An automated immunoprecipitin system has been utilized to quantitate the concentration of 10 specific proteins in the plasma of man. Values obtained by this technique are in agreement with the published concentrations for these specific plasma proteins. This technique was utilized to determine the sequential change s in 10 individual plasma proteins of volunteers exposed to Salmonella typhi. In those volunteers who developed typical
typhoid fever
, plasma concentrations of the acute phase proteins, alpha1-acid glycoprotein, alpha1-antitrypsin, and haptoglobin, as well as C3 complement were significantly increased with the onset of febrile illness. In contrast, the concentration of plasma albumin and tranferrin were depressed while plasma IgM became elevated during early convalescence from this infection. No significant changes were observed in the plasma concentrations of alpha2-macroglobulin, IgG, or IgA. In the exposed volunteers who did not become ill, the only significant change was a brief
depression
of alpha1-antitrypsin. During
typhoid fever
the patterns of change for individual plasma acute-phase globulins were different from those reported for patients with hepatitis, myocaridal infarction, or surgery.

It was shown that administration in the course of one week, before or after a single use of killed or chemical
typhoid
vaccine of dibiomycin, biomycin, or biomycin in combination with erythromycin in comparatively high doses produced no negative effect of the production of typhus antibodies and the intensity of antitoxic immunity in albino mice. The same antibiotics failed to influence the antibody formation in guinea pigs if they produced no toxic effect on the animals; but in case of development of toxic phenomena connected with the administration of the mentioned antibiotics a strong
depression
of antibody production was observed in guinea pigs.

There were phasic changes of immune response in rats and mice with adjuvant disease: an increase of antibody production on the 7th day after the injection of complete Freund's adjuvant and its
depression
on the 21st day; there was also observed suppression of normal antibody production to O- and Vi-antigens of
typhoid
bacilli.

The effect of
typhoid
bacterial polysaccharide on the primary and secondary immune response to SRBS was studied. The polysaccharide was shown to have both stimulating and depressive effect on the population of antibody-producing cells. This effect depended on the time and the number of polysaccharide injections. Thus, a single polysaccharide injection made on the day preceding immunization resulted in the maximum stimulation in the system of IgM- and IgG-producing cells, while the maximum
depression
of these cells could be observed after 2 polysaccharide injections: on the day preceding immunization and on the day of immunization. In the secondary immune response considerable stimulation of the populations of antibody-producing cells was observed after polysaccharide injections made on days 2 and 3 after reimmunization.

Similar changes in the nonspecific immunity indices were noted in experiments on rats and guinea pigs with adjuvant disease:
depression
of blood serum bactericidal activity and increase of the lysozyme and complement level, as well as inhibition of the phagocytic activity of leukocytes and production of normal antibodies to O- and Vi-antigens of
typhoid
bacilli in rats. Changes in disturbances of natural immunity factors in adjuvant disease correlated with the severity of the process in both animal species.

The skin response to phytohemagglutinin was investigated as an index of cellular immunity in group of 50 children made up of 11 with thypoid fever, 13 with bronchial asthma type I, 15 with Hodgkin's disease and 11 controls. A mild but statistically significant (p less than 0.005)
depression
was found in the skin response of patients with
typhoid fever
and moderate in patients with Hodgkin's disease. In patients with bronchial asthma, it was similar to control cases; thus, in the future, finer parameters are considered necessary in these cases to investigate cellular immunity.

The authors studied the ultrastructure of synapses of the dog sensory-motor cortex in response to intravenous injection of
typhoid
endotoxin. The most pronounced shifts were noted in the dendrite neuron apparatus. Activation in the cortical structures was connected with an increase in the number of functioning synapses and reconstruction of interneuronal connections. There was an increase in the length of the contacting membranes, and of the quantity of synaptic vesicles; the content of dense core vesicles is released.
Depression
is associated with dystrophic changes in the synapses.

Intensive chemotherapy in patients with leukemia produced immunosuppression. The level of immunocompetence correlates with prognosis. The immunological function of 29 children with acute lymphoblastic leukemia (ALL) in complete remission and on 2 different maintenance therapies was evaluated and compared with 16 normal children (Group A). Sixteen children (Group B) with ALL received 6 mercaptopurine (6MP) daily and methotrexate (MTX) twice a week, and 13 children (Group C) received 6MP and MTX weekly for maintenance. There was
depression
of both cellular immunity, measured by the number of T cells and skin tests, and humoral immunity, measured by number of B cells, primary antibody production to
typhoid
vaccine, and levels of immunoglobulins. However, continuous maintenance therapy (Group B) produced significantly more severe immunosuppression of cellular immunity than the intermittent therapy (Group C). Humoral immunity was equally depressed in both groups of leukemia patients, but was less altered than cellular immunity. Concomitantly, patients with intermittent maintenance chemotherapy had less hematologic
depression
, fewer episodes of infection, and fewer died in complete remission. Patients of both groups with higher levels of immunocompetence had better prognosis with longer duration of complete remission than patients with severe immunosuppression. Out of 6 patients with "favorable immunocompetence" only 1 relapsed at 7 months and the other 5 remain in complete remission from 8 to 31 months. Among 23 leukemic patients with "unfavorable immunocompetence," 15 relapsed and 8 remain in complete remission from 9 to 26 months.

The results of medical examinations carried out on 212 missionary personnel from one missionary society returning on leave to the UK are presented. The great majority of missionaries worked in developing countries. They served in 27 countries altogether and for a total of 488 person years. The commonest illnesses reported overseas were malaria (87.3 per 1000 person years at risk), diarrhoea (63.5), anxiety (63.5),
depression
(41.0) and giardiasis (38.9). More illnesses were reported from West Africa (698 per 1000 person years at risk) than from any other region. Ten people (4.7%) were repatriated for health reasons and 10 relatives also returned as a consequence. Sixty per cent of those returning did so because of psychiatric illness. The highest rates of immunization achieved were for yellow fever (100% of those travelling to affected countries), tetanus (93%), polio (85%),
typhoid
(71%) and tuberculosis (53%). The results of urinalysis (100% of adults), full blood counts (78% of adults) and stool tests (74% of all people) are reported. The study shows that the history and psychiatric examination are an important part of the medical examination of people returning from overseas. Physical examination and urinalysis did not contribute much information, although the full blood count and absolute eosinophil count were useful tests.

Disturbances of blood coagulation were studied in 32 consecutive patients with
typhoid fever
on their admission to hospital. Estimations of prothrombin time, activated partial thromboplastin time, fibrinogen, fibrin degradation products (FDPs), factors VII, VIII and XII, alpha I antitrypsin, plasminogen, CI esterase inhibitor, and platelet counts were performed as well as liver function tests and blood counts. Five patients had laboratory evidence of disseminated intravascular coagulation (DIC) and two had a generalised bleeding disorder which in the other three was inapparent. The platelet count in the group as a whole was low (P less than 0.05) and the FDPs in most cases were mildly elevated. The pre-kallikrein values were depressed in three of the five with DIC, whereas factor XII was not reduced. These results indicate that bleeding disorders in
typhoid fever
are uncommon. The
depression
of pre-kallikrein indicates that the DIC is probably triggered by activation of the intrinsic coagulation pathway. Most patients had lymphopenia and monocytopenia but only two had neutropenia.
