The efficacy of thioguanine in the treatment of severe cases of
psoriasis
is demonstrated. This treatment is valuable in selected cases of severe
psoriasis
in whom other treatment is ineffective or impossible due to side effects. The effect of thioguanine on
psoriasis
lesions appears to run parallel with
depression
of the bone marrow. The bone marrow toxicity has to be considered. Patients previously treated with methotrexate are very sensitive to thioguanine and close follow up is mandatory with adjustment of the thioguanine dose according to blood white cell and thrombocyte levels.

Comprehensive examinations of dermal morphology and function, carried out in 50 patients suffering from
psoriasis
, have revealed manifest hypoxia,
depression
of the intramitochondrial energy production, reduction of the cellular synthetic potency, increased protein catabolism, impaired transcapillary transport and vascular and tissue permeability, immediate and delayed type hypersensitivity local immune reactions. Basing on these findings, the authors have developed a new effective method for multiple modality treatment of
psoriasis
patients. The new scheme includes, besides the routine modalities (sedative and lipotropic drug, vitamins, UV irradiation, diets, ointments), subcutaneous injections of oxygen into the large foci (200 ml daily, 12-18 injections per course, alternating the injection sites) and oral intake of copper sulfate and reduced iron (3 times daily after meals for 20-30 days). The study has involved 112 patients with
psoriasis
(61 patients in the test group and 51 in the reference one). Clinical and morphologic examinations have demonstrated the advantages of the new method: clinical remission has occurred 1.8 times more often, the length of treatment has been cut by 7 days, an earlier and more complete recovery of the morphofunctional characteristics of the dermis has been achieved.

This research sought to quantify the handicapping effect of skin conditions in a far more rigorous way than had previously been attempted. One hundred people who had attended a hospital outpatient clinic during a specified period for treatment of their acne,
psoriasis
or eczema were interviewed in their homes. A comprehensive and structured interview schedule was used and interviewees were encouraged to talk at length about the impact that their skin conditions had had on their lives. Detailed data were collected that show the serious effect that these diseases can have in several domains. The findings record not only the physical discomfort and inconvenience sufferers may meet but also the consequences for their personal and social life and daily functioning. There is evidence from interviewees' employment experiences of limited opportunities, and functional and interpersonal difficulties in the workplace. 64% of people said that their skin disease affected their socio-economic activity. The extent to which sufferers experienced embarrassment, anxiety, a lack of confidence and
depression
is documented. 40% of people felt that their social life was affected and there was evidence of particular stresses and demands in personal relationships. The social impact of skin disease is discussed.

Among 82 inpatients with

, 67% (55 patients) reported moderate or severe pruritus. The degree of depressive psychopathology discriminated between the mild, moderate, and severe pruritus groups at admission. Prospectively, the change in
depression
scores correlated with the change in pruritus pretreatment to posttreatment. Pruritus severity did not correlate significantly with stress due to life events, age at onset, ethyl duration of
psoriasis
, age, sex, marital status, and average reported daily alcohol (ethanol) consumption. Among the objective dermatologic measures, including extent and severity of lesions, only the degree of perilesional irritation discriminated between the mild, moderate, and severe pruritus groups at admission, and prospectively, change in this measure correlated with the change in pruritus pretreatment to posttreatment. This finding was most likely related to the self-excoriation associated with pruritus. Intrapsychic factors (eg, the severity of
depression
) rather than external psychosocial or well-defined dermatologic factors, were the most significant correlates of pruritus in

This paper reports studies of patients with the following disorders: peptic ulcer, hypertension, bronchial asthma, irritable bowel syndrome (IBS), ulcerative colitis (UC), urticaria,
psoriasis
and alopecia. The investigations focused on dysthymic states, measured by Foulds' Scale of Anxiety and
Depression
(SAD) and--except for the first three disorders--the Present State Examination (PSE). On the SAD, all the above groups scored significantly higher than somatically ill controls in anxiety, and all except ulcer patients scored significantly higher in
depression
. The PSE designated more than half of these patients as cases, except in the
psoriasis
group. Most patients were assigned to the PSE syndromes of anxiety states or neurotic depression, with the former being more common in UC and urticaria, and the latter more common in IBS, alopecia and rheumatoid arthritis. The variation within skin diseases and within gastrointestinal diseases suggests that neurotic symptoms are typical of each disease rather than of the system involved in the disturbance.

The chemotactic activity of purified monocytes and neutrophils was studied in twenty-two psoriatic patients prior to the initiation of psoralens plus ultraviolet A (PUVA) or anthralin therapy and during the treatment. Patients with untreated, active
psoriasis
had increased chemotactic response of their monocytes and neutrophils to N-formyl-methionyl-leucyl-phenylalanine. Plasma from untreated
psoriasis
patients had an enhanced chemotactic activity toward neutrophils from persons without
psoriasis
. Treatment with PUVA or anthralin resulted in a progressive clearing of psoriatic lesions and in a significant
depression
of leukocyte chemotaxis 2 weeks after the start of the respective therapy. The chemotactic activity of both monocytes and neutrophils was normalized after 4 weeks of anthralin therapy and after 6 weeks of PUVA treatment. The chemotaxis-enhancing properties of plasma from
psoriasis
patients returned to normal values already after 2 weeks of treatment.

Three experimental groups of patients with urticaria,
psoriasis
and alopecia were compared for hostile personality characteristics, states of anxiety and
depression
, neurotic syndromes and stress with a control group of patients with other skin diseases. The patients from each experimental group were found to be less dominant, more intropunitive, more extrapunitive and more neurotic than the control group. The following neurotic syndromes differentiated the experimental groups: non-specific anxiety states in urticaria patients; neurotic depression in the patients with alopecia, and a variety of neurotic syndromes in the
psoriasis
patients. All experimental groups scored significantly higher than the controls in stress experienced during the year preceding the onset or the exacerbation of the illness.

Polymorphonuclear leukocyte (PMN) and monocyte (MO) chemotaxis in methotrexate (MTX)-treated psoriatics was determined by a radioactive in vitro assay. MTX was administered orally once a week in a dose of 7.5 mg to 15 mg. Six untreated patients were examined before start and during the first week of the MTX treatment. A profound
depression
of the enhanced levels of PMN and MO chemotaxis was seen within the first 48 hours after intake of MTX, but the chemotactic activity recovered to initial values during the following days. Fifteen
psoriasis
patients in long-term treatment with MTX had normal chemotactic responses of both PMN's and MO's.

Somatostatin treatment was administered to 20 psoriatic patients according to the following protocol: Continuous infusion (250 micrograms/h) for at least 2 days followed either by short infusions (1 h) at 8 A.M. and 8 P.M. (12 cases) or by repeating the initial 2-day infusion (eight patients). Before treatment (day 0) and on day 6, biopsy specimens were taken for routine examination (12 patients) and for ultrastructure (seven patients). In vitro immunological studies were carried out on peripheral blood lymphocytes (six patients) on day 0 and day 8. In two patients, somatostatin was stopped because of serious side effects. Thus, clinical results were evaluated in 18 patients, on day 30. In ten of them no improvement whatsoever occurred, two had a partial clearing and an almost complete remission was achieved in six others. Ultrastructural studies showed, on day 6, enlargement of the intercellular spaces with deposits of granular material of glucidic composition, associated with features of cellular damage. Percentages of T and B cells were unmodified but a significant
depression
of mitogenic stimulation by PHA and ConA was clearly observed on day 8. Even if somatostatin treatment may have a beneficial effect in some patients it seems much less valid than other well-known therapies for
psoriasis
