the discharge diagnoses. Psychophysiological disorders, psychoses and special symptom diagnoses were likely to be correctly reflected in the discharge diagnoses, while

It has been mentioned that herpes simplex antibodies are increased in depressed patients. In the present study, serums from 21 patients with reactive depressions, nine with psychotic depression, 33 schizophrenic and 15 normal controls were studied, and herpes simplex antibodies were measured. Neither was statistically significant difference found between any of the groups nor could antibody titres be correlated with severity of
