The presence of polymorphonuclear neutrophils (PMN) is a constant feature of Psoriatic epidermis. The reasons for this migratory activity are not clear and may be related to the presence of chemotaxigens or chemoattractants (circulating immune complexes, complement split products, or arachidonic acid metabolites) in serum and/or epidermis. On the other hand, functional abnormalities of psoriatic PMN may play a significant role as well. PMN chemotaxis, e.g., against increased chemoattractants has been found to be significantly increased in
psoriasis
. On the basis of these findings it may be suggested, that potent chemotaxigens or chemoattractants together with hyperactive PMN by the liberation of lysosomal enzymes cause a self-augmented increase of chemotactic factors. Recent experiments using PMN and sera from patients with
psoriasis
appear to support this concept. Modulating influences of immunoglobulins, specifically immunoglobulin A have recently been discovered. In chronic stationary
psoriasis
chemotactic activity of PMN is strongly inhibited which appears to be due to increased levels of IgA. Further, in inflammatory dermatoses with predominantly neutrophils similar IgA-related chemotaxis
depression
has been found. This appears to be a newly found modulation of PMN-dependent inflammatory reactions in the skin.

We describe the case of a patient with
psoriasis
who developed severe colitis, with toxic megacolon, after a course of treatment with methotrexate. The duration of her colitis, which was eventually resolved by using conservative measures, was far greater than that of other drug-related complications, which included bone marrow
depression
, jaundice, and renal failure. The literature pertaining to this rare manifestation of methotrexate toxicity is reviewed. A prolonged illness and relative sparing of the distal colon appear to be characteristic of this form of colitis.

The 30-item General Health Questionnaire (GHQ) (Goldberg, 1972) was administered to 196 consecutive new dermatology out-patients and 40 consecutive admissions to dermatology beds. Thirty per cent of the out-patients and 60 per cent of the in-patients obtained high scores, while half the high scorers in each group scored high on the Wakefield Self-Assessment
Depression
Scale (Snaith et al, 1971). These findings suggest that dermatology out-patients have a higher prevalence of psychiatric disorder than the general population, and dermatology in-patients a higher prevalence than general medical in-patients. High GHQ scores were associated with (a) diagnoses of acne, eczema,
psoriasis
or alopecia; with (b) extensive lesions on exposed parts of the body; and with (c) the use of high potency topical steroid. We indicate other areas that might be profitably explored in a full-scale study.

Measures of
depression
and obsessionality were made in patients with dysmorphophobia,
psoriasis
and control subjects. The
depression
score distinguished all three groups. Patients with dysmorphophobia had significantly higher scores compared to the patients with
psoriasis
, who in turn had significantly higher scores compared to a control group. Patients with dysmorphophobia and
psoriasis
had equally elevated obsessional symptom scores compared to controls. It is unlikely, therefore, that dysmorphophobic symptoms are due to an obsessional state.
Depression
, however, was found to be an important clinical feature of dysmorphophobia.

This paper describes our experience with the use of razoxane (ICRF 159), a new anti-mitotic drug, in the treatment of thirty-six patients with severe
psoriasis
. The drug has proven highly effective both in cutaneous and in arthropathic disease. The drug is well tolerated by patients and appears to be free of hepatotoxicity. However, it causes
depression
of the neutrophil leukocyte count, an effect which is entirely dose-dependent; the hazard to patients can be minimized by careful supervision of therapy.

A consecutive unselected series of 60 in-patients suffering from dermatologic disorders (
psoriasis
, chronic urticaria and fungal infections of the skin) was examined. Twenty patients with each illness were included. Stressful life events immediately before illness onset, levels of psychological distress, and alexithymic traits were investigated. Patients with
psoriasis
and chronic urticaria were exposed to stressful life situations before disease onset and suffered from psychological distress (anxiety,
depression
, inadequacy) significantly more than those with fungal infections. Implications for psychosomatic research and treatment are discussed.

Vitamin D3-derivatives are known to be effective in differentiation and proliferation of epidermal cells. However, under certain circumstances, they also may show a hypercalcamic activity which can be a serious limitation in their use for dermatological application. For keeping small the negative side effects of vitamin D3-derivatives and for increasing the drug concentration in the skin we investigated their incorporation in liposomes to optimize their use for
psoriasis
treatment. The incorporation of vitamin D3-derivatives in liposomes of different lipid composition was studied by HPLC, DSC and freeze-fracture electron microscopy. Incorporation rates of more than 80% of the offered drug were found with significant variations related to the number of hydroxyl-groups in the A-ring, opened or closed B-ring, and some modifications in the side chain of the steroid-molecules. In general, the incorporation rates in egg-PC liposomes have been of about 3% up to 10% higher than in DMPC liposomes. Results, obtained by DSC and freeze-fracture electron microscopy, show a
depression
of the phase transition of DMPC bilayers by incorporation of vitamin D3 already at a concentration of 10 mol%. These results support the idea that vitamin D3 and its analoga investigated are incorporated into the lipid bilayer modifying there the lipid-lipid interactions.

Lithium is frequently used in the management of several psychiatric disorders including acute mania, bipolar (manic-depressive) disorder, and recurrent
depression
. We describe a patient in whom hidradenitis suppurativa developed while the patient was receiving lithium. The cutaneous side effects of lithium are reviewed. The most frequent are
psoriasis
, acneiform lesions, folliculitis, alopecia, and a maculopapular/macular eruption. Many of these side effects respond less readily to conventional therapy while the patient is receiving lithium.

Interactions between polymorphonuclear leukocytes (PMNL) and monocytes are changed in inflammatory diseases of the skin; these changes manifest by enhanced stimulation of PMNL migration under the effect of monocytes in
psoriasis
and by
depression
of this migration in eczema. In
psoriasis
HLA-DR-antigens and endogenic monocyte prostaglandins participate in this modulation of cooperative interactions between phagocytic cells, whereas in eczema PMNL superoxide anion takes part in this process. These data may be useful in the diagnosis of inflammatory diseases of the skin and disclose new aspects in the pathogenesis of
psoriasis
and eczema connected with phagocytic system cells.

We studied 179 psoriatic patients by semistructured colloquia and psychometric tests and determined their cutaneous psycho-neurophysiological profiles by biofeedback methods. The Paykel scale for stressful events showed that 72% of psoriatics had experienced significant stressful events about one month before the appearance of the
psoriasis
. The Zung test for anxiety and
depression
showed a high level of anxiety in the psoriatic patients. 64% of the patients who were treated by BFBtraining had a decrease in their PASI index for severity and the extent of the disease and also fewer recurrences at the one-year follow-up. The results of the World Experience Inventory indicated difficulties related to body image and to relationships with others.
Psoriasis
influenced the sexuality of the patients. It is always difficult when one is afflicted by ill health to enjoy life and the general scores of SWL (Satisfaction with Life), were significantly lower than those of a control group.
