without a subsequent stimulation phase was obtained in mice by raising the ET dose. The reasons of the discrepancy between these findings and those of Hanna and Watson (1965b) are discussed. Pyrogenic tolerance to ET is not accompanied by accelerated carbon clearance and is not impaired by RES blockade. A possible mechanism of ET tolerance is suggested.

. Focus was given to current intellectual and personality attributes as well as a broad spectrum of background factors in social, familial, educational, and sexual history. One-tailed t-tests were computed on the myriad permutations of patient group and the multiple derived scores from the personality and intellectual assessments. The results show no significant differences between the patients having their first infection of VD and the patients with repeat VD; however, when these 2 groups are pooled and compared with the controls, the VD patients are significantly higher on the
depression
, schizophrenia, psychasthenia, hysteria, and psychopathic scales. A similar pattern exists on reasoning ability and internal versus external locus of control. The VD patients are seen to perceive themselves as much more externally controlled than do the controls. It is concluded that the VD patients might profit from psychologic treatment; such treatment resources might aid not only in the control of repeat infections but in the general social integration of the patients.

The single most important risk factor for adolescent suicide is a previous attempt. It is unclear if physicians can identify adolescents who have attempted suicide. Our objectives were to determine the prevalence of previous attempts in an adolescent clinic population, if physicians identify attempters, and if attempters demonstrate persistent distress. Of 332 patients aged 12-19 years attending a medical clinic, 48 (14.5%) reported a previous attempt. The physician's visit note documented that only 8 (16.7%) of the 48 attempters were asked about suicidal behavior. Attempters were more likely than nonattempters (p less than 0.05) to be female; to come to clinic without a guardian; to give a chief complaint pertaining to
sexually transmitted disease
, obstetrics-gynecology, or mental health; and to report previous mental health care. Attempters had poorer mean scores (p less than 0.05) on validated subscales for family relationships, social competence,
depression
, unpopularity, somatic complaints, thought disorders, delinquency, aggression, and identity. We conclude that suicide attempts are common among adolescent clinic patients, that physicians may not recognize attempters, and that attempters remain distressed and in need of care. Physicians who see adolescents for routine medical problems must consider the potential for self-destructive behavior, regardless of the presenting complaint.

Although the availability of antibiotics has reduced the general incidence of donovanosis, the disease remains prevalent in India, Papua New Guinea, parts of southern Africa, and the West Indies. Clinical variants of this
sexually transmitted disease
include ulcerogranulomatous, hyertrophic, necrotic, and cicatrical. Described here is the case of a 20-year-old Zulu female who developed donovanosis with hypertrophic and ulcerogranulomatous lesions during the third trimester of pregnancy. When the patient presented at 36 weeks, she reported a 10-week history of vaginal ulceration and a 6-week history of swelling in the inguinal region. The ulcerative lesion, located at the introitus, had raised edges 2 cm in diameter, while the dry keloidal-like lesion 2 x 4 cm was present in the inguinal region. This is the first reported case of a combination of two variants of donovanosis in one individual. Histologic examination revealed mildly acanthotic surface squamous epithelium with underlying granulation tissue containing plasma cells, lymphocytes, pockets of neutrophils, and scattered histiocytic cells. Donovan bodies with the typical safety pin appearance were demonstrated by a Giemsa stained tissue smear. Treatment with 500 mg of erythromycin twice daily for 2 weeks resolved the disease. Both the incidence and severity of donovanosis appear to increase during late pregnancy, presumably because of
depression
of lymphocyte proliferative responses.

This study was conducted to describe the difficulties perceived by general practitioners concerning 24 common clinical problems and to compare their perceptions with those of faculty members in family medicine. A random sample of 467 general practitioners and all 182 faculty members in family medicine in Quebec were sent one of four open-ended questionnaires, each of which dealt with six clinical problems; 214 general practitioners and 114 faculty members participated. A total of 5111 difficulties were reported; the number reported by each subject varied from 0 to 13 (mean 2.6 [standard deviation 2.09]) per problem. The problems that generated the most difficulties were

, confusion in the elderly, chronic back pain, loss of autonomy in the elderly and

The most frequent difficulties were with the patient's noncompliance with treatment, clinical diagnosis, failure of a specific treatment, inadequate health care resources and the physician's own emotional reactions. The difficulties for each problem were the same in the two groups 70% of the time. Physician's perceptions of their difficulties can be useful in the planning of initial training and continuing medical education.

History of diagnosed illnesses, medical symptoms, and reproductive outcomes and their relation to combat intensity and herbicide exposure were studied, via a mailed questionnaire, among 6810 American Legionnaires who served during the Vietnam War (42% in Southeast Asia, 58% elsewhere). Heart disease,
venereal disease
, and benign fatty tumors were reported significantly more often by Vietnam veterans than by controls. Combat intensity was significantly dosage-related to history of high blood pressure, ulcers, arthritis and rheumatism, genito-urinary problems, nervous system disease, major injury, hepatitis, and benign fatty tumors. Agent Orange exposure was significantly dosage-related to history of benign fatty tumors, adult acne, skin rash with blisters, and increased sensitivity of eyes to light. Rates of the latter two conditions and of change in skin color were especially elevated in men whose military occupations involved direct handling of herbicides. Five "symptom complex" scales were constructed via factor analysis to measure degrees of feeling faint, fatigue or physical
depression
, body aches, colds, and skin irritation. Means of all five scales were significantly higher in Vietnam veterans compared to controls, and in herbicide handlers compared to nonhandlers. Both combat and Agent Orange exposure were significant, independent predictors of each of the five scales. Neither combat nor Agent Orange exposure was associated with difficulty in conception, time to conception of first child, or to birthweight or sex ratio of offspring, but maternal smoking was strongly related to reduced birthweight. The percentage of spouses' pregnancies which resulted in miscarriages was significantly higher for Vietnam veterans than controls (7.6% vs 5.5%, P less than 0.001). Logistic regression analysis showed that Agent Orange exposure and maternal smoking were both independently and significantly associated with miscarriage rates in a dose-related manner.

A health needs assessment was developed to facilitate implementation of a comprehensive school-based adolescent clinic. Students were asked about their willingness to use a school-based clinic for certain health and emotional problems. 600 students in grades 9-12 completed anonymous self-administered questionnaires. 28% of respondents reported recently feeling depressed; 12% reported a prior suicide attempt. 25% of students felt they were overweight. Of 56% who had experienced intercourse at least once, only 1/3 had ever used birth control. 21% smoked cigarettes and 27% marijuana, 24% used other drugs, and 38% used alcohol. Students who reported
depression
and past suicide attempts were significantly (p 0.001) more willing to use the clinic for counseling needs than students not so reporting. Those with perceived weight problems reported more willingness to use a school clinic for nutrition information than those who did not feel overweight. Currently sexually active students were also more willing (p 0.001) than nonsexually active students to use the clinic for sexuality information and
sexually transmitted disease
screens. Respondents who used drugs, alcohol, or cigarettes, however, were no more willing than non-substance-using peers to use clinic services for relevant health information. As a group, adolescents have some unique age-related health problems. Because adolescence represents a time when health behaviors and attitudes are being formed, it is a crucial time for health intervention. These findings suggest that high school students with unmet health needs are willing to receive health care and education at a school-based clinic.

The purpose of this study was to determine what issues teenagers want discussed or covered when they visit primary care physicians and to assess to what extent such discussion takes place. A questionnaire was administered to 1,564 students aged between 13 and 18 years in six high schools. Mean participant age was 15.3 years; 801 were male and 763 were female. Questions were drawn from both physical and psychosocial aspects of teenage life. The teenagers answered as to whether they would like to discuss the suggested topics on visits to physicians, and whether in fact such a discussion had taken place. The three topics of most interest to teenagers were physical fitness, nutrition, and growth. Teenagers wanted to discuss these topics in over 80 percent of the visits, and they indicated that actual discussion took place in just under 50 percent of the visits. Discussion of

was desired by teenagers 70 percent of the time, with a discussion rate of only 18 percent; contraception at 66 percent with a physician discussion rate of only 22 percent. If physicians discuss exercise, nutrition, and growth with teenage patients, in over 80 percent of cases they will be providing the patient with valued information. This initial dialogue will establish a base of communication that may allow for the discussion of issues teenagers often find more difficult (such as contraception,

The results of this preliminary experiment suggest that chronic ethanol exposure might affect either the synthesis of imipramine binding sites in megakaryocytes or the structural environment of imipramine binding sites in the platelet membrane. Two hundred and twenty-six consecutive patients were included in this study, 191 men and 35 women, aged 28-74 years (mean 56). Patients were classified into two groups. Group I consisted of 165 patients with either no coronary disease, single or double vessel disease. Group II included 61 patients with triple vessel or left main stem stenosis. It was found that the sigma

/delta HR index at 25 mm. beat-1 X min. 10(2) provided the best separation between Groups I and II patients. All but 3 of the 61 patients in Group II had a sigma

/delta HR index greater than 25. In contrast, all but 4 of the 165 patients in Group I had an index less than 25. The sensitivity, specificity and positive predictive value in the identification of Group II patients by using the index were 95%, 98% and 94% respectively. Utilization of ST segment
