Most extrapyramidal side effects of neuroleptics are easily recognized and cause little problem in diagnosis and treatment. The exceptions are those which mimic psychopathology: akathesia and akinesia. Akathesia resembles anxiety. Akinesia is similar to
depression
,
residual schizophrenia
, and demoralization. Differential diagnosis may require interruption of the neuroleptic. Whether neuroleptics can cause
depression
not associated with akinesia has never been established. The first obligation of the clinician is to be sure that the depressive symptoms are not concomitants of akinesia, before treating with antidepressants which may not be the appropriate treatment. There is no convincing evidence that one neuroleptic drug is more likely to cause akinesia than another, although this had not been studied extensively using a broad definition of akinesia.

Anxiety and
depression
were assessed with self-rating and observer-rating scales on admission and at discharge in 225 inpatients with hebephrenic, catatonic, paranoid or
residual schizophrenia
. The control subjects were 104 patients with endogenous depression and 63 with anxiety neurosis. The results were also compared with findings for a reference group of 2493 inpatients with a wide range of psychiatric disorders and a representative sample of the general population consisting of 1952 persons. On admission the group of schizophrenic patients was markedly more anxious than the group of healthy persons, slightly less anxious than the psychiatric reference group and much less anxious than the group with endogenous depression and anxiety neurosis. Anxiety correlated significantly with acute paranoid symptoms, whereas
depression
dominated just prior to discharge, when the acute symptoms had subsided. This indicates that anxiety is a consequence of the underlying schizophrenic disorder and also that the psychotic symptoms may not be a defense against anxiety.

The authors administered the Carroll rating scale for
depression
to 100 patients with
residual schizophrenia
and found that 41 were also depressed. Of these 41 patients, 23 were given a dexamethasone suppression test; 7 (31%) failed to suppress cortisol. A matched control group, i.e., 23 patients with
residual schizophrenia
but without
depression
, suppressed cortisol normally after taking dexamethasone. The authors found no clinical differences between depressed patients with normal DST results and depressed patients with abnormal DST results.

The central serotonergic system is implicated differentially in the pathogenesis of
depression
and schizophrenia. The dorsal raphe nucleus (DRN) is the main source of serotonergic innervation of forebrain limbic structures disturbed in both disorders. The study was carried out on paraffin-embedded brains from 27 depressed (15 major depressive disorder, MDD and 12 bipolar disorder, BD) and 17 schizophrenia (9 residual and 8 paranoid) patients and 28 matched controls without mental disorders. The transcriptional activity of ribosomal DNA (rDNA) in DRN neurons was evaluated by the AgNOR silver staining method. A significant effect of diagnosis on rDNA activity was found in the cumulative analysis of all DRN subnuclei. Further analysis revealed an increase in this activity in residual (but not paranoid) schizophrenia compared to depressed (both MDD and BD) patients. The effect was most probably neither confounded by suicide nor related to antidepressant and antipsychotic medication. Our findings suggest that increased activity of rDNA in DRN neurons is a distinct phenomenon in
residual schizophrenia
, related presumably to differentially disturbed inputs to the DRN and/or their local transformation compared with depressive episodes in patients with affective disorders.
