This second paper on the reassociation of dreams confirms 1. the value of reassociating a key dream (The Beetle Bug Dream) and 2. the use of lysergic acid diethyl amide (LSD) as an adjunct to psychoanalytic therapy. The patient (Joyce) was a young mother whose very severe eczema and asthma were accompanied by an incapacitating depression. The following summary of the three interviews indicate briefly the psychodynamic materials developed which helped Joyce uncover her confused sexual identifications and fear of lesbianism. In Interview 96 the initial Beetle Bug Dream was studied cautiously. On analysis the tentacles of the Beetle Bugs symbolically represented the hands of a man mauling a girl's skin. A sexual approach by a man is considered an attack. "Bugs get under the skin. If Jack (my husband) touches my skin, that spot itches." A second Beetle Bug is described. The special aspect of this unique Bug is that it was not threatening. This Bug is discussed in terms of the transference: the silent analyst and a father who responded to her childhood travail by silence are compared. Reassociation of the Beetle Bug Dream without LSD took place one year later. The session (Interview 235) lasted about 45 minutes and the verbatim recording occupied 12 typewritten pages. Free association led to the realization that the patient was programmed in part to identify with her father--a silent, passive individual. It was apparent that a psychological struggle for femininity, induced by the confused identification led to a threat of penetration by the sting of the Beetle Bug and, where a man was involved, penetration by the penis. Interview 236 (three days later) was conducted under LSD 25. The Beetle Bug Dream again formed the basis of this interview, which lasted four hours and occupied 46 pages of typewritten, verbatim material. A new feeling of independence from maternal influence is felt. "It used to be if I just thought of her, I could start scratching." The relationship with her husband is complicated by "deep, dark secrets." She is afraid of her husband. Her mother emphasized she was "one" with her father. Contact with aggressive males during adolescence, as well as seductive episodes were disturbing. In camp she feared that she might be a lesbian. This fear persisted into adult life. The threat of the Beetle Bug sting is developed further. Hostile feelings toward the penis are revealed in detail, The part of the penis in the vagina could be chopped off--the penis was what the sting of the Beetle Bug would be if it penetrated the skin. The bridegroom attacks the bride. An unwanted pregnancy is equivalent to the broken sting remaining in the skin. These revelations led to insight regarding unusual violent anger toward her child during toilet training. The data support the view that publication of verbatim recordings are a necessary condition for establishing psychoanalytic procedures as a basic science of medicine and of the validity of psychodynamic theory...

All physicians should be aware of the possible complications of induced abortions if only because the procedure is so commonplace. Some 250,000 induced abortions occur annually in France, amounting to 24.4 abortions per 100 live births. The rates of different complications of induced abortions before 12 weeks are .5-5/1000 for uterine perforation, .5-3.4% for hemorrhage with or without placental retention, 1% for endometritis, .3% for salpingitis .5% for continuing pregnancy, and .006 to .3/10,000 for death. A well done curettage is preferable to a poorly performed aspiration procedure. If an aspiration is done, the practitioner should bear in mind that retention of 50-200 cc of blood clots may occur if dilatation is insufficient. Symptoms appear 1-5 days after the abortion and end with expulsion of the clots or aspiration. Curettage is useless, as the clots do not represent a true retention. Uterine contractions during the aspiration can occasionally prompt a premature decision that evacuation is complete. Retention is difficult to diagnose immediately after aspiration but can be sonographically confirmed after the 8th day. Aspiration should be done after the 6th week and before the 12th week. Aspiration before the 6th week is often painful and is associated with higher rates of partial retention and of complete failure. Endouterine aspiration, regardless of technical proficiency, establishes a pathway between the vagina and the uterine cavity, which exposes the latter to the risk of trauma, endometrial lesions, and perforation. Induced abortion promotes infection by 2 mechanisms. Latent infections that were not detected in the medical history or physical examination can emerge and cause endometritis, which should be treated by ice, rest, and antibiotics. Or contamination of the passage by an infected cervical mucus can lead to salpingitis, abscess, and pelviperitonitis, or even general peritonitis. More often, these conditions develop from inadequately treated nonretentional endometritis. The condition should be treated with antibiotics and ice. Postoperative hemorrhage is unusual and is most frequently caused by retention. Psychological complications of abortion can be minimized by effective counseling. The counselor should seek to identify any history of psychological pathology or particularly stressful current situation. A certain amount of regret is a normal psychic response to abortion, but more serious symptoms such as suicidal thoughts or obvious depression may indicate the need for specialized care. Experience demonstrates that serious psychic reactions are rare and that a population at high risk can be defined. It includes very ambivalent women, those coerced into abortion, and those at the legal time limit. Women with a recent history of death or illness of a child, intrauterine death in the preceding pregnancy, or spontaneous abortions are also at risk.

On the basis of the studies of the embryogenesis of the vagina, the authors consider that malformations classically described as being partial aplasia should not be separated from the total absence of the vagina. The important feature is the association of a functioning or non functioning uterus with the absence of the vagina. They believe that it is incorrect to describe the pouch of menstrual retention associated with a functioning uterus as "haematocolpos" and that is not justified to describe the cup-shaped vestibular depression as "hemi-vagina". According to the authors, although vaginal aplasia with a functioning uterus forming a pouch of menstrual retention constitutes an absolute indication for surgery, surgery is not justified in cases of vaginal aplasia with a non functioning uterus. If Frank's method fails in these cases, the patient or the couple should be referred to a sexologist, as women with this anomaly retain a perfect femininity, although unable to conceive.

A pantropic, attenuated RNA virus [TC83 strain of Venezuelan encephalitis (VE) virus] grew in hamster testes after intratesticular inoculation. Virus did not grow in testes after subcutaneous injection nor in uninoculated testes after unilateral testicular injection. Although viremia disappeared within 6 days when virus-neutralizing antibodies appeared in serum, viral growth in testes continued for 2-3 weeks after intratesticular inoculation. Virus also persisted for 12 days in testes of hamsters immunized 4 weeks previous to intratesticular inoculation. Thus, there seemed to be a barrier between germinal epithelium and blood to early virus-neutralizing antibodies which developed within 2-4 weeks of infection. Cytonecrosis, inflammation, depression in the number of germinal cells, atrophy, fibrosis and eventual sterility occurred with viral growth in testes of normal hamsters, but no histopathology other than atrophy and aspermia was seen in epididymides. Similar changes occurred in testes of some previously immunized hamsters. Virus antigen was found in diploid, but not haploid germinal cells, and virus (either attenuated or virulent VE) did not adsorb to or grow in sperm maintained in vitro. Males with infected testes and virus in the epididymis remained fertile for about 2 weeks after inoculation. Virus was only occasionally transferred to the uterus-vagina of normal females by copulation with males with infected testes, and only rarely did such females become infected. Usually, pregnancy developed and progressed normally, and no convincing transfer of virus to progeny occurred. In normal female hamsters inoculated intrauterinely or subcutaneously, virus did not reach higher concentrations in uteri than in blood, virus disappeared concurrently in both and no histopathology developed in uteri. However, infection of females by intrauterine inoculation of attenuated VE virus 2 hours before mating prevented pregnancy whereas intrauterine inoculation of saline or an adenovirus did not.

Low threshold (0.5-15 g) mechanical punctate stimulation of the labia and vagina depressed dorsal horn neurones in the cat with spinal transection at L1. Similar stimulation 2 mm or more from the outer margin of the labia was without effect. The depression was observable within 1-2 sec, and usually required 4-6 min for full recovery, although a smaller more prolonged depression may also have occurred. Stronger stimuli evoked greater inhibition, requiring longer times for recovery. Vaginal stimulation depressed 9 of 10 sensory dorsal horn neurones (including non-nociceptive, wide dynamic range and nociceptive specific). Depression included a decrease in spontaneous activity and a decrease of naturally elicited responses to noxious and innocuous cutaneous stimuli; therefore the effect is not modality related. One unit without sensory input was unaffected by vaginal stimulation. In the one case tested, vaginal stimulation depressed glutamate-evoked excitation, suggesting that the depression of naturally, elicited responses was via a postsynaptic mechanism.

Persistent bleeding from the vulva was the only presenting clinical sign in five non-pregnant pluriparous mares varying in age from eight to 20 years. These were two hunter types, one shire, one thoroughbred and one Arab pony. The haemorrhage originated from ulcerated varicose veins present on the dorsal wall of the vagina adjacent to the vestibulovaginal junction. All five mares were successfully treated, by submucosal resection (two), ligation of vessels (two) or diathermy (one). In four mares there was evidence of vulval incompetence caused by depression of the perineum. The importance of this and the role of impaired venous return during and after pregnancy are discussed.

While radiotherapy and antineoplastic chemotherapy often control malignancies they may, paradoxically, cause new cancers to develop as long-term complications. Although almost any type of neoplasm can occur, radiation-induced malignancies are most likely to affect the myelopoietic tissues and the thyroid gland. The former tissues are also most frequently involved by chemotherapy. The combination of intensive radiotherapy and intensive chemotherapy is particularly leukemogenic. Acute myeloid leukemia has occurred with increased frequency following treatment of Hodgkin's disease, non-Hodgkin's lymphoma, multiple myeloma, ovarian cancer, polycythemia vera, carcinoma of the thyroid gland, and carcinoma of the breast. Radiation-induced malignancies usually occur in the field of irradiation. For example, radiotherapy for carcinoma of the cervix may be followed by the development of carcinomas of the endometrium, vagina, urinary bladder, colon , rectum, and anus, as well as mesotheliomas of the peritoneum and osteosarcomas of the pelvis. Tumors developing in an irradiated field include a substantial number of soft tissue sarcomas or osteosarcomas. There is a 20-fold increase of second cancers following treatment of childhood malignancies, mostly sarcomas of bone and soft tissues, but including leukemia, and carcinomas of the thyroid gland, skin, and breast. The latent period between radiotherapy and the appearance of a second cancer ranges from 2 years to several decades, often being 10-15 years. With chemotherapy the mean latent period is shorter, approximately 4 years. The mechanism of oncogenesis by radiotherapy or chemotherapy is poorly understood and probably involves a complex interplay of somatic mutation, co-oncogenic effects, depression of host immunity, stimulation of cellular proliferation, and genetic susceptibility. The danger of developing second malignancies following radiotherapy or chemotherapy emphasizes the need for lifelong follow-up of patients given these forms of treatment; particularly in those with a long life expectancy as are those treated for childhood neoplasms.

A case of inversion of the uterus following abortion is reported. The 35-year old patient, admitted October 10, 1978 to the Medical College and Hospitals in Calcutta, India was referred by a private practitioner with a history of amenorrhea for 16 weeks, bleeding for 3 days, expulsion of the fetus 3 days earlier, and something coming down per vaginum for 2 days. The patient was para 4+0 (all full term normal deliveries) and home delivery for the last child 1 1/2 years earlier. She had a history of regular menstrual periods. Her general condition was poor. The examination revealed a gangrenous mass coming out of the vulva with a very offensive smell. There was a raw surface on which placenta like tissue was attached. No active bleeding was seen. Fundus and cervix of the uterus could not be felt. On rectal examination the uterus could not be felt, a cup-like depression was felt at the site of the uterus. The provision diagnosis was inversion of uterus following abortion. Treatment was started with sedatives and antibiotics, and arrangements were made for a blood transfusion. The vaginal mass was covered with glycerine and acriflavine gauze, and a hysterectomy was decided upon after improvement of her general condition and control of the infection. On October 14th, the patient was placed in knee chest position and posterior vaginal wall was retracted with Sims' speculum when the inverted lump was spontaneously reduced within the vagina. The inverted uterus was felt in the region of the vaginal vault. Glycerine acriflavine pack was given which was taken out and repack was given daily until the operation. The hysterectomy was performed on October 23rd. The abdomen was opened up by a transverse incision and the pelvis was explored. In the region of the uterus a cup-shaped depression was noted. Tubes and ovaries of both sides were seen hanging laterally from the cupped area. The left tube was found congested and thickened. Reduction of uterus was done by making a vertical incision over the posterior rim of cervix and with pressure from below by a sponge holding forceps by an assistant. The uterus was found to be just bulky. A total hysterectomy was performed. The postoperative period was uneventful. The histopathological report showed chorionic villi with degeneration and necrosis. In the case reported, etiology of inversion of the uterus following an abortion may be because of a lack of muscle tone along with traction of placental tissue by a traditional midwife.

The clinical, virological and serological responses of seven female donkeys (Equus asinus) to inoculation with the KY-84 strain of equine arteritis virus (EAV), a strain that causes moderate to severe clinical signs in horses, was investigated. In the donkeys, the only clinical signs observed were fever (mainly 3-9 days after inoculation), mild depression in four animals, and a slight nasal or ocular discharge in three. All of the donkeys became infected with EAV as shown by recovery of the virus for periods of up to 14 days from the nasopharynx and buffy coat and, in three out of four donkeys sampled, from the cervix or vagina. Virus replication in the donkey appeared to mirror that previously described for the horse. The donkeys had "sero-converted" to EAV by the 10th day after inoculation. Additional studies are needed to obtain a better understanding of the pathogenesis of EAV in donkeys.

The incidence of cervical cancer is known to decrease with the Westernization of life style. The purpose of this study was to see whether or not the practice of pseudopregnancy conditioning (psp-c) and/or non-Western (rice-rich or rice-and salt-rich) diet conditioning (dt-c) in female mice can reproduce a specific steroidal disorder that is associated with cervical cancer--general depression of androgen and partial depression of corticoid in urine. The effects of psp-c and dt-c were assessed by estimating various steroids in urine and plasma of mice, as collected at the terminal stage of the experiment. Macroscopic and microscopic changes of genital organs were also investigated by dissection. Results obtained are as follows: 1) the practice of psp-c and/or dt-c induced a total of 13 deciduomas in the vagina, the cervix and the corpus of experimental mice. No deciduoma was found in virgin mice fed a standard (Western-style) diet. 2) The combination of 2 reproductive markers (psp-c and non-psp-c) and 3 dietary markers (standard diet, rice-rich diet and rice- and salt-rich diet) produced a variety of changes in the excretions of all androgens, progestins and corticoids in the urine of mice for each of 5 experimental groups. A common steroidal trait was extracted from the urinary steroid data of 5 experimental groups--general depression of androgens, progestins and majority corticoids relative to tetrahydrocortisol. By the collation test, the combination of psp-c and rice- and salt-rich diet conditioning was found to be the best choice for reproducing the urinary steroid changes specific for cervical cancer. 3) The results of plasma steroid analysis, though not incompatible with the urinary steroid data, were fragmentary and not very informative in this study. The observed resistance of mice to carcinogenic insults together with the possible role of deciduoma in cervical carcinogenesis is discussed in the light of relevant information including the anti-carcinogenic action of vitamin C.
