The factorial and discriminant validity of the Center for Epidemiological Studies
Depression
(CES-D) scale was examined for a sample of 116 parents who were participating in family support programs designed to prevent child abuse and neglect. Participants' self-reports of depressive symptoms as measured by the
CES
-D were analyzed in relation to their self-esteem (measured with the Rosenberg Self-Esteem scale) and state and trait anxiety (measured with Spielberger's State-Trait Anxiety Inventory). Factorial validity was adequate, and results indicated a moderate correlation between the
CES
-D and self-esteem and state anxiety. However, a high correlation was obtained between the
CES
-D and trait anxiety, which suggests that the
CES
-D measures in large part the related conceptual psychological domain of predisposition for anxiousness.

This study examined the inter-observer reliability and validity of the Center for Epidemiologic Studies
Depression
Scale (CES-D) as a measure of depressive symptomatology in stroke patients, and its utility as a screening tool for
depression
in this population. The
CES
-D Scale is a brief questionnaire originally designed for use in community surveys. Twenty-seven non-aphasic patients enrolled in the Stroke Data Bank at the University of Maryland were interviewed by a research nurse using the
CES
-D. On the same day, each patient was independently evaluated by a research assistant using a psychiatric battery for
depression
and measures of cognitive, physical, and social functioning. Forty-one percent (11/27) of the patients were depressed according to clinical criteria for major or minor
depression
. With a cutpoint corresponding to the upper (most severe) 20% in community surveys, the
CES
-D Scale picked up 73% (8/11) of the depressed patients. In this sample no nondepressed patient scored over 16 on the
CES
-D (no false positives). The
CES
-D Scale scores correlated significantly with the other
depression
measures (r = .57 to r = .82, p less than .002) and did not correlate with the measures of cognitive, physical, or social functioning. Based on 24 patients who received a
CES
-D Scale score from both the nurse and the research assistant, inter-rater reliability was high (r = .76, p less than .001). Thus, the
CES
-D was found to be reliable and valid as a screening tool for assessing
depression
in stroke patients.

We investigated the association of social supports and stresses with depressive symptoms in a sample of 111 predominantly low-income mothers of young children. The prevalence of high depressive symptoms, as measured by the Center for Epidemiologic Studies--
Depression
Scale (CES-D), was 48 per cent. Among unmarried women, everyday stressors were strongly associated with depressive symptoms, while life events were weakly related. Associations between these variables were not found for married women. Comparing the relative importance of two different types of support--the quality of primary intimate relationships, and the social network--only the quality of the husband-intimate relationship was associated with
CES
-D scores among married women, whereas the social network demonstrated a moderate, inverse association with
CES
-D scores among unemployed women.

An unbiased evaluation of hospice effectiveness requires a randomized controlled trial (RCT), but the implementation of such a design is difficult. This paper describes the methods of the only RCT to date and reports on the validity and reliability of scales selected or developed to assess attainment of hospice goals. The patients involved are those in a VA hospital in Los Angeles who have cancer and a prognosis of about 6 months and who consent. They and their significant others are interviewed periodically until the patient's death to measure such hospice variables as pain,
depression
, anxiety, satisfaction with care and the environment, activities of daily living, and bereavement effects. Many of the scales, including McGill Pain,
CES
-D
Depression
, and Satisfaction with Care scales, have been validated in other studies with similar samples. The authors' test-retest reliability analyses show that these scales are reliable with advanced cancer patients and their significant others with coefficients ranging from 0.77 to 0.97.

In this study,
depression
among patients visiting general practitioners was examined by administering a short self-report measure of
depression
to 1,250 patients visiting 37 general practitioners in three Canadian centres (Winnipeg, Brandon and Virden). According to scores on the
CES
-D scale, 66.8% of the patients were judged to be normal, 11.8% were experiencing mild
depression
, 13.3% moderate
depression
and 8.1% were experiencing fairly severe
depression
.
Depression
scores were highest among female patients, patients who lived in Winnipeg, patients who were unemployed and patients who were divorced or separated.

The Center for Epidemiologic Studies'
Depression
Scale (CES-D) was developed to measure depressive symptoms in community populations. To be useful for epidemiologic studies of
depression
, the scale should measure the same thing in various subgroups. This study examines the psychometric properties of the
CES
-D among men and women, including its factor structure, reliability, and characteristics of its subscales. A national sample of 1,360 married men and women, collected in 1978, was used. Factor analysis produced four major factors in the
CES
-D: depressed affect, enervation , lack of positive affect, and interpersonal problems. These factors are generalizable across men and women with two exceptions--crying spells, which are a good indicator of depressed mood for women, do not indicate depressed mood for men; and feeling one's life is a failure is associated with depressed affect for women and with a lack of positive affect for men. The two items in the interpersonal factor (feeling that people dislike you and that people are unfriendly ) do not show the expected pattern of association with gender, since men have significantly more interpersonal symptoms than women. Women have more symptoms of depressed affect, enervation , and lack of positive affect.

The dearth of population-based studies and epidemiological investigations on the mental health problems of Asian-Americans, especially since the change in the immigration laws in 1965, has led to contradictory speculations about the prevalence rates of mental illness and the general mental health status among Asian-Americans, as opposed to other segments of the population. We administered the Center for Epidemiologic Studies
Depression
(CES-D) scale to 499 samples drawn from a Northwestern coastal city in order to make an initial assessment of the amount of
depression
experienced by Asian-Americans. The investigation compared the Asian-Americans'
CES
-D scores with those of whites and other minority groups, examined the scale's patterns of factor loading by ethnicity, and discovered that, even with statistical controls, there exists a distinction among the individual groups of Chinese, Filipinos, Japanese, and Koreans with respect to their score averages of depressive symptoms.

Although several self-rating scales for
depression
have been proposed in the past two decades, very few have been validated on non-English-speaking populations. The present study was carried out in northern Italy in order to examine the validity of the
CES
-D self-rating scale for
depression
in another language and culture. The scale was used with 40 depressives and 40 normals who were matched. The Hamilton Rating Scale for
Depression
(HRSD) was used as an additional test of concurrent validity. The results show that the
CES
-D is a valid measure in that it sensitively discriminates between depressed patients and normals and presents satisfactory correlations with the observer rating scale (HRSD) in both groups. The scale in its Italian translation is likely to be helpful in the assessment of
depression
of Italian immigrants in North America and Australia, especially in those whose English is poor.

A total of 1921 consecutive adult patients seen in the Departments of Internal Medicine and Obstetrics-Gynaecology at a prepaid group practice completed a self-administered depressive symptomatology questionnaire, the Centre for Epidemiologic Studies
Depression
Scale (CES-D). New health practitioners and physicians, who were not mental health specialists, blindly judged the presence or absence of depressive symptomatology. Twenty-one per cent of the patients suffered from depressive symptoms according to the
CES
-D, but only 15% of these were judged to be depressed by their physician or new health practitioner. The variables which predict the recognition of depressive symptomatology are discussed.

The authors gave the Center for Epidemiologic Studies
Depression
(CES-D) Scale, a self-report
depression
symptom scale, to 528 subjects drawn from a larger longitudinal community survey. Interviewers also assessed respondents using the Schedule for Affective Disorders and Schizophrenia (SADS), a structured clinical interview; on the basis of the SADS the subjects were then given diagnoses according to the Research Diagnostic Criteria. The results corroborated earlier findings of a modest relationship between self-reported symptoms of
depression
and the diagnosis of
depression
. The authors conclude that the
CES
-D Scale may be useful as an initial or first-stage screening test.
